Registration ChecklistWelcome to the Duxbury Public Schools. To streamline the registration process, please see instructions below.

Step 1: To begin the New Student Registration process please go to the following link to complete our online registration form. https://www.duxbury.k12.ma.us/domain/54. Once your online registration is complete, you will be provided a list of required documents to be brought to our Central Office. Central Office is located on the side entrance of the Chandler School at 93 Chandler Street. Please enter through the Coppens Field entrance.

Step 2: Verification of ResidenceSubmit proof of residency by providing one item from Group A and one from Group B.

Group A Group BCurrent Lease – Notarized Valid Massachusetts Driver’s License/ID with

current address (not a change of address sticker)

Property Deed Current Vehicle Registration

Utility bill – dated within the last 60 days Payroll Stub – within the last 60 days

Cable Bill Property Tax Bill - dated within past year

Cell Phone Bill

Required Documentation

_____ Original Birth Certificate (with raised seal)

_____ Up to date Physical Health and Immunization Record

_____ Copy of current IEP or 504 plan (if applicable)

_____ Custodial Agreement (if applicable official copy from Court)

_____ Student Record Request Form
