same) Potential Grounding issue.

Hubbell Canada LP is voluntarily notifying all customers that certain 50A Twist-Lock® or BRYANT® Locking, 3 Pole, 4 Wire Receptacles (with part #s and pictures attached hereto as Exhibit A) built by Hubbell Wiring Device Kellems from February 23, 2016 through August 31, 2016 (the “Affected Receptacles”), may have loose rivets that could, in certain applications, affect the reliability of the ground connection in such Affected Receptacles. If you or your customers received 50A Twist-Lock®, 3 Pole, 4 Wire Receptacles (or any applications that use same) prior to February 23, 2016, you may disregard this letter.

While there has been no reported injury, nor has there been reported equipment or property damage to date, Hubbell Canada LP is concerned that these Affected Receptacles may create a risk of shock under certain conditions. The manufacturing anomaly has been corrected and process improvements have been instituted to ensure that all products manufactured after August 31, 2016 are not affected.

The three-letter date codes of the Affected Receptacles start with either Lc, Le, Lg, Lj, Lk, Ll, or Lr. Date codes can be located either on the products themselves or on the associated packaging as shown in the photos below:

Important: Any package labeled with a red star indicates the product has been inspected and a return is not necessary.

Immediate action required by you:

1) Stop shipping and selling 50A Twist-Lock® or BRYANT®, 3 Pole, 4 Wire Receptacles (or any applications that use same) that have any of the date codes listed above. All uninstalled inventories of these Affected Receptacles should be immediately quarantined and you should contact Hubbell Canada Customer Service department at 1-800-465-7051 or via email at [email protected] for a Return Material Authorization (RMA) for the Affected Receptacles as instructed by Hubbell Canada's customer service team. Please include your account number, catalog number and return quantity for your RMA request.

2) Share this Product Advisory with your customers who purchased Affected Receptacles. Your customers will then need to contact Hubbell Canada's customer service department at 1-800-465-7051 or via email at [email protected] to follow item a) or b) below, as applicable.

a. Uninstalled Affected Receptacles. If your customer has any uninstalled Affected Receptacles with the date codes listed above in its possession, please advise them to return the Affected Receptacles through you for a Return Material Authorization (RMA).

b. Installed Affected Receptacles. For installed Affected Receptacles, we have an easy in-field solution that customers can implement involving the installation of a grounding clip. They can contact Hubbell Canada's customer service department via email at [email protected] or call 1-800-465-7051 for such clips. Please include the quantity of clips required and the mounting method (front or back mounted). Illustrations of the clip solution (for both front and back mounted Affected Receptacles) are below; more detailed installation instructions will be provided with the clips.
