Since pelvic lipomatosis was first described by Engels in 1959， approximately 90 cases have been

reported．

XtNre reviewed 88 reported cases， 84 males and 4 females， ranging in age from 9 to 80 years old．

Of the patients 24 were white， 48 were black and 16 were not specified or unknown． 59 of them under－

went biopsy of the pelvic mass．

32 were followed during 1 month to 13 years． Only 1 case died of uremia． Some of them were

successfu！ly treated by surgical procedures or conservative therapies． The others were uneventful

without any treatments．

緒 言

われわれは先にpelvic lipomatosisの1例を経験

し，欧米の文献を中心に考按を加えて本誌に発表し

た1），その後欧米では多数の症例が追加報告される

が，本邦では特に泌尿器科領域においては追加発表を

認めていない、このたびわれわれの症例を再び診察す

る機会を得たので，6年余後の尿路造影所見を紹介す

るとともに，その後集録しえた文献を中心に考察をお

こなう．

症 例

患者：65歳9ヵ月 男子（再来時）

初診：1972年12月1日

再来：1979年7．月1日

1972年12月1日（当時59歳）， 左下腹部痛を主訴と

して，周東総合病院を受診した．内科，整形外科お

よび泌尿器科での諸検査の結果，膀胱後腫瘍（脂肪

腫）を疑われて手術を受けた．骨盤内の脂肪組織は

capsulateされておらず，全摘除は不可能であった．

諸検査所見，手術所見よりpelvic lipomat3sisと診断

された．術後著変なく経過し，症状は消失した．退院

後より今日まで何ら訴えもなく家業に従事している．

現症：体格 頑健型．栄養良好．眼瞼結膜に貧血を

認めず，眼球結膜にも黄染を認めない．血圧，135／65

mmHg．頸部，胸部に異常を認めない．腹部は平担で

軟左下腹部，下腹部正中および鼠径部に手術療痕を

認める．肝，脾，腎はいずれも触知しない．外陰部

には異常を認めない。直腸内指診で前立腺は触知しえ

ない．排尿状態は，起床時は2時間に1回，就寝後は

排尿なし．しかし尿線はやや無力である．

検査所見：検査室検査はいずれも正常範囲内であ

る．

泌尿器科的レ線検査では，KUBで骨盤部がradio－

Iucentの傾向を示しているが，異常陰影は認められな

い（Fig． l A， B）． UCGでは，尿道は前立腺部で急に

前方に奢寵し，また延長している．膀胱はelongated

bladder（banana－shaped bladder）の状態を呈してい

る（Fig．2A）．正面像でも膀胱底部の挙上を認め，斜

位像とはほぼ同じ形態を示している（Fig．2B）． DIP

では5分像で両腎の腎杯は正常に造影され， 30分像

目は両側の腎孟，腎杯および尿管が造影されており，

尿管は全体的に拡張の傾向を示している．膀胱は逆涙

滴状（inverted tear drop shape）に造影きれている

（Fig． 3 A， B）．

これらの尿路造影所見を初診時のそれと比較してみ

ると，ほとんど変化を認めていない．

考 察

pelvic tumor”と題して本疾患が報告きれて以来2），

すでに20余年経過し約90例の報告がされている．われ

われはそのうちの88例を収録しえたので1～42），おもに

統計的考察をおこなう．このうち非常に稀とされてい

る女性症例も4例報告されている7・9・23）．Table lは収

録しえた症例を記している．年齢分布は9歳から80歳

までである．人種別に見ると黒人が48例（54．5％）で

過半数を占めており，白人24例（27．3％）がこれに続

いている．またpelvic massの組織学的検査が59例

（67．O％）についておこなわれているが，悪性所見を呈

したものは報告されていない．われわれの症例でも同

様の所見を呈していたD．組織学的検査は条件が許す

限り，他の疾患でも類似のレ線学的所見を呈するもの

があるので，それらを否定する意味でもおこなった方

が良いと考える43・4s）． Table 2は実際におこなわれた

外科的治療を挙げている．本疾患は良性の脂肪組織の

圧迫による尿路への影響が，主として症状に表われる

ので，術式としては尿路変更術が主流をなしている．

数人の報告者の中には，外科的処置を否定しているも

のも見られるが，閉塞症状が強く腎機能障害が進行

する症例には， これの適応と考える．Table 3は

follow－up cascs 32例について記した．期間は最低1

ヵ月から最高13年である．死亡症例はCraneらの報告

した30歳黒人男子1例のみであり36）， その予後は決

してmiserableなものではない． Carpenterは症例を

60歳で分けて，60歳以下の群では病状の進行傾向が強

いと延べているが21），確かにTabIe 2の手術適応と

なった年齢分布を見ると，そのことを物語っている．

しかしTable 3を見ると，その群でも外科的処置によ

り改善が見られている．Ya11aらの2症例もこの後

supravesi caldiversionが施行されている27）．また数名

は治療なしに変化なく経過している．

結 語

先に報告したpelvic lipomatosisの1例を再び診察

したので，その後の尿路造影所見を中心に報告すると

ともに，88例の文献的考察を加えた．pelvic lipoma－

tosisは正しく診断し，病状の進行が見られるような

ら，適切な処置を加えることにより良い結果が期待で

きる．

本論文の要旨は，1979年12月9日，第27回日本泌尿器科学

会山ロ地方会で発表した．
