① Ultrastructural changes.
• Margination or progressive loss of nuclearchromatin.
• Focal rupture of the nuclear membrane.
• Breakdown of the plasma membrane.
• Development of flocculent densities inmitochondria.

② Changes in the nucleus.
1. Pyknosis: condensation of nuclear chromatininto a dark blue (H&E), “Hyperchromatosis”.
2. Karyorrhexis: It means splitting orfragmentation of the nuclear chromatin intotiny basophilic granules as a consequence ofnuclear membrane rupture.
It is more prominent in nuclei of deadneutrophils in case of “abscesses and purulentexudates”.

③ Changes in cytoplasm staining1. Eosinophilia: denaturation of basic
amino groups results in increasedaffinity for acidic dyes such as eosin.
2. Lysis of the cytoplasm giving palevacuolated appearance.
3. Reduce differential staining of nucleus& cytoplasm.
4. Complete loss of the cellular outlinese.g. in case of Tuberculosis “T.B” .

Gross Characteristic of necrosis:1. Loss of color or paleness due to “hemolysis of
erythrocytes” in necrotic area.
2. Loss of tensile strength “enzymatic digestion tocytoskeleton, e.g; cell membrane & intercellularconnection”.
3. Necrotic tissues have bad odor or foul smellingdue to ”Bacterial colonization”, then bacterialfermentation of protein result in production ofhydrogen sulfide & ammonia e.g. in case ofGangrene.

Nota Bene (NB):1. If dead tissues are ( congested, hemorrhagic or
contain hemolyzed blood) necrotic tissuesbecome blackish-red in color.
2. Necrosis in liver & lung appear dark “ doubleblood circulation”.
3. Necrosis in heart, kidneys & muscles appearpale “ single blood supply”.
4. A series of chemical changes occur in tissuesafter death similar to necrosis macro µscopically called (Postmortem changes &autolysis ).

Postmortem change:
Generally distinguished from necrosisby being diffuse and not associatedwith inflammatory response.
Autolysis:
Digestion of cell by enzymes releasedfrom lysosomes; occurs after cell diesor it means self digestion by lysosomalenzymes.11/5/2019 18

Patterns of Tissue Necrosis:
1. Coagulative necrosis
2. Liquefactive necrosis
3. Gangrenous necrosis
4. Caseous (Caseation necrosis)
5. Fat necrosis
6. Fibrinoid necrosis11/5/2019 19
![Page 20: Tissue and Cellular Injury - Suli Pharma_cell... · 2019-11-05 · Gangrene: Definition: necrosis of big tissue with superadded putrefaction, black, foul-smelling appearance or define](https://reader033.vdocuments.us/reader033/viewer/2022042317/5f05b8777e708231d4145e28/html5/thumbnails/20.jpg)
1. Coagulative (Coagulation) Necrosis:
• Preservation of the structural outline of the
dead (coagulated) cell for several days.
• The most common form of necrosis (particularly
in myocardium, liver, kidney).
• Characteristic of infarcts in all solid organs
except in the brain.
• Myocardial infarction is a very good example.
11/5/2019 20
![Page 21: Tissue and Cellular Injury - Suli Pharma_cell... · 2019-11-05 · Gangrene: Definition: necrosis of big tissue with superadded putrefaction, black, foul-smelling appearance or define](https://reader033.vdocuments.us/reader033/viewer/2022042317/5f05b8777e708231d4145e28/html5/thumbnails/21.jpg)
Morphology of Coagulative Necrosis:
• Gross lesions:
pale color, normal firm texture at the beginning become soft later due to digestion by macrophages(may lead to rupture of infarcted myocardium)
• Microscopic lesions:
1. first few hours no abnormalities
2. later progressive loss of nuclear staining, with preservationof cell boundaries cell detail is lost, but architecture preserved.
3. This type of necrosis is frequently caused by lack of bloodsupply and is exemplified well in infarcts of solid organs, e. g.heart, skeletal muscle and kidney.
4. finally damaged cells are removed by macrophages
5. The presence of necrotic tissue usually evokes inflammatoryresponse followed by repair.

2. Caseous (Caseation) Necrosis:
Result from a mixture of coagulated protein
with fat lead to formation of caseous
materials resembling cheese.
Some times mixed with blood ”hemorrhage”
or calcification.
Dead tissue architecture is completely
obliterated (no structure) & cellular outline
are not seen.
Characteristic of tuberculosis infection.11/5/2019 27

Gross lesions:
“Caseous” ( Cheese-like) friable yellow-white
gross appearance of the central necrotic area
which is soft, granular, and a cream-cheesy
appearance.
Microscopic lesions:
Structureless amorphous granular eosinophilic
debris surrounded by granuloma (aggregations
of macrophages)
Loss of cellular outlines & differential staining.11/5/2019 28

3. Liquefactive (Liquefaction) Necrosis:
• Complete digestion of the dead cells with rapid
dissolution ”dead tissues undergo liquefaction”.
• The necrotic area is soft and filled with fluid e.g.
abscess: in localized bacterial infections
• Characteristic of bacterial and some fungal infection
accumulation of white blood cells “dead neutrophils”.
• Also seen in hypoxic cell death in the central nervous
system (brain, because its lacks supporting stroma)
• NO cell structure remains.
11/5/2019 34
![Page 35: Tissue and Cellular Injury - Suli Pharma_cell... · 2019-11-05 · Gangrene: Definition: necrosis of big tissue with superadded putrefaction, black, foul-smelling appearance or define](https://reader033.vdocuments.us/reader033/viewer/2022042317/5f05b8777e708231d4145e28/html5/thumbnails/35.jpg)
Causes of liquefactive necrosis:
1. In the brain due to high lipid content & lowcoagulated protein content.
2. In case of abscesses, neutrophils in thenecrotic tissues extrude their lysosomescontaining “hydrolytic enzymes” whichdegraded & liquefy the necrotic tissues aswell as themselves “heterolysis”.
11/5/2019 35
![Page 36: Tissue and Cellular Injury - Suli Pharma_cell... · 2019-11-05 · Gangrene: Definition: necrosis of big tissue with superadded putrefaction, black, foul-smelling appearance or define](https://reader033.vdocuments.us/reader033/viewer/2022042317/5f05b8777e708231d4145e28/html5/thumbnails/36.jpg)
Gross lesions:
1. Consist of cavity containing yellow to white, opaque fluid.
2. Area of L. N. in the brain will be demarcated by a zone ofinflammatory cells.
3. The wall of the cavity is irregular.
Microscopic lesions:
1. Necrotic area appear as empty space no distinct outline
2. Pink staining “eosinophilic” amorphous proteniciousmaterials of liquefactive necrosis found within the space.
3. Abscess is a localized collection of “pus” consist mainly ofdead neutrophils & necrotic debris.

4. Fat necrosis:• It describes focal areas of fat destruction.
• Occurs in the abdominal cavity or under the skin “subcutaneous”.
Causes of fat necrosis:
1. Pancreatic or “Enzymatic” fat necrosis:
Typically seen in :
Acute Pancreatitis or in case of pancreatic neoplasms.
Release of pancreatic lipases liquefy fat cells & hydrolyze triglyceride esters released fatty acids + calcium Fat saponification. 11/5/2019 43

2. Traumatic fat necrosis:
Occur outside the abdominal cavity.
Result from mechanical trauma to subcutaneous adipose tissue e.g.:
Fat necrosis of subcutaneous & muscular area of the back region after prolong recumbence (bedridden patients).

The affected area become opaque, solidor slightly granular , white and chalky.
Microscopical lesions:
Shadowy outlines of necrotic fat cellswith basophilic calcium deposits orpinkish color depending on the present ofsodium or potassium, surrounded by aninflammatory reaction.

Gangrene:Definition: necrosis of big tissue with superadded putrefaction, black, foul-smelling appearance or define as a necrotic tissueinvaded by a saprophytic bacteria. It occurs in necrotic tissueswhich are exposed to air-borne saprophytes (bacteria) such asskin, lung, intestine and mammary glands.
Necrosis of big tissues putrefactive black green
Or organ organisms infection appearance
(black or green due to breakdown of hemoglobin and reaction withH2S produced by the bacteria).
Fe + H2S FeS ( Black or Green color).

a. Dry gangrene:Conditions only occurs on the skin (fingers and
toes) surface in which there is little fluid in thenecrotic tissue following arterial obstructionwith the presence of saprophytic organism.
There is a line of demarcation of between theviable tissue and dark tissue.
Usual causes of ischemia( Loss of blood supply):
1. Freezing, vasoconstriction (frost bite).
2. Vasoconstriction due to poisonous venom.
3. tight bandage and cast.

b. Wet (Moist) gangrene:Condition due to arterial and venous obstruction occur;
it occurs in tissues which are filled with blood at thetime of their death such as the lung, intestine, mammary glands, appendix and uterus.
Wet swollen, foul-smelling, black or green contain gasbubbles and has much hemorrhage.
Rapidly fatal because of systemic toxemia rupture ofthe affected organs.
Significance of wet gangrene:
Gangrenous tissue produces severe toxins which tendsto absorbed into the blood circulation resulting intoxemia and death.11/5/2019 55

Causes of wet gangrene:1. Gangrene in the lung is associated with
acute aspiration pneumonia due toinhalation of irritant oral medication.
2. The intestinal gangrene is associatedwith twists or torsions that result inblockage of the vascular supply orblockage of the venous drainage.

c. Gas gangrene:
Occur in deep contaminated wounds.
Considerable muscle damaged by gas formationbacteria.
Characterized by swollen obviously, gas bubblesformation. The infection rapidly spreads andassociated with severe toxemia.
Only occasionally in civilian practice but is aserious complication of war wounds.

Consequences of necrosis:
① Acute or chronic inflammation.
② Immunological reactions to sub cellularcomponents released by dead tissue orself-antigens altered by denaturation.
③ lysis and absorption.
④ Ulceration and cavity formation.
⑤ Organization convert to connective tissue.
⑥ Encapsulation and/or calcification.
