General tips• Submit a Year-end Report and Claim Form every year even if
you don’t think your operation will trigger a claim. Submitting this form annually allows you to maintain your farming history and your file to be processed more efficiently in claim years.
• For partnerships, each partner must submit a separate Year-end Report and Claim Form but only one partner needs to complete Sections C and D.
• Section A shows the information that Agricorp currently has on file. If the information is incorrect, has changed or if you need to add a new farm, contact Agricorp immediately.
• Answer each profile question in Section B and provide as many details as possible in the Notes field. For timely processing, ensure your information is complete.
• If you’re involved in custom feeding or sharecropping arrangements, complete the applicable tables. On the sharecropping arrangements table, in the Percentage of input costs column, record your share of the allowable expenses associated with producing that crop (e.g., seed, fertilizer, pesticide).

Completing Section CCrop report
Record acres intended for sale and intended as feed for your livestock.
Record the total produced for that crop in the year.
Include all sales, whether from inventory carried over from previous years or harvested this year.
Complete all pre-populated rows, even if you didn’t grow that crop this year.
Example: This producer grew corn and winter wheat this year and sold their soybean inventory from the previous year.

• Use these tables to report your livestock production, purchases and sales.
• Some tables are prepopulated. Complete the prepopulated rows even if the number is zero. If you have any new livestock categories to report this year, you must add them to the tables and complete all columns.
For swine breeding operations, use the following descriptions to complete the tables:
• Swine – Farrow-Early Weaner (born on farm and sold as early weaners)
• Swine – Farrow (born on farm and sold as weaners)
• Swine – Farrow-Finish (born on farm and sold as market hogs)
Indicate the number of sows that gave birth and how many piglets were born. These piglets were raised to market weight and sold as market hogs so they fall under the Swine – Farrow-finish category.
List livestock purchases and sales using the applicable weight categories.
Example: This producer raised sows, market hogs, feeder cattle, beef cows and open heifers. They purchased feeder cattle and bred heifers.

Include any crops that you had in the ground at the end of the year.
