detail with explanations of why the procedure was done and the equipment/technology used. This will vary by physician operator.

Cardiologist will sign up to present cases prior to the Cardiac Imaging Conference. CV Techs will pull films and physician will discuss

cases with peers. Cases will be timely cardiac cath and stress testing procedures/cases for discussion and/or input. Case discussions will

be used as a learning tool to facilitate improved patient care.

FACULTY DISCLOSURE: Disclosures will be made prior to the start of all CME Accredited Activities.

TARGET AUDIENCE IS RESTIRCTED: Cardiothoracic Surgeons, Cardiologist and Medical Staff working with the Cardiac Cath Lab.

Dentists & Dental Hygienists: Participants who attend the entire session and complete an evaluation online will receive a certificate for 1.0 contact hours. This program is accredited by the Michigan Center for Rural Health (MCRH). MCRH is an approved Board of Dentistry (MBOD) Provider Program – # 299150014..This approval is valid from April 2015 through November 2018.

Technology requirements: This presentation will be conducted via webinar. You can listen to the program via your telephone and follow-along watching the PowerPoint on your individual computer. If you are not able to access a computer during the presentation, you may still participate by listening to the program.

Certificates: Certificates will be sent to the participant via e-mail upon successfully filling out and returning the attendance form and completing the evaluation form on-line. Handouts & Required Forms: Presentation handouts - available on the website 2-3 days prior to the grand round.
