This is Signum DSC, a web application that is both a form management tool anddigital signature capture device.

Like all web applications, a secure login is required for all users.

After a successful login, the SIGN FORMS page requests the visiting patient’s IDnumber. If this patient is in the database, the first and last name will appear;otherwise, more information may be requested.

If the user works at several locations, the current one is chosen and CONTINUEis clicked.

A list of available forms appears, and the forms for this patient are chosen.

The next page confirms the chosen forms.

The first form appears on the screen for the patient to read…

…and then sign on the signature pad next to the monitor.

The signature appears on the pad and on the screen as the patient is signing.

Some forms require responses that will be placed in the body of the form to besigned. Textboxes, drop-down lists, and radio buttons can be customized easilyand added to any form.

…and signed by the patient.

Notice that the response that the user provided now appears on the form.

Both forms have been saved as secure PDF files. The user can click the red formnames to view or print the forms.

Here is the resulting PDF file. This type of PDF can never be altered or edited.

Notice the user’s signature was added automatically as a witness. Every user’s signature is captured and saved for this purpose.

The date is also added under the signature.

Later, a user can search for signed forms by date, patient ID, or patient name.
