A Homecare Program for Elders
This program may help you to continue living at home
instead of going to a nursing home. It is state and fed-
erally funded and administered by the Connecticut De-
partment of Social Services. Under the program, your
needs are reviewed to determine whether you can re-
main in your home with help from home care services.
You may qualify for the program if you are: A Con-
necticut resident 65 or older, you meet the program’s
income and asset guidelines financial criteria and/or
you are at risk of nursing home placement, and you
meet the programs income and asset guidelines. A care
manager will determine whether you qualify. If you
qualify, the program can offer you help with household
activities like cleaning, cooking and other chores as
well as visiting nurses or home health aides. Clients in
the higher income ranges may be required to contribute
toward the cost of their care. You may enroll in the
program either by
1. Phoning 1-800-445-5394 to start the health screen-
ing process or
2. If you apply to a nursing home, or you are currently
a resident of a nursing home, or you are in the hos-
pital they will provide you with a home care request
form. Also, they will help you complete the form
to help and determine if you qualify before you call
the above number.
For more information on this program, Contact: CT
Department of Social Service, Alternative Care Unit,
11th floor, 25 Sigourney Street, Hartford CT 06106
