GENERAL INFORMATION

Students or prospective students can obtain additional information about The Salon Professional Academy by directly contacting the school. Contact can be made in a variety of ways and, depending on the information needed, may vary by department. The Salon Professional Academy, established April 2009, offers training in Cosmetology. Students are trained to provide services in hair, skin care, and nail technology. We train students for entry-level positions in a professional salon environment. Part of their education is providing quality, affordable services to the public. General information on the school or student information is required to be available under the Family Educational Rights and Privacy Act of 1974 (FERPA) and can be obtained on the website at: www.huntsvilletspa.com, by calling (256)721-6772, or via email at [email protected]. Paper copies of this information can also be made available upon request. Administrative office hours are Tuesday-Saturday from 9:00am to 4:00pm. All programs, documents, and communications are presented in English. The Academy has deep respect for the diversity of languages within our communities. For the well-being, safety, and effective communication with the general public, students, and faculty, The Salon Professional Academy has designated English as it primary language.

PURPOSE

This Student Consumer Handbook is published by the Financial Aid Office of The Salon Professional Academy. Its purpose is to describe the services available to students through this office as well to provide important information about policies and procedures. This handbook will answer most of your questions relating to financial aid, however, it is important that students are aware that federal regulations are subject to change and may impact policies and procedures stated in this publication. Students should visit the Financial Aid Office for the most current information. General questions can usually be answered on a walk-in basis. Specific questions may require an appointment.

MISSION STATEMENT The Salon Professional Academy is dedicated to student success by enriching lives and cultivating professionals through innovative,

personalized education in a creative and inspiring environment that prepares graduates in the business skills and fiscal responsibility

necessary for success in their career.

STATEMENT OF PRINCIPLE

The Salon Professional Academy strives to provide the resources and support that our students need in order to attend school,

graduate, and find employment in a professional salon environment. We are committed to assisting our students and providing

resources and support that are in the best interest of our students.

RESPONSIBILITY OF HANDBOOK/CATALOG INFORMATION Each student is responsible for knowing the information provided in this handbook and catalog. The

Academy reserves the right to change policies and/or to revise curricula at its discretion without notice.

COMMUNITY

The Academy is located in Huntsville, Alabama. Huntsville has a market area population of 365,000. The community has many great

places to live, work, and play within minutes of the Academy.

FACILITIES AND EQUIPMENT

The Academy is a beautiful 9,000 square foot educational facility with state-of-the-art equipment and spacious hair styling,

manicurist, skin care, student lounge, classroom, and office areas located at 4925 University Drive, Suite 134, Huntsville, AL 35816.

HOW TO APPLY

1) Send completed application form to The Salon Professional Academy.

2) Have high school and post-high school transcripts sent to The Academy.

3) Schedule a visit and tour.

4) Meet staff and students.

5) Meet with the Financial Aid Director and discuss tuition payment options and Federal Financial Aid.

6) Learn about curriculum, books, kits, apparel code, and payment plans.

7) Sign enrollment agreement and pay enrollment fee.

EDUCATIONAL REQUIREMENTS

A student must have a high school diploma, G.E.D. certificate, or official transcript showing proof of high school graduation. Students

who are home schooled must have a diploma or transcript approved by the Alabama State Board of Cosmetology and Barbering.

Students are admitted on the basis of educational background. The Academy does not admit ability-to-benefit students.

NON-DISCRIMINATION

The Academy does not discriminate on the basis of sex, age, race, color, religion, or ethnic origin in admitting students.

ADMISSIONS REQUIREMENTS FOR U.S.CITIZENS

The following are required for admission to all programs at The Academy:

1) Enrollment application completed.

2) Copy of student’s official high school diploma, G.E.D. certificate, Certificate of Home Schooling Program Completion or copy of the

student’s high school and/or college transcripts with graduation date.

3) Copy of the student’s driver’s license, two 2”x2” passport photos, and social security card.

4) Fully executed enrollment agreement and financial agreement with required enrollment fee of $200 that is due at signing. Enrollment

fee is not eligible for VA funding. 5) Students who have been terminated or withdrew from school may re-enroll (if determined eligible) and will be responsible to pay any remaining balance from the previous enrollment plus any increase in tuition rates, if applicable. Students who have been terminated or withdrew from school and re-enroll (if determined eligible) will pay a $200 re-enrollment fee and will be charged for contracted hours at the current tuition rate. All re-enrolling students will be provided the school’s Re-enrollment Policy and will be evaluated by the School Director for placement in the curriculum and kit needs. Re-enrolling students may be required to purchase the current school kit. Students applying for re-entry or transfer-in from other schools may be required, as a condition of enrollment, to bring delinquent prior student loans to a current status.

ADMISSIONS REQUIREMENTS FOR NON-U.S.CITIZENS

Applicants who are not citizens of the United States have the same requirements as U.S. citizens, except that a valid visa or green card must be provided in place of the documentation that only U.S. citizens can provide, such as a birth certificate and social security card. Foreign High School transcripts are evaluated by the Alabama Board of Cosmetology and Barbering before enrollment.

ORIENTATION

All courses will have a complete orientation on the Saturday before the first day of class.

CLASS SIZE EARLY ENROLLMENT IS ENCOURAGED. The Academy limits the class size for all programs. For each class, we accept: Cosmetology: 20 students Esthetics: 8 students Nail Tech: 10 students Student Instructor Program: 4 students The student/educator maximum ratio is 20/1.

CLASS START DATES

Cosmetology classes typically start in January, March, May, July, September, and October. See insert #1 for class start dates. Esthetics

classes have not been scheduled at this time. Nail Tech classes have not been scheduled at this time. Student Instructor classes start

throughout the year as students are accepted.

TRANSFER POLICY

A transfer student may be accepted after careful evaluation of the student's academic records. No more than 20% of the program can

be credited from another institution unless the student is transferring from another Salon Professional Academy. Each transfer must be

evaluated on an individual basis. Transfer students pay full tuition. Transfer students must begin at the beginning of the program. All

transfer hours accepted are applied at the end of training. We do not recruit students already attending or admitted to other schools

offering similar programs. We do not guarantee the transferability of our credits to any other institution. Students receiving VA benefits

must provide official transcripts for previous education and training, and appropriate credit will be given and the training period

shortened proportionately.

HOURS

The Academy is open Tuesday through Saturday plus evenings as follows Tuesday 8:30am – 5:30pm, Wednesday and Thursday

8:30am – 8:00pm Friday and Saturday 8:30am – 4:30pm. Hours of attendance depend on the schedule assigned.

HOLIDAYS

The Academy is closed for the following holidays: New Year’s Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day,

Christmas Eve, and Christmas Day.

BOOKS AND KIT

Appropriate books and kits are provided by the Academy and issued to the student during training at the appropriate time. Students

provide their own paper supplies.

Students wear academy-approved professional apparel and shoes.

Physical Demands of Cosmetology There are physical demands placed on the body in any career. In cosmetology, care must be taken to protect your back, legs and feet. • One way to do this is strength training to enhance your back, abdomen and leg muscles. Regular weight lifting exercises will benefit individuals in this profession. Regular exercise will help to promote all over body conditioning and will improve circulation in your legs and feet. We recommend consulting your doctor before beginning any exercise program. •This job requires standing for long periods of time, so it is suggested that you wear proper fitting and supportive shoes and support hose. These are not a requirement, but will help increase your chances of longevity in the profession. Safety Requirements of Cosmetology Safety suggestions for this profession include: • Wearing shoes that would not be slippery when walking on a damp floor. You will be working with water which means there is always a risk of water spills. All water spilled should be wiped up as quickly as possible. Damp hair lying on the floor can also pose a chance of slipping. All hair needs to be swept up immediately following each haircut to minimize accidents. It is the responsibility of each stylist to promote a safe working environment. • Gloves are to be worn during chemical services to reduce the risk of any allergic reaction that an individual may have to certain chemicals. • Anytime a product accidentally gets in your eyes, flush your eyes thoroughly with water. (It is recommended that this is done for at least 15 minutes.) Seek medical attention if irritation continues.

CAMPUS SECURITY

Campus Security: Annual Security Report & Drug Prevention Policy Campus Security Act Information Disclosure: The safety of our campus community is very important to The Salon Professional Academy. Involvement of our campus and community officials help ensure that we are able to provide a safe environment for learning and growth on campus. The following information is provided regarding our campus security and we ask that students read it carefully and ask questions about anything they may not understand. This information is provided to encourage a safe campus and, as a member of our campus, we ask that students share in this responsibility to prevent crime from occurring on our campus. In compliance with the Campus Crime Statistics Act, we are required to provide students with our annual security report. Included in this report are statistics concerning reported crimes which have occurred on campus or on public property surrounding the campus for the previous three years. In addition, prior to October 1st each year, The Salon Professional Academy updates this report and presents it to the students and staff. At this time, a local police officer also comes on campus to discuss the topics covered in this report with the campus population. The Director of Education also utilizes this time to identify any hazards or other safety concerns that need to be addressed. To request more information on these requirements, clarifications on the procedures within this report, or any information contained in this report, students may contact the Director of Education. Preventing campus crime is a responsibility that should be taken very seriously. Students are to never assume that someone else has already reported a crime or suspicious activity. All criminal actions, suspicions, or emergencies must be reported to an educator, administrator, or owner immediately. The staff member will then assist in reporting to the local police or other appropriate security force. A staff member made aware of a crime will notify the rest of the staff as soon as possible, perhaps even prior to notifying police, depending on the situation. It is critical that all staff be aware of any report of crime and that the local police be notified immediately. Timely Warning The Salon Professional Academy will issue a warning when an incident that represents a serious or ongoing threat to the safety of the campus is brought to the attention of the administration, law enforcement, or other offices. The administration will make every effort to inform the campus that a warning has been put in place within 24 hours of the determination being made. The decision to issue this alert will be based on the specific incident and the factors surrounding it. The decision to issue a warning will be made by the Director or the Director of Education and will contain as much of the following information as possible and if available: _ Date and time the warning was issued _ Connections to previous incidents _ Composite drawing of the suspect _ Description of the suspect _Any other information that may be relevant _Any information relating to tips to remaining safe Depending on the circumstances surrounding the warning, The Salon Professional Academy will inform the campus through means such as Facebook, signage in or around the building, or possibly notifying local television and radio stations.

Building Information The facilities are open Tuesday through Saturday according to assigned class/styling area schedules. The building may also be open for educational classes for licensed professionals in Cosmetology or to groups securing the use of the facilities through the owner, such as cosmetology industry vendors and suppliers for training and education purposes. Only educators, administrators, and owners have keys to the building thus preventing internal crimes to as great an extent as possible. The Salon Professional Academy is protected by an alarm system and full sprinkler system as required by the city of Huntsville. The Academy utilizes and monitors security cameras placed inside and outside the building.

The Salon Professional Academy has no off-campus locations of student organizations which are officially recognized (examples include: sororities, fraternities, etc.). Emergency Information Huntsville Police Department, emergency, Dial 911 Huntsville Police Department, non-emergency dispatch (256) 722-7100 Stephanie Schultz, Consultant (256)679-2601 Lee Schultz, Owner (256)508-0702 Campus Safety All reported incidents are reviewed and applicable information is collected and presented to the proper authorities. In order to ensure that our campus remains safe, it is important for members of the campus to report this information in a timely manner. Any suspicious activity should be considered a reasonable suspicion and reported. (Examples of suspicious activities include seeing an unescorted guest in an unapproved area, doors propped open or unauthorized individuals using campus equipment or offices.) If it seems a situation appears abnormal in any way, students and staff are asked to report it. The Salon Professional Academy does not have specific policies or procedures for confidential crime reporting, but crimes can be reported confidentially to the local authorities by simply requesting that the complaint be made anonymously. Emergency Response and Evacuation Procedures In the event and confirmation of a dangerous or emergency situation which could pose an immediate threat to the health or safety of students, employees, or guests on campus, The Salon Professional Academy administration and staff will immediately notify everyone on campus. Individuals will be notified of what the danger is and what actions should (or should not) be taken to best protect themselves from the danger. In addition to notifying individuals, the local Police Department and/or Sheriff’s Department will be contacted and will consult with the administration in an effort to maintain order and isolate the situation until the danger is contained. Additionally, The Salon Professional Academy will post timely written notices and warnings explaining the danger and instructions on how to ensure the safety and health of individuals by avoiding the affected area. On-Campus Notification Upon confirmation of an emergency or dangerous situation, The Salon Professional Academy will: • Immediately notify all staff in the building. Staff members will then be responsible for notifying all students and guests and ensuring the proper procedure for the specific danger (example: Fire Exit Procedure) is followed. • Contact the local authorities (if necessary) to assist in the containment of the emergency or dangerous situation. • Contact the Director and Owner (if they are not presently in the building) to inform them of the situation and consult with them in an effort to maintain order and isolate the situation until the danger is contained. • If the danger is contained to a small area of the building, the administration will post written notices and warnings on the inside and outside of all entry/exit doors and in other locations as necessary. The notices will include a description of the danger, what actions should be taken to minimize any possible harm, and an estimated duration of the danger. (If the danger consumes the entire building, the administration will post these notices, if possible, avoiding areas that could pose a threat of possible harm to staff members.)

On-Campus Notification of a Potential Danger (Example: heavy storms or tornados) If there is anticipation of a danger which causes the campus to close for a period of time, the administration at The Salon Professional Academy will: • Notify local television and radio stations and post on the Academy’s Facebook page of the closure. • Post written notices and warnings on the inside and outside of all entry/exit doors and in other locations as necessary. The notices will include a description of the danger, what actions should be taken to minimize any possible harm, and an estimated duration of the danger. • If the campus is open prior to the danger, the administration will notify all staff of the possible danger and what actions can be taken to avoid such danger. The staff will then notify all students and guests on campus who may be affected by the danger. If time permits, all guests who have scheduled appointments will be notified of the closure and possible rescheduling options. Notifying the Local Authorities Depending on the nature of the problem, the proper authorities will be notified by the staff (or administration, if possible) of The Salon Professional Academy. Chosen authorities will be dependent on the nature of the danger, and could include any or all of the following: the Huntsville Police Department, the Huntsville Fire Department, Ambulance services, or Emergency Services (911). At least once annually, the administration at The Salon Professional Academy will provide the Huntsville Fire Department and the Huntsville Police Department with a floor plan of the building. The administration will also notify these agencies of any new construction, long-term functions or any other events which may affect routing or access to the campus. In addition to floor plans, the Huntsville Police Department, Huntsville Fire Department, and other local emergency officials are encouraged to tour and walk through the campus regularly. Testing of Emergency Response Evacuation Procedures At regular intervals during training, staff and students will be reminded of security and safety procedures including crime prevention, personal safety on and off campus, fire safety, dangerous weather procedures, etc. Local police speakers will be scheduled at least once annually for all students and staff. In addition, The Salon Professional Academy conducts in-house testing of emergency evacuation procedures at least once annually. Students and staff complete a walk-through of the fire and tornado exit procedures and are informed of what to do in case of an emergency.

Safety Procedures First Aid A first aid kit is located in the dispensary and the educators’ office.

Cut finger: Educator should immediately check the wound to see how deep the cut is. Small cuts should be washed, dried, and then covered with a bandage.

Cuts requiring stitches: If possible, a staff member should take the student to the doctor.

Fainting Do not move the person; call 911. Give facts as much as possible. Keep calm and notify other staff members for help if necessary. Make the person comfortable. Fire Exit Procedure If you smell smoke or see fire, report it immediately to an educator. DO NOT PANIC!!! Proceed as follows:

Students in the salon area, exit single file out the front doors. If you have a client at the time, the client is your responsibility. After exiting from the school, proceed into the parking lot away from the building.

Receptionists at the front desk will help clients in the reception area out the main doors. Proceed into the parking lot. Students in the classrooms exit through the rear door in single file. Proceed to the front by going around the end of the

building. Tornado Warning Procedure Stay away from all windows. Proceed as follows:

Students and clients in the styling area will walk to the esthetics room or manicure room. Be seated on the floor making space available for others. Remain there until instructed to go back to your previous activity.

Annual Campus Crime Statistics The Salon Professional Academy will inform students and employees of any crimes committed on campus within two days of the incident. This information will also be entered into the Campus Crime Log within two days of the incident, unless the disclosure is prohibited by law or could impact the confidentiality of the victim. Incidents which have occurred in the most recent 60 days and entered in the crime log are open for public inspection during regular business hours and incidents which are older than 60 days will be provided for public inspection within two business days of the request. The Campus Crime Statistics listed are prepared annually and are provided to all students and employees.

• Forcible sex offenses: Any sexual act directed against another person, forcibly and/or against the person’s will, or not forcibly or against the person’s will, where the victim is incapable of giving consent. Reported sexual offenses may include forcible rape, forcible sodomy, sexual assault with an object and forcible fondling. • Non-forcible sex offenses: Unlawful, non-forcible sexual intercourse. Reported offense may include incest and statutory rape. • Robbery: The taking or attempting to take anything of value from the care, custody, or control of a person or persons by force or threat of force or violence and/or making the victim fearful. • Aggravated assault/battery: An unlawful attack by one person upon another for the purpose of inflicting severe or substantial bodily injury. This type of assault may be accompanied by the use of a weapon or other means likely to produce death or great bodily harm. • Simple assault/battery: All assaults and attempted assaults which are not of an aggravated nature and do not result in serious injury to the victim. • Burglary: The unlawful entry of a structure to commit a felony or theft. • Motor vehicle theft: The theft or attempted theft of a motor vehicle. • Destruction, damage or vandalism to property: To willfully or maliciously destroy, injure, disfigure, or deface any public or private property, real or personal, without the consent of the owner or person having custody or control by cutting, tearing, breaking, marking, painting, drawing, covering with filth, or any other such means as may be specified by local law. • Arson: Any willful or malicious burning or attempting to burn, with or without intent to defraud, a dwelling house, public building, motor vehicle, aircraft, personal property of another, etc. • Liquor law violation: The violation of laws or ordinances prohibiting manufacturing, selling, transporting, furnishing, or possessing of intoxicating liquor; maintaining unlawful drinking places; bootlegging; operating a still; furnishing liquor to a minor or intemperate person; using a vehicle for illegal transportation of liquor; drinking on a train or public conveyance; and all attempts to commit any of the aforementioned. • Drug law violation: Violation of state and local laws relating to unlawfully possessing, selling, using, growing, manufacturing and making narcotic drugs. • Weapon law violation: The violation of laws or ordinances dealing with weapon offenses, regulatory in nature, such as: the manufacturing, sale or possession of deadly weapons; carrying deadly weapons, concealed or openly; furnishing deadly weapons to minors; aliens possessing deadly weapons; and all attempts to commit any of the aforementioned. • Hate crimes: Any of the crimes listed here which manifest evidence of prejudice based on race, religion, sexual orientation, ethnicity, disability or gender. • Larceny-theft: The unlawful taking and carrying away of someone else's personal property with the intent to deprive the possessor of it permanently. • Intimidation: Coercing or inhibiting through the use of fear. Non-Harassment Policy (Includes sexual harassment) Our Academy does not tolerate and expressly condemns any harassment of our employees, students, or guests by any employee, student, or non-employee. Any form of harassment relating to any persons race, color, gender, religion, national origin, age, sexual orientation, or disability is a violation of our policy and will be treated as a disciplinary matter. For our purposes, the term “harassment” includes, but is not necessarily limited to: slurs, jokes, and other verbal, graphic, or physical conduct relating to an individual’s race, color, gender, religion, national origin, age, sexual orientation, or disability. “Harassment” also includes unwelcome sexual or social advances, request for sexual favors, and other verbal, graphic, or physical conduct of a sexual nature. “Harassment” also includes all forms of sexual violence including, but not limited to rape, sexual assault, sexual battery, and sexual coercion. Proven violation of this policy by a student shall subject that student to disciplinary action up to and including termination from the program and, if deemed appropriate, providing information on the act to the local authorities. Proven violation of this policy by an employee shall subject that employee to disciplinary action up to, and including, termination and providing information on the act to the local authorities. If you feel you are being harassed by any other student, employee (supervisory or non-supervisory) because of your race, color, gender, religion, national origin, age, sexual orientation, or disability, or are subjected to sexual harassment, you should at once make your feelings known to the owners or the director. We will give top priority to any such matter. We will see that the matter is investigated and, where appropriate, that disciplinary action is taken. During the investigation of a harassment complaint, both the accused and the accuser will be entitled to the same opportunities including having others present during disciplinary proceedings and being notified of the outcome of the disciplinary proceeding. Victims will be provided assistance in notifying local authorities and identifying off-campus services including, but not limited to, victim services, educational programs, and support groups. If necessary, the victim will also be given options which will allow changes to be made to the victim's academic schedule. Harassment of our students in connection with their learning by non-employees may also be a violation of this policy. Any student who becomes aware of any harassment of any student by a non-employee should report such harassment to the owner or the director. Appropriate action will be taken against violations. Crime Log The Salon Professional Academy maintains a written crime log. The crime log details the time and date the crime was reported, the nature of the crime, the general location of each crime, and the disposition of the complaint, if known. Any and all crimes that are reported to The Salon Professional Academy Staff that occurred on campus will be recorded in this log. This log is part of the school’s Operations Manual.

The Salon Professional Academy will make an entry to the log within 2 business days (Tuesday – Saturday, except days when the school is closed) of the report of the information to The Salon Professional Academy staff, unless that disclosure is prohibited by law or would jeopardize the confidentiality of the victim. The Salon Professional Academy may withhold one or more of the required pieces of information if there is clear and convincing evidence that the release of the information would:

Jeopardize an ongoing criminal investigation or the safety of an individual Cause a suspect to flee or evade detection, or Result in the destruction of evidence

The school will disclose any information that is initially withheld for any of these reasons, once the adverse effect is no longer likely to occur. The Salon Professional Academy will make the crime log for the most recent 60-day period open to public inspection during normal business hours. The school will make any portion of the log older than 60 days available within two business days of a request for public inspection. The school is not required to provide a timely warning with respect to crimes reported to a pastoral or professional counselor.

DRUG AND ALCOHOL ABUSE INFORMATION

A conviction related to a drug offense could cause a student to lose Title IV Financial Aid eligibility. DRUG AND ALCOHOL ABUSE PREVENTION The Academy prohibits the unlawful possession, use or distribution of illicit drugs and alcohol by students and staff on our property or as part of any of our activities. The Academy will immediately contact law enforcement officials to report these activities.

The Academy provides education annually and refers students and staff to local services. The health risks of the use of illicit drugs and alcohol abuse require providing education and referral for students and staff. Area drug abuse information, counseling, referral and treatment center information are made available to students and staff members.

The Academy will expel students and terminate staff involved in unlawful possession, use, or distribution of illicit drugs and alcohol. The Academy will refer such cases to the proper authorities for prosecution. Students and staff may be reinstated upon completion of an appropriate rehabilitation program. As a condition of employment, employees must notify The Academy of any criminal drug statute conviction for a violation occurring in the workplace not later than five days after such conviction.

There are serious legal sanctions for illegal use of drugs and/or alcohol. There are serious health risks associated with drug and/or alcohol use. Health risks associated with the use of illicit drugs and the abuse of alcohol include: impaired mental and physical health, neurological disease/damage, memory and intellectual performance interference, mental and physical depression, uncontrollable violence, impulsive behavior, convulsive seizures, homicide, suicide, cardiac disease or damage, cardiovascular collapse or heart failure, gastrointestinal disease or damage, ulcers or erosive gastritis, anemia, liver and pancreatic disease, liver failure or pancreatitis, deteriorating relationships, and death.

FAMILY EDUCATIONAL RIGHTS AND PRIVACY ACT OF 1974 (FERPA) POLICY

Family Policy Compliance Office (FPCO) Home

The Family Educational Rights and Privacy Act (FERPA) (20 U.S.C. § 1232g; 34 CFR Part 99) is a Federal law that protects the

privacy of student education records. The law applies to all schools that receive funds under an applicable program of the U.S.

Department of Education.

FERPA gives certain rights to students and parents or guardians of dependent minors with respect to education records. These rights transfer to the student when he or she reaches the age of 18 or attends a school beyond the high school level. Students to whom the rights have transferred are "eligible students."

Parents/guardians of dependent minors or eligible students have the right to inspect and review the student's education records maintained by the school. Schools are not required to provide copies of records unless, for reasons such as great distance, it is impossible for parents/guardians of dependent minors or eligible students to review the records. Schools may charge a fee for copies.

Parents/guardians of dependent minors or eligible students have the right to request that a school correct records which they believe to be inaccurate or misleading. If the school decides not to amend the record, the parent/guardian of dependent minors or eligible student then has the right to a formal hearing. After the hearing, if the school still decides not to amend the record, the parent/ guardian of dependent minors or eligible student has the right to place a statement with the record setting forth his or her view about the contested information.

Schools must have written permission from the parent/guardian of dependent minors or eligible student in order to release any information from a student's education record every time. However, FERPA allows schools to disclose those records, without consent, to the following parties or under the following conditions (34 CFR § 99.31):

a. School officials with legitimate educational interest. b. Other schools to which a student is transferring. c. Specified officials for audit or evaluation purposes. d. Appropriate parties in connection with financial aid to a student. e. Organizations conducting certain studies for or on behalf of the school. f. Accrediting organizations. g. To comply with a judicial order or lawfully issued subpoena. h. Appropriate officials in cases of health and safety emergencies; and State and local authorities, within a juvenile justice system, pursuant to specific State law.

Schools may disclose, without consent, "directory" information such as a student's name, address, telephone number, date and place of birth, honors and awards, and dates of attendance. However, schools must tell parents and eligible students about directory information and allow parents and eligible students a reasonable amount of time to request that the school not disclose directory information about them. Schools must notify parents and eligible students annually of their rights under FERPA. The actual means of notification (special letter, inclusion in a PTA bulletin, student handbook, or newspaper article) is left to the discretion of each school. SAFEGUARDING CUSTOMER INFORMATION

Procedure for Maintaining and Safeguarding Student Records

All student financial records are kept in a cabinet which is located in the financial aid office. The financial aid office is locked when unoccupied. The Financial Aid Representative, Academy owners, and the Director are the only persons who have keys to the Financial

Aid office. The building in which the Financial Aid office is located is protected by a security system. Student records are maintained on a computer software system called FAME.

SATISFACTORY ACADEMIC PROGRESS POLICY

The Satisfactory Academic Progress policy (hereafter referred to as SAP) applies to all students. Satisfactory Academic Progress in

attendance and academic work is a requirement. Students must maintain Satisfactory Academic Progress. To determine SAP, all

students are evaluated in academics and attendance at periodic checkpoints. Students are advised of their academic and

attendance status via a progress report. Students will be notified in writing of all SAP evaluations.

ATTENDANCE PROGRESS

Students are expected to attend classes as per their enrollment agreement. Students are responsible for clocking in and out

appropriately to document their hours. The only documentation accepted for student hours is the time clock system. Late arrivals

must check in with the educator of their class or the salon area when they arrive. Continued tardiness may result in disciplinary

action up to and including termination. Students who are going to be absent are expected to call the school 30 minutes prior to the

beginning of their scheduled shift. All absences are recorded and made a part of the school permanent record. The student is

responsible for class material and/or tests missed while absent. Students are encouraged not to miss any days during their

scheduled foundations period. Students who miss 10 consecutive scheduled days (14 calendar days) without communicating with

the School Director/Educator may be terminated on the 11th consecutive school day. Students who persist in repeating patterns of

absenteeism will be advised and will be subject to appropriate disciplinary action. Students who have excessive absences may be

terminated. Re-enrollment is at the discretion of the school.

MAXIMUM TIME FRAME

Students must attend a minimum of 85% of the cumulative scheduled hours to maintain SAP and complete the course within the

maximum allowed time frame. The maximum time frame is equal to 118% of the contract hours of the course. The maximum

time frame in which students have to complete:

fees accumulated past the students contract end date will be the responsibility of the student.

ACADEMIC PROGRESS

Students are assigned classroom study and practical assignments. Classroom study is evaluated by written exams given after each unit

of study that are built upon knowledge that has been taught to that point in their hours and unit finals that require a minimum grade of

85 or better. Practical assignments are evaluated as completed and counted toward course completion only when rated as satisfactory

of better. Students must maintain a grade average of 85% GPA and pass a final written and practical exam prior to graduation. The

Academy utilizes a 100-point numerical grading scale according to the following:

95 -100 A = Excellent

90-94 B = Very Good

85-89 C = Satisfactory

0-84 Below standard – Unsatisfactory

Inadequate grades may indicate a lack of student inability or motivation. When a student is struggling in one or more areas of study or

skill level, the educator will advise the student on how the deficiency can be successfully completed and determine a deadline for the

work to be completed satisfactorily. The student will be notified immediately after a grading period of how the deficiency can be

successfully completed. Students may receive an “Incomplete” from an educator when the student is attempting to learn a skill or

subject but needs additional time to complete the work successfully. Students will be given timelines to complete the work and a

description of the work yet to be completed.

DETERMINATION OF PROGRESS

Students meeting the requirements for academics and attendance at the end of each checkpoint period will be considered making

SAP until the next checkpoint. In order for a student to be considered making SAP, the student must meet both 85% attendance

and 85% academic minimum requirements. Evaluations will determine if the student has met the minimum requirements for SAP.

Students deemed not maintaining SAP may have their Title IV Funding interrupted unless the student is on warning or has prevailed

upon appeal resulting in a status of probation. The frequency of evaluations ensures that students have a least one evaluation by

midpoint of course. Students will be evaluated at the following checkpoints:

WARNING

Any student failing to meet requirements for attendance or academic progress at a checkpoint will be placed on warning status and

considered to be maintaining SAP during the warning period. Students are not required to appeal a warning status and the student

will be eligible for financial aid disbursements during the payment period. Documentation will be placed in the student’s file. The

student will be advised in writing of the actions required to attain SAP by the next evaluation. If at the end of the warning period,

the student has still not met both the attendance and academic requirements, he/she may be placed on probation status and, if

applicable, deemed ineligible to continue to receive Title IV funding.

PROBATION

Students who fail to meet minimum requirements for attendance or academic progress after the warning period may be placed on

SAP dismissal status. During SAP dismissal status, the student will be deemed ineligible for Title IV Funds and/or Veterans Benefits

for the subsequent payment periods or until SAP is reestablished. If the student appeals the SAP dismissal decision, and prevails

upon appeal the student will instead be placed on Probation Status. Students who wish to appeal the school’s determination (for

reasons such as death of a relative, injury/illness of the student, or other special circumstances) must appeal within the required

time frame and on the school-required form (see Appeals). If the school grants the student’s appeal, the student will be placed on

Probation Status until the next checkpoint. A student may be placed on Probation Status for one checkpoint period. Only students

who have the ability to meet the SAP Policy standards by the end of the evaluation period may be placed on Probation Status. The

student will be advised in writing on the actions required to attain SAP by the next evaluation. An academic and/or attendance plan

will be developed that will allow the student to meet SAP at the next checkpoint if followed. However, if at the end of the

probationary period, the student has still not met both the attendance and academic requirements, he/she will be determined as

NOT making satisfactory academic progress.

APPEAL PROCEDURE

If a student is determined as NOT making SAP, the student may appeal the negative determination. Student must submit

documentation and completed school appeal form to the school administration within 10 calendar days of the determination, as to

why the student failed to make SAP and what has changed in the student’s situation that will allow the achievement of SAP at the

next evaluation. An appeal hearing will take place within 10 business days of receipt of the written appeal. The hearing will be

attended by the student, parent/guardian, if applicable and appropriate school staff. Appeal documents will be reviewed and a

decision will be made and reported to the student within three (3) business days after this hearing, and will be communicated to

the student in writing. A copy of the appeal and decision documents will be retained in the student file. This decision will be final.

If student prevails upon appeal, the SAP determination will be reversed and FSA funding will be reinstated, if applicable.

FINANCIAL AID

Students who do not meet SAP requirements are no longer eligible to receive Title IV funding, unless the student is placed on

warning or has prevailed in their appeal of the SAP determination and is placed on probation until the next checkpoint. Students will

be notified in writing of all SAP evaluations.

A withdrawal date on a student who had been previously attending could be up to, but not to exceed 14 calendar days from that student’s actual last date of attendance. An active student officially withdraws when the Academy’s administrative office has received written notification of their intention to withdraw from school. An active student is considered unofficially withdrawn when they have been absent for 14 calendar days from their last date of physical attendance without notifying the school’s administrative office. This will be the determination date of withdrawal. REESTABLISHMENT OF STATUS A student determined NOT to be making SAP may reestablish SAP and financial aid eligibility by:

1) Making up missed tests and assignments and increasing grade average to 85% or better, and/or 2) Increasing cumulative attendance to 85%. An educator must be present to receive attendance credit.

COURSE INCOMPLETES, REPETITIONS AND NON-CREDIT REMEDIAL COURSES Course incomplete, repetitions, and non-credit remedial courses are not offered at The Salon Professional Academy. LEAVE OF ABSENCE POLICY A student may be granted a leave of absence for circumstances beyond the control of the student. The leave of absence must be

requested and approved in writing prior to a leave of absence occurring. The student must complete a Leave of Absence Request

Form, which requires a list of the reason(s) for the leave of absence, provide applicable documentation supporting the reason

stated, and has the student’s signature. A copy will be made and given to the student at the time of signing and the original is

placed in the student’s record file.

Emergency leaves of absence, without prior written request may be granted provided the student completes the Leave of Absence

Request Form and it is returned to the school via mail or in person at a later date. The start date of the leave of absence will be

established as the first day the student was unable to attend. The Academy will document its decision on the Leave of Absence

Request Form.

The maximum time frame for a leave of absence is 180 days. The school permits more than one leave of absence in a 12-month period provided that the total numbers of days of all leaves of absences does not exceed 180 days in a 12-month period. Additionally, when taking a leave of absence, the student is required to take a minimum of 7 calendar days per approved leave. The student is required to keep in contact with the Director or Financial Aid representative to make plans for their return. If no contact has occurred via phone or mail, notice will be given to the student in regards to their future at the Academy. A student who must take an approved Leave of Absence (LOA), or must withdraw from training for nonacademic reasons, may

return to the program with no loss of SAP if the student was making SAP when they left. A student may be granted an LOA for any

of the following reasons:

1) Financial Hardship 2) Medical Issues 3) Extenuating circumstances beyond the student’s control.

On the day the student returns from an LOA, the student is required to inform the Financial Aid Director and the Education office of

their return. The student’s contract will be extended for the same number of days the student was on LOA without any penalty to

the student. The student’s contract and maximum time allowed will be extended for the same number of days the student was on

the leave of absence without any penalty to the student. This will be documented with an addendum to the enrollment agreement.

All changes to the enrollment agreement will be initialed by all parties. No additional charges will be assessed as a result of a

requested leave of absence.

While on a leave of absence the student is not considered to have withdrawn and no refund calculation is required at that time. If a

student does not return to the Academy at the expiration of an approved leave of absence, (or a student takes an unapproved leave

of absence), the student’s withdrawal date for purposes of calculating a refund is always the student’s last day of attendance.

TRANSFER HOURS With regard to SAP, a student’s transfer hours will be counted as both attempted and earned hours for the purpose of determining when the allowable maximum time frame has been exhausted. REENTRY STUDENTS/INTERRUPTIONS Students who have been terminated or withdrew from school may re-enroll (if determined eligible) and will be responsible to pay any remaining balance from the previous enrollment plus any increase in tuition rates, if applicable. Students who have been terminated or withdrew from school and re-enroll (if determined eligible) will pay a $200 re-enrollment fee and will be charged for contracted hours at the current tuition rate. All re-enrolling students will be provided the school’s Re-enrollment Policy and will be

evaluated by the School Director for placement in the curriculum and kit needs. Re-enrolling students may be required to purchase the current school kit. Students applying for re-entry or transfer-in from other schools may be required, as a condition of enrollment, to bring delinquent prior student loans to a current status. If enrollment is temporarily interrupted for a Leave of Absence, the student will return to school in the same progress status as prior to the leave of absence. Hours elapsed during a leave of absence will extend the student’s contract period and maximum time frame by the same number of days taken in the leave of absence and will not be included in the student's cumulative attendance percentage calculation. Students who withdraw prior to completion of the course and wish to re-enroll will return in the same Satisfactory Academic Progress status as at the time of withdrawal. PLANS FOR IMPROVING AN ACADEMIC PROGRAM The Salon Professional Academy will make readily available any plan for improvement of any academic program upon the determination by the institution that such a plan exists.

ADMINISTRATION AND FACULTY

The Salon Professional Academy’s faculty consists of the Director, Admissions Representative, Financial Aid representative, Director of Education, Support Staff, and a minimum of one educator per 15 students.

MISREPRESENTATION

The Salon Professional Academy strives to represent itself transparently. The Academy does not misrepresent the nature of the educational programs, financial charges, employability of graduates, or any other information. All questions regarding information about facilities and curriculum can be directed to the education office. Questions about enrollment and placement should be directed to the Admissions office and any questions concerning financial aid should be directed to the Financial Aid office. All offices have an open door policy.

COPYRIGHT MATERIALS AND INFRINGEMENT

The purpose of the Copyright Infringement Policy is to comply with copyright law for the use of copyrighted material. In addition, this policy seeks to make aware to all users the seriousness as well as possible consequences for unauthorized use of copyrighted material. The Salon Professional Academy strictly prohibits any and all of the following: copyright, trademark, patent, trade secret, or other intellectual property infringement, including but not limited to using any copyrighted names, text or images, offering pirated computer programs or links to such programs, serial or registration numbers for software programs, copyrighted music, etc., as policy on the use of copyrighted material on the Institution’s computer system and network.

The Salon Professional Academy respects the copyrights of those involved in creating and distributing copyrighted material, including music, movies, software, and other literary and artistic works. It is the policy of The Salon Professional Academy to comply with copyright law. If users utilize copyrighted materials for educational, instructional, research, scholarship, and like areas, the Academy will follow the legal doctrine of fair use currently a part of the copyright law. The Academy’s students and staff will not make unauthorized copies of copyrighted material on or using The Salon Professional Academy’s computer system, network, or storage media. Also, the Academy’s staff and students will not store unauthorized copies of copyrighted works using The Academy’s system, network and/or storage media. The Academy’s staff and students should not download, upload, transmit, make available, or otherwise distribute copyrighted material without authorization using the Academy’s computer system, network, and Internet access or storage media. This is inclusive of utilizing unlicensed/unauthorized peer-to-peer file services that would promote copyright infringement. The Salon Professional Academy reserves the right to monitor its computer systems, networks and storage media for compliance with this policy, at any time, without notice, and with or without cause. Additionally, the Academy reserves the right to delete from its computer systems and storage media or restrict access to any seemingly unauthorized copies of copyrighted materials it may find at any time and without notice. Users who violate this policy are subject to disciplinary action as appropriate under the circumstances. Such disciplinary action may include termination, expulsion and other legal actions. EMERGENCY RESPONSE/EVACUATION Emergency procedures regarding safety, fire, and tornadoes are presented to the students in orientation. These procedures are also available at any time from the Admissions, Financial Aid, or Education offices. Tornado and Fire Drills are performed annually. TIMELY EMERGENCY WARNINGS Any emergencies are reported immediately to the Lead Educator, then Director. The process and procedures that must take place will be determined by the Director according to what type of emergency is occurring. VACCINATION POLICY The Salon Professional Academy does not require any vaccinations or records for attendance or admission.

VOTER REGISTRATION

The Salon Professional Academy provides voter registrations to every student enrolled.

SERVICES TO STUDENTS WITH DISABILITIES

The Salon Professional Academy will provide reasonable accommodations to any student with a disability. The student should discuss the disability with the Lead Educator. The student should explain in writing what accommodations are required and provide any needed documentation. The Lead Educator and Director will develop and implement a plan to accommodate the student. All facilities at The Salon Professional Academy are handicap accessible.

CAREER PLANNING

Placement

The Academy maintains contacts in the cosmetology profession to assist students in job placement. Employers are encouraged to

interview students and every effort is made to secure a job opportunity for each graduate. Students are prepared in the latter part of

training to seek employment. Job opportunities are announced and posted. The Academy cannot guarantee job placement, however,

the Academy does follow-up on graduates to help prepare new students for future job placement.

Reciprocity

Licensed professionals in cosmetology, esthetics, nail techs, and student instructors from Alabama may apply for licenses in their field of

expertise in other states and must comply with each state’s laws and rules to become licensed there.

CAREER OPPORTUNITES

There are many opportunities open to licensed professionals in the fields of cosmetology, esthetics, nail tech, and student

instructor. The Academy prepares all graduates for the licensing exam and entry-level positions in their career.

Additional industry experience could lead to employment as a manufacturer sales/education person, a distributor sales consultant,

or in admissions and financial aid in cosmetology schools. Additional licenses are required to become an instructor in a school of

cosmetology arts and sciences.

GRADUATION REQUIREMENTS

The student may graduate from The Academy when they have successfully completed the Alabama Board of Cosmetology and

Barbering required hours, all assignments, tests, project sheets, and grade sheets. The student must have at least 85% attendance

and an 85% grade average. The student will graduate when all tuition, fees and over-contract charges per the terms of their

Enrollment Agreement have been paid and an exit interview has been completed. A student will receive a diploma when all graduation

requirements are met. HOUSING

Although The Academy does not offer housing, housing may be obtained through www.apartmentguide.com.

EXTRA EDUCATION

Extra education events are available for students at The Academy.

GRADING PROCEDURES The Academy utilizes a 100-point grading scale: Cumulative grades in each 8 week period are 85 – 100% is passing, 0-84% is not

passing. All finals exams must have a grade of 75% or the student will need to retake final. Students are allowed to take the final

up to 3 times with all grades being averaged together, these tests will be a different test each time. If student fails to pass final in

any of the 5, 8 week periods in which the final was failed will have to be repeated and this will automatically put the student 8 days

over contract, unless in foundations then the entire 8 week period will have to be repeated. Grades are assessed for all

instructional trainings. Work habits, appearance, conduct, initiative, cooperation, and attendance are also considered. After the

first 8 week period (foundations) 50% of your cumulative grade is based on clinical classroom services which include all

guest or mannequin work. Our grading system in the clinical classroom is based on the number of services needed for your Level in

which all students once they have completed Foundations start at a Level 1 and the rubric for each service. Project sheets are

given at each module that you will have 8 weeks to finish. You must have a minimum 800 services in 44 weeks which is a

combination of salon services and project sheets. Failure to maintain this will result in dropped from program.

Numerical grades are considered according to the following scale: 95 - 100 EXCELLENT 90 - 94 VERY GOOD 85 - 89 SATISFACTORY 0 - 84 BELOW STANDARDS – UNSATISFACTORY

Incompletes

Incompletes are at the discretion of the educator. When a student is making every attempt to learn a skill or subject but requires

additional time to complete the work successfully, the student may be given an “Incomplete.” The student will receive deadline and

description of work that must be completed from the educator.

Inadequate Grades

A student will be informed immediately after a Satisfactory Academic Progress point if they are weak in an area of study or skill. The

educator will determine a deadline student will have to complete the work satisfactorily. The student will be advised of progress during

the grading period.

CONDUCT

Students may be suspended for poor performance, absence, tardiness, disrespect, or inappropriate behavior including, but not

limited to, bad attitudes. If a student is suspended, the student will be advised about what the student must do to correct the

problem. The Academy’s intention is to establish professional behavior for the likelihood of success in the industry. If a student is

not showing a positive attitude and demonstrates the possibility of limited growth for success in their career, it is the responsibility

of the Academy to inform the student of how the deficiency can be corrected. Suspension or termination from the program is at

the discretion of The Academy and is a possible remedy for any behavior depending on the nature and severity of the conduct in

question.

*Also note that any student who has been suspended may not make up those hours. Suspended hours will

automatically put students over contract.

TERMINATION

The Student’s enrollment may be terminated by The Academy for failure to meet any school policies including, but not limited to,

insufficient progress and nonpayment of tuition. In the case of termination by The Academy, The Student will receive a refund

according to The Academy’s Withdrawal, Cancellation and Settlement Policy. The Academy will only issue an official transcript of

hours to students who withdraw or are terminated prior to program completion when the student has successfully completed the

required paperwork, attended an exit interview, and made satisfactory arrangements for all debts owed The Academy as approved

by The Academy.

COMPLAINT PROCEDURE

If the student has a complaint that needs to be addressed, the student should use the following procedure:

1. The student should submit in writing a signed complaint to an educator, the director, or owner outlining the allegation or nature of the complaint.

2. Upon receipt of the complaint, the educator, director, or owner will: a. Review the complaint b. Respond to the student within 10 days of receipt of the written complaint with a possible resolution c. Schedule a meeting to discuss the complaint and resolution. The student will be provided a copy of this written

record at the time of the meeting. If the problem cannot be resolved through discussion, the student will be referred to The Academy’s complaint committee

consisting of an owner, an educator and an administrative office staff member. The complaint committee will meet within 21

calendar days of the receipt of the complaint and review the allegations. If more information from the student is needed, a letter

will be written outlining the additional information. If no further information is needed, the committee will send a letter to the

student within 15 calendar days stating the steps taken to correct the problem or information to show that the allegations were not

warranted or based on fact.

Complaint Policy for Students Receiving VA Education Benefits-Any complaint against the school should be routed through the VA GI

Bill Feedback System by going to the following link: http://www.benefits.va.gov/GIBILL/feedback.asp. The VA will then follow up

through the appropriate channels to investigate the complaint and resolve it satisfactorily.

FINANCIAL AID CODE OF CONDUCT

(Based on Federal Regulations and the Higher Education Opportunity Act)

Personnel Affected

This code of conduct applies to all financial aid employees and all other employees and agents who have responsibilities with

respect to educational loans or who have contact with guaranty agencies or lenders (either federal or private loan lenders). All

decisions and actions must be consistent with the following principles:

1. Student Choice - All students must be given an equal and fair opportunity to choose from all lenders and loans, federal and private. These choices will not be limited to lenders or loans that are available through or recommended by The Salon Professional Academy. The Academy does not have a preferred lenders list.

2. Student Interests - The Salon Professional Academy will seek relationships with lenders which can provide the best benefits for students such as interest rates, fees, payment terms, and services.

3. Avoidance of Conflict of Interest - A conflict of interest is created when an employee’s personal situation interferes

with his/her judgment which can make him/her unable to act in the best interest of The Salon Professional Academy or

the students attending the institution. All employees must avoid conflicts of interest. No employee shall have any

relationship with any lender guaranty agency nor accept any fees or anything of value other than token marketing items

and nominal conference refreshments from any lender.

The Salon Professional Academy will not accept any service or anything of value from any lender or guaranty agency with exception

of the following services: loan processing materials and financial literacy materials, training or financial aid staff related to loan

processing, entrance and exit counseling services conducted under the supervision of a financial aid officer from The Salon

Professional Academy, emergency staffing services, and/or reimbursement of reasonable costs for domestic travel and training

conferences hosted by lenders and/or guaranty agencies.

Student Privacy

It is the policy of The Academy to abide by the Family Educational Rights and Privacy Act of 1974 (FERPA.) This act guarantees a

student's right of access to the student's personal file and the student's rights to the privacy of that file. When a student turns 18 years

old or enters a postsecondary institution at any age, these rights under FERPA transfer from the student's parents to the student.

Information from a student's file will only be released upon written permission from the student. Written permission is required for

each and every request prior to the release of information. Accrediting agencies and government officials may gain access to a

student’s files without the expressed permission of that student. This policy will be given at orientation and is available at any time

through the Admissions, Financial Aid, or Education offices at The Salon Professional Academy. For any additional information regarding

Code of Conduct

All financial aid employees and all other employees and agents are required to comply with all of the following rules (prohibited

relationships) in this Code of Conduct regarding lender relationships and loans. In addition, they are required to notify the

director/owner if they become aware of facts indicating that there may have been a violation of the Code of Conduct regarding lender

relationships and loans.

No financial aid employee or any other employee or agent, shall act as an employee, consultant or sales representative or

officer for any lender or guaranty agency.

No financial aid employee or other employee shall serve on the Board of Directors (compensated or uncompensated) for any

lender or guaranty agency.

No financial aid employee or any other employee shall serve on any lender or guaranty agency’s advisory board.

No agent of The Salon Professional Academy will be allowed to serve on a lender or guaranty agency’s advisory board

without written approval from the director/owner to ensure the agent will comply with this policy.

HOW TO APPLY FOR FINANCIAL AID

Completing the Free Application for Federal Student Aid (FAFSA) is the first step in securing financial aid. All students must complete this in order to obtain any federal aid, regardless of the financial status of the family. This will determine the student’s eligibility for a wide range of financial aid. There is a year-end deadline of June 30th to complete the FAFSA. We encourage all students to apply early in order to ensure they receive their disbursements within a reasonable time.

To ensure we are able to properly process your FAFSA, make sure your completed FAFSA includes the Salon Professional Academy’s School Code: 042110. This code will be used by the federal processor to ensure we receive your FAFSA results, which will determine your federal aid eligibility. To ensure accuracy, whenever possible, use income and other information from your completed tax return from the previous year, rather than estimated figures. Once your FAFSA results are received by The Salon Professional Academy, financial aid awards will be determined, according to your eligibility. WHO RECEIVES FEDERAL STUDENT AID?

Our most basic eligibility requirements are that you must:

Demonstrate financial need

Be a US citizen or an eligible non-citizen

Register with Selective Service (if you haven’t already and are male between the ages of 18 and 25)

Maintain satisfactory academic progress in a postsecondary school, and show you are qualified to obtain a postsecondary

education by:

TYPES OF FINANCIAL AID AVAILABLE

FEDERAL PELL GRANT Pell Grants provide a foundation of financial assistance that may be supplemented by other resources and don't have to be repaid after graduation. Pell Grants are determined after the financial status of a student is fully reviewed by the US Department of Education. Based on a student's financial circumstances, a specific amount of money is disbursed annually toward the student's education through the Pell Grant. How much you can expect to receive from a Pell Grant is solely based on your financial situation and other criteria.

WILLIAM D. FORD DIRECT STAFFORD LOANS Loans made through this program are referred to as Direct Loans, because eligible student and parents borrow directly from the US Department of Education. You must be enrolled as at least a half-time student to be eligible for a loan. Direct Loans include the following:

Direct Stafford Loans

Direct Subsidized Loans – you must have financial need to receive a subsidized loan. The US Department of Education will pay (subsidize) the interest that accrues on your Direct Subsidized Loan during certain periods.

Direct Unsubsidized Loans – financial need is not a requirement to obtain an unsubsidized loan. You are responsible for paying the interest that accrues on your Direct Unsubsidized Loan.

Direct PLUS Loans (or Parent PLUS Loans) are loans that parents can obtain to help pay for the cost of education for their dependent undergraduate children.

G.I. BILL If you've spent time in the military, you are eligible to use your G.I. Bill toward your cosmetology education. The G.I. Bill was specifically designed for college or vocational education. Certified by State Approving Agency for Veterans’ Educational Benefits. *These approvals are state and school specific and may not apply to all schools. LOAN DISCLOSURES

Student loan information published by the US Department of Education (The Guide to Federal Student Aid) is available in the Financial Office.

NSLDS (National Student Loan Data System) – student loans will be submitted to the NSLDS and will be accessible by guaranty agencies, lenders, and schools determined to be authorized users of the data system.

HOW FINANCIAL NEED IS CALCULATED

1. Apply for federal student aid – Complete the Free Application for Federal Student Aid (FAFSA). For FAFSA on the Web, go to

www.fafsa.gov. Using FAFSA on the Web is faster and easier than using paper. Be sure to have your tax forms from the

previous year available to use when completing the FAFSA. The Salon Professional Academy’s school code is 042110.

2. The Academy will receive your Institutional Student Information Record (ISIR). The Financial Aid representative will review

your ISIR and your Expected Family Contribution (EFC) calculated on your FAFSA application. The Financial Aid

representative will then figure out how much aid you can expect to receive through grants and loans.

This begins with understanding the admissions process, determining the best option to pay for school, and striving to always maintain Satisfactory Academic Progress. We are here to assist students in all aspects of this process, but in the end each student is responsible for his/her own education. Students are expected to pay for their tuition by one of the methods outlined in their enrollment agreement, which is signed prior to beginning classes. Failure to make these payments could result in termination from the Academy. The Salon Professional Academy will not penalize students for delays in aid due to regulations, timing of financial aid awards, or other circumstances which are out of the student’s control. However, if a student fails to take action on items required for aid to be processed, the student will be held responsible and it may jeopardize that student’s ability to continue in the program. It is essential that students watch for correspondence from the school or other agencies which may send notices via e-mail or snail mail regarding financial aid. VERIFICATION Verification is the confirmation through documentation that the information provided on a student's Free Application for Federal Student Aid (FAFSA) is correct. The federal government requires all Title IV approved institutions to verify or confirm the data reported by students and their parent(s) on the FAFSA. The verification process ensures that eligible students receive all the financial aid to which they are entitled and prevents ineligible students from receiving financial aid to which they are not entitled. A student may be selected for verification either by the U.S Department of Education Central Processing System (CPS) or by The Salon Professional Academy’s Financial Aid Office. If you are selected for verification by CPS, there will be an asterisk next to your Expected Family Contribution (EFC) on your Student Aid Report (SAR). You will also be informed of having been selected for verification in the SAR acknowledgement letter, which will be included with your SAR documents. If you are selected for verification, the school Financial Aid Office will notify you via email to the e-mail account you provided to the school upon enrollment, via phone or in person (if you are an active student.) If you are notified by The Salon Professional Academy’s staff that you have been selected for verification, you have 30 days from the date you received notification or up until the first day of class (for new students), whichever comes first, to submit the required documentation. Documentation required to complete verification may include a copy of the most recent tax return transcripts for the student and parents (if applicable), a completed verification worksheet and any additional documents and forms required, depending upon the information the student and/or parent provided on the FAFSA. Students should submit the requested information in a timely manner and by the stated deadline to prevent any delays in the processing of their awards. Failure to submit the requested documents will result in cancellation of all federal financial aid awards. Students are advised to retain copies of any document submitted to the Financial Aid Office. If corrections or updates are required as a result of the verification process, the school will submit the corrections to the U.S. Department of Education and the student will receive a corrected SAR. All documents provided by a student for verification will be retained in the student's file for auditing purposes. The student's Expected Family Contribution (EFC) may change based on the corrections made. If the award changes due to verification, the students will be notified in writing by The Salon Professional Academy. If a student is selected for verification, it is essential that the student provide the information requested or the student's financial aid package cannot be processed. It is the responsibility of the student to:

Think about how much they are borrowing: how the amount of the loan will affect their future finances and what

their repayment obligation means before they take out a student loan.

Sign a promissory note: they are agreeing to repay the loan according to the terms of the note even if they do not

complete their education, cannot get a job after they complete the program, or they did not like the education that

they received.

Make payments on time: they are required to make payments on time even if they do not receive a bill,

repayment notice, or a reminder. They also must make monthly payments in the full amount required by their

repayment plan. Partial payments do not fulfill their obligation to repay their student loan on time.

Continue to pay their loans while waiting for deferment or forbearance approval.

Keep in touch with their loan servicer: notify your servicer when you graduate, withdraw from school, drop below

half-time status, change your name, address, or Social Security Number, or transfer to another school.

ENTRANCE COUNSELING

In addition to meeting with a Financial Aid representative, the student must also complete the required Entrance Loan Counseling prior to receiving any disbursements. This is a one-time requirement and must be completed in advance by using the Federal Direct Loan Entrance Counseling link on the Direct Loan website at: www.studentloans.gov. The student is required to complete the quiz using their name, Social Security Number, and date of birth. Once the student has completed the quiz, the results will be electronically available to the Financial Aid office, and will be placed it in the student’s file. It is required that all Direct Loan borrowers complete Entrance Loan Counseling, even if the borrower has had loan counseling at another institution.

MASTER PROMISSARY NOTE

In addition to meeting with a Financial Aid representative and completing Entrance Loan Counseling, the student must also complete a Master Promissory Note prior to receiving any disbursements. This must be completed in advance and can be completed through the Master Promissory Note link on the Direct Loan website at: www.studentloans.gov. It is recommended to turn off all pop-up blockers to enable successful completion of the Master Promissory Note. In order to complete the Master Promissory Note, the student will need their Federal PIN, driver’s license (or state ID) number, and the names and addresses of two personal references from two different households.

EXIT LOAN COUNSELING

If a student who was awarded a Direct Loan withdraws, drops below half-time enrollment, or graduates, the student must complete Exit Loan Counseling. The exit counseling can be completed online through the Direct Loan website at: www.nslds.ed.gov. The exit counseling will provide the student important information regarding repaying their loan, average monthly repayment amount, deferment, loan cancellation and consequences that may occur due to student loan default. After completing this counseling, the student must present a confirmation of completion. This form will then be placed in the student’s file. Please contact the Financial Aid Office if you have questions in regards to Exit Loan Counseling.

REPAYMENT OF LOANS

There is a set time period after a student graduates, leaves the school, or drops below half-time status before the student must

begin repayment of loan(s). This period of time is called a grace period and gives the student the time to get financially settled and

select a repayment plan. The grace period for a Direct Stafford Loan is six months. PLUS Loans do not have a grace period.

Depending on the type of loan a student receives and the repayment plan chosen, the student may have from 10 – 25 years to repay

the loans. Monthly repayment amounts will depend on the type of loan, size of debt, length of repayment period, and repayment plan

chosen. For more information, go online to www.studentaid.ed.gov/repaying.

LOAN REPORTING REQUIREMENTS

Subsidized and Unsubsidized Federal Loan information will be submitted to the National Student Loan Database System. This system is accessible by guaranty agencies, lenders, borrowers and institutions determined to be authorized users of the data system.

HOW FUNDS ARE DISBURSED

Student Aid Report (SAR)/Institutional Student Information Report (ISIR) Once the FAFSA is complete, the student will receive a Student Aid Report (SAR) via e-mail or regular mail, depending on the method of communication provided by the student. If the student provided the school code on the FAFSA, the Academy will receive an Institutional Student Information Report (ISIR). These reports will detail the information provided by the student on the FAFSA. Verification Additional information may be needed from the student in order to complete the financial aid process. The Financial Aid office will contact the student and inform him/her of any additional information that has been requested. More information on verification can be found on page 37 of the Student Consumer Handbook under the Verification section or by contacting the Financial Aid Office. Award Letter Once all the necessary information is processed and deemed to be accurate, the Financial Aid office can generate the student’s Award Letter. The Financial Aid office will contact the student by phone or by mail with a copy to the student. An appointment to discuss the financial aid award with the student will then be scheduled. At that meeting, the Financial Aid representative will explain to the student what aid he/she is eligible for and any additional steps that must be taken prior to the student being able to receive a disbursement. The student will be also be given a chance to accept a portion or all of the aid provided. Counseling and Master Promissory Note If the student is eligible and accepts Direct Loan assistance, the student must also complete Entrance Counseling and sign a Master Promissory Note prior to being able to receive any disbursements. If the parent of a dependent child is eligible and accepts Parent PLUS Loan assistance, the parent must also complete a Master Promissory Note prior to being able to receive any disbursements. More information on these processes can be requested through the Financial Aid Office. Financial Aid Disbursements Cosmetology students abiding by the Satisfactory Academic Progress (SAP) Policy are eligible for financial aid disbursements (up to their maximum eligibility) at the following checkpoints: 0-450, 451-900, 901-1200, and 1201-1500 hours. Each time a disbursement is requested, the student is required to meet with a Financial Aid representative to sign a Disbursement Acknowledgement Form. The student’s ledger will be attached to the form, which will be signed by the student and the Financial Aid representative or the Director of Education before being filed in the student’s permanent file. A copy is also given to the student.

When the disbursement is received by the Academy, the percentage of tuition and fees owed to the school for that disbursement period will be credited toward the cost of the student’s tuition. If there are additional funds available after this credit, a disbursement check will be prepared and given to the student within 14 days.

Pell Grant: If the student is eligible for a Pell Grant, the Financial Aid office will request the first disbursement during the first week the student begins school.

Direct Loans: If the student is eligible for and chooses to take a Direct Loan, the Financial Aid office will request the first disbursement thirty (30) days after the start of their program.

Parent PLUS Loans: If the parent is eligible for and chooses to take a Parent PLUS Loan, the Financial Aid office will request the first disbursement thirty (30) days after the start of their program. Unlike the other types of financial assistance, if there is an overage after tuition and fees have been credited, the overage check will be sent directly to the parent or student within seven (7) days.

WITHDRAWAL, CANCELLATION AND SETTLEMENT POLICY (REFUND POLICY)

For applicants who cancel enrollment, students who withdraw from enrollment, and students whose enrollment is terminated by The Academy, a fair and equitable settlement will apply.

Applicants not accepted by The Academy shall be refunded all monies paid to The Academy, less the non-refundable enrollment fee of $200. Students utilizing VA benefits are responsible for this enrollment fee.

If a VA student withdraws on or before the first day of class, they will receive a 100% refund, and all refunds due to VA students will be made within 40 days.

If the student (or his/her parent or guardian) cancels their enrollment in writing within 3 business days of signing it, all monies collected by The Academy will be refunded, less the non-refundable enrollment fee of $200, regardless of whether the student has begun attending classes.

If the student cancels their enrollment more than 3 business days after signing, but prior to starting classes, a refund of all monies paid to The Academy, less the non-refundable enrollment fee of $200, will be paid.

The official cancellation or withdrawal shall occur on the earlier of the following dates: The postmark on the written notification The date said notification is delivered to The Academy in person The date of expulsion by The Academy Or 30 days after the last day of attendance (as determined by the school through monitoring attendance at least every 30

days and submission of hours sent to The Alabama Board of Cosmetology and Barbering) Or the earlier of the expiration date of an approved Leave of Absence if the student fails to return or the student notifies

the institution that they will not be returning. If the student begins classes but withdraws prior to course completion (after 3 business days of signing this contract), the

following schedule of tuition earned by The Academy applies:

The refund is calculated based on the student’s last date of attendance. Once the student completes 50% of the scheduled course hours, no refund is due and the remaining unpaid tuition is due to

The Academy. All monies due The Academy must be paid before any transcripts will be released. Any monies due to the student shall be refunded within 45 calendar days of the withdrawal date as defined above whether

official or unofficial. In the case of a disabling illness or injury, death in the student's immediate family or other documented mitigating

circumstances, a reasonable and fair refund settlement will be made at The Academy's discretion. If The Academy cancels a course or program or no longer offering instruction after The Student has enrolled and instruction

has begun, The Academy will at its option: provide a pro rata refund of tuition for all students transferring to another school based on the hours accepted by the

receiving school; or Provide completion of the course and/or program; or Participate in a Teach-Out Agreement; or Provide a full refund of all monies paid.

If permanently closed or no longer offering instruction after The Student has enrolled, and instruction has begun, The Academy will at its option: provide a pro rata refund of tuition for all students transferring to another school based on the hours accepted by the

receiving school; or Provide completion of the course and/or program; or

Participate in a Teach-Out Agreement; or Provide a full refund of all monies paid. A list of all students enrolled at the time of Academy closure including the amount of each pro rata refund will be

submitted to the appropriate agencies. If the course is canceled subsequent to The Student's enrollment, and before instruction in the course and/or program has

begun, The Academy may at its option provide a full refund of all monies paid or provide completion of the program. The Academy does not participate in any teach-out plans with other institutions.

If the student withdraws or is terminated by The Academy prior to course completion, a Withdrawal Fee of $150.00 will be assessed and charged to the student's tuition account. Other miscellaneous charges the student may have incurred (i.e. extra kit materials, books, products, unreturned Academy property, etc.) will be calculated separately at the time of withdrawal.

The Academy expects tuition and expenses to be paid in a timely manner. The Academy provides the courtesy of a collection phone call or written notice to a student when a payment becomes a few days past due. Once a student’s account exceeds 90 days past due and no alternative payment arrangements have been made, the account is subject to collections and attorney, and reported to the appropriate credit bureaus who abide by this policy. All fees incurred due to collection efforts will be added to The Student’s total balance due.

The Academy provides necessary documentation to collection agencies, attorneys, and credit bureaus to assist in collecting payment for past due accounts. Signature from a third party agency is required on documentation provided.

If the student’s promissory note or contract for tuition is sold or discounted to a third party, the third party agrees to comply with The Academy’s Withdrawal, Cancellation and Settlement Policy.

RETURN TO TITLE IV POLICY (R2T4)

The Federal Return of Title IV Funds formula (R2T4) dictates the amount of Federal Title IV aid that must be returned to the federal government or the lending institution by the school and/or the student. The federal formula is applicable to an eligible student receiving federal aid when that student withdraws on or before the 60% point in time within the payment period. If a student does not begin training, the R2T4 formula does not apply. The federal formula requires a Return of Title IV calculation if the student received or could have received (based on eligibility criteria) federal financial assistance in the form of Pell Grants, PLUS loans, and/or Stafford Loans and withdraws on or before completing 60% of the payment period. The percentage of Title IV aid earned is equal to the percentage of the payment period that was completed as of the withdrawal date if this occurs on or before the 60% point in time. The percentage that has not been earned is calculated by subtracting the percentage of Title IV aid earned from 100%. Return to Title IV (R2T4) Paperwork This form is used to determine the amount of federal student funds that must be returned to the Title IV program on the school’s behalf. The Financial Aid Director will complete this form. Returning Funds to the Department of Education If the R2T4 form determines that the school must return federal money, the business office transfers the money back into the school’s federal account. The third party processor is informed of the amount and date that the money was transferred back into the federal account. Collection Policy Tuition accounts may be submitted to a privately contracted collection agency 90 days after a student has been dropped or terminated from the program. If the student re-enrolls, the account will be removed from the collections process. Returning Funds to the Student If it is determined that the student has overpaid for the portion of the program completed, the student will be issued a refund check. The refund will be mailed to the student’s current address unless other arrangements have been made with the Financial Aid Office.

TUITION PAYMENTS

Tuition can be paid by either financial aid, cash, check or credit card. There is a 5% discount if tuition is paid in full at the start of the student’s education. If tuition is not paid in full at the start of the student’s education, the total tuition is divided into 10 equal monthly payments over the education term if financial aid does not cover all tuition costs. Students using VA benefits to pay tuition, will not be charged any late fees or penalties due to delayed disbursement by the VA. Enrollment Fee: $200.00 due when Enrollment Agreement is signed. VA Students are responsible for this fee Cosmetology/1500 hours: Cost of tuition is $18,900.00, which includes textbooks and kit Esthetics/1000 hours: Cost of tuition is $12,600.00, which includes textbooks and kit Nail Tech/750 hours: Cost of tuition is $8,453.00, which includes textbooks and kit Student Instructor/650 hours: Cost of tuition is $8,190.00, which includes textbooks and kit Contract Amendment Fee: A fee of $50 will be charged for any changes or amendments made to the Enrollment Agreement or Financial Agreement not including approved Leaves of Absence. This fee shall be paid prior to the enrollment or financial change or amendment. Students provide their own stationery-type school supplies. Students also purchase professional apparel and shoes that meet Academy requirements.

DEFAULT MANAGEMENT

Default Management is required by the Department of Education for institutions who participate in the Direct Loan program. This is used as a tool to promote student success and in an effort to reduce student loan defaults in this program. There are 9 activities which make this successful: Entrance Counseling, Financial Literacy for Borrowers, Communication Across Campus, Exit Counseling, Timely and Accurate Enrollment Reporting, NSLDS Date Entered Repayment (DER) Report, Late State Delinquency Assistance (LSDA), Loan Record Detail Report (LRDR), Data Review, and Analyzing Defaulted Loan Data to Identify Defaulter Characteristics. The Salon Professional Academy has adopted the Department of Education’s Default Management Plan. We work with Wright International Student Services (WISS) in order to successfully follow this plan in an effort to reduce student loan default rates in the Direct Loan program.

PLACEMENT INFORMATION

The Salon Professional Academy provides each student with graduation and placement information prior to enrollment. The graduation rates are based on the requirements set forth by the Department of Education, whereas the placement rates are based on the information that we provide to our accrediting body annually. These placement statistics will include details regarding the source of the information, the timeframe used in calculating the rates, and the methodology used to compile the rates. Additional information on placement or types of employment obtained by graduates of The Salon Professional Academy can be obtained at any time by contacting the Admissions Office.

ACADEMY POLICIES

Policies

Prior to the signing of the enrollment agreement, each student is given a copy of the Academy’s Policies. Student agrees to abide by

ALL of The Academy Policies including standards of attendance, conduct, appearance, and courtesy to all. All students attending The

Salon Professional Academy must maintain Satisfactory Academic Progress (SAP) which is defined as reaching each checkpoint of

instructional training with an average of 85% GPA and 85% attendance.

Standards

Students at The Academy must be an example of good grooming and should be in the best of health. All state cosmetology laws and

rules are practiced and taught at The Academy. Lack of basic honesty and integrity will result in being dropped from the program.

Attendance

The following attendance policy applies to students in all programs:

Be Prompt! If you are going to be tardy, it is important that you call the school 30 minutes prior to start time depending on

schedule assigned and speak directly to an Educator and inform them that you are going to be late.

Once you arrive on campus report to an Educator before entering class or the salon area. If student more than 3 has tardies in a grading period then they will be suspended for one day and will be automatically

over contract. Attendance is vital to our curriculum and the overall success of our students. Consequently the school has a stringent

attendance requirement. The Curriculum is broken into grading periods consisting of five 8-week periods for full time (5 day program) and five 12-week

periods for part time (3 day program). Includes theory and clinic classes.

1. Foundations - 300 Hours 2. *Chemical Texturizing - 300 Hours 3. *Anatomy and Physiology - 300 Hours 4. *Advanced Color, Cutting and Finishing - 300 Hours 5. *State Board prep/Salon Business - 300 Hours

* Advanced classes can be taken in any order upon completion of Foundations.

If the student misses 30+ hours in any grading period they are required to repeat the period.

FOUNDATIONS is the building block that all other classes including clinic floor is built upon, therefore attendance in foundations is

vital to your success.

There will be evaluations every two weeks. 1. At each evaluation you must have maintained an 85% GPA and 85% Attendance. If at any point during these

evaluations you haven’t met the above criteria the process is as follows : 1) Verbal Warning 2) Probation status (a written formal action plan to rectify subpar progress. Assignment will be given

by the Educator in charge and will be a 500 word limit). 3) Final warning: the student has not met all prior criteria then they will be dismissed from the

program. Each checkpoint will be documented and placed in student file.

ADVANCED CLASSES are essential to completing all the requirements set forth by state board, therefore attendance in these

classes is vital to your success as well.

There will be evaluations half way through, and at the end of a grading period. 1. At each evaluation you must have maintained an 85% GPA and 85% Attendance. If at any point during these

evaluations you haven’t met the above criteria the process is as follows : 2. midway checkpoint

1) Verbal Warning 3. end checkpoint

1) All required projects have been turned in. 2) All models have been completed. 3) All tests and finals have been completed.

4. If at the end checkpoint you have not maintained an 85 in this period, then the Educator will go over the steps required to bring grade to an 85.

Each checkpoint will be documented and placed in student file. Saturdays and Advanced classes are mandatory. Attendance on Saturday and Advanced classes is mandatory for

all students. If a student misses a Saturday or an Advanced class they must provide official documentation to support their absence upon their return to school. The following is also accepted

o A planned and approved LOA (leave of absence). o An Absence Request form submitted and approved (at least seven days in advance with 85% attendance and

85% GPA (see absence request form). If the student is absent without documentation on a Saturday or Advanced Class day they will be suspended for one day.

A student will be suspended for one day if they “NO CALL-NO SHOW” two times in a single 8 week period. A student will be suspended for one day if the “NO CALL-NO SHOW” on Saturday or Advanced theory class. If a student is tardy after any suspension, the student will be advised that further tardiness will result in being dismissed

from the program. Any absences must be reported thirty minutes prior to the beginning of scheduled shift Monday-Saturday.

You must call the school and speak to an Educator to report an absence. Students shall not leave The Academy except for meal breaks. Special and emergency time off must be granted by the Director of Education. If absent the day of an exam, the student will take exam upon returning to school. Exams can be taken in advance for a planned and approved absence.

A student cannot miss more than 270 hours in their enrollment of the Academy. Students may NOT leave The Academy early without permission of an educator.

**Other leave must be requested at least seven days in advance, if the student has an 85% grade average, 85% attendance,

NO tardiness, NO absences, No suspensions, NO disciplinary actions, AND makes up all work missed (see absence request form).

Please remember that taking any time off may put you over your contracted graduation date and result in over

contract charges.

EXCUSED ABSENCE POLICY

Absences for the following circumstances are excused if the student calls 30 minutes prior to their scheduled arrival time:

Illness of student or student’s child Funeral Car accident Doctor appointment Maternity leave or complications of pregnancy

The student must always speak with an Educator or Owner. Phone messages left on The Academy answering machine,

with another student, or with front desk staff will not be accepted as excused.

Other absences may be excused if requested at least 7 days in advance and if the student has NO tardies, NO unexcused absences, NO

suspensions or disciplinary actions, AND makes up all work missed. (See absence request form.)

Please remember that taking time off may put you over your contracted graduation date and result in additional

charges.

UNEXPLAINED ABSENCE

The unexplained absence of a student from The Academy for more than 14 consecutive calendar days shall constitute as a notice of cancellation to The Academy. For purposes of cancellation, the cancellation date shall be the last day of attendance.

TEST/ EXAM MAKE UP POLICY

Special and emergency absences may be granted by the educators. Students are responsible for scheduling Test/Exam make ups. All Test/Exam make up work must be handed in within one week or it will result in a zero. When absent the day of an exam, it must be made up within one week of the students return. If a student does not make up a test or exam within this timeframe, a zero will be given.

OVER CONTRACT CHARGES

If the student does not complete training by the contracted graduation date, an additional fee of $18.00 per hour will be charged until the required hours are completed. VA Students are responsible for these additional fees.

APPEARANCE

Your image must be professional. You are selling beauty services; you must look the part. A fashionable appearance includes clean, pressed clothing in good condition and clean shoes. Your hair, makeup, nails, posture, clothing, shoes, and smile are part of your total look. Your appearance influences the guest. Your hair must be styled and makeup applied before arriving. You need an educator’s permission to have personal services done during school time. Practice good hygiene including daily bathing and the use of deodorant/antiperspirant. Your breath must be fresh; keep breath mints or spray handy. Wear your nametag at all times. If you lose your nametag, please see the Admissions Office to replace it. A fee of $12.00 will be charged to replace nametags.

TIME RECORD POLICY

Students record their hours daily by using the vascular time clock scanner that utilizes the student’s vascular pattern. Students are registered with their vascular pattern and instructed during orientation in the use of the vascular time clock scanner. This is how you record your hours. It’s like an electronic signature. You are responsible for your hours. Students must use their vascular pattern to clock in/out using the scanner at the beginning of their shift and at the end of their shift. A one-hour lunch is automatically deducted, however, if a student leaves the building for any other reason during the day, they must clock out before leaving and clock back in when they return. Students clock in just before the scheduled starting time, but no more than 15 minutes early. To clock in, insert the back of hand into the scanner. Do not hesitate, or it may sign you in and out all at once. Students risk being expelled from school for falsifying time records in any way. Handwritten hours are not accepted. The vascular scanner is the only way to record a student’s hours; no other form of documentation is acceptable.

DRUG PREVENTION PROGRAM & POLICY

The Salon Professional Academy upholds standards of conduct that prohibit the unlawful possession, use or distribution of illicit drugs and alcohol by students either on Academy property or during any Academy activities. The Academy will immediately contact law enforcement officials to report such activities. The Academy believes that the health risks involved with the use of illicit drugs and with alcohol abuse require providing education and referral services for students involved. The Academy will provide such education annually and will refer students when necessary. Area drug abuse information, counseling, referral, and treatment centers will be provided upon request. The Academy may expel students involved in unlawful possession, use, or distribution of illicit drugs and/or alcohol. The Academy will refer such cases to the proper authorities for prosecution. Students may be reinstated upon completion of an appropriate rehabilitation program. There are serious legal sanctions for the illegal use of drugs and/or alcohol. There are also serious health risks associated with such use.

PHONES AND CELL PHONES

Students must ask permission from an educator to use the school phones. Personal phone calls should be limited to emergencies only. If making a long-distance call from The Academy phone lines, student must pay for the call with a credit card. Cell phone use is restricted to the lounge or outside on breaks or lunch periods. Students must turn their cell phone off when they arrive at school and store it in their locker. Cell phones are not allowed in the classrooms or the salon area.

CARE OF TEXTBOOKS, KITS, AND EQUIPMENT

Textbooks and kits are provided. Students must have them in school every day. If an item is lost or broken, it must be replaced promptly. The Director will order replacements for students at cost. Borrowing is not allowed. Students are instructed to lock up their professional tools. Textbooks are a student’s personal, professional library. They should write notes in them to aid in their learning. Keeping their work area (classroom or styling area) clean and neat at all times is imperative. Students must clean up all work spaces after using them. They will be responsible for assigned sanitation duties daily. These duties must be checked by an Educator before leaving each day. School equipment is not to leave the school at any time.

COURTESY

Common courtesy is expected from students and staff. They are expected to use "please" and "thank you." Students address educators and guests properly by using Mr., Mrs., Miss, and Ms. (as they prefer) and their name. They are never to refer to anyone by an unprofessional nickname or by last name only. They are asked to address fellow students by their first names or by their first and last names.

LIBRARY

A library is provided for student use in the back hallway of The Academy. Students may check out books and other materials during the day and take them home overnight. They must be returned the next day. Students are required to have a staff member sign when an item is checked out and when it is returned. Failure to do so may result in in the student being charged replacement fees for lost books and other materials.

HOURS

School hours are assigned by the program taken. A student’s schedule is part of their enrollment agreement and is subject to change. Students may not attend more than 8 hours per day. School holidays include: New Years, Memorial Day, Fourth of July, Labor Day, Thanksgiving, Christmas Eve, and Christmas Day. A student may be sent home for all or part of a day for not following the Academy’s policies. The vascular scanner time clock keeps student’s hours records in minutes. Even one minute late is a tardy!

LOCKERS

Each student will use the locker provided by The Academy to store personal items. Students are provided with a combination lock or padlock that must be returned at the end of their program in working condition or a fee will be charged. Students must keep their locker clean and cannot put anything belonging to The Academy or anyone else in their locker. Nothing should be stored on top of lockers. This includes sodas, towels, etc. Lockers must be cleaned out 7 days from the day they graduate, withdraw, or are dropped from the program.

POSTURE

When working on Cosmetology projects, students must remain standing. This will help their back and posture greatly and looks better. Specialty course students should also use good posture practices.

EXPECTATIONS

Students must be aware that everything they do will be evaluated and grades will be kept. Part of their professional role is looking, acting and feeling the part of a beauty industry professional. By the end of their training at The Salon Professional Academy, they will be expected to perform all skills they have been taught satisfactorily. They will not be allowed to refuse any services an Educator asks them to perform.

FOOD AND BEVERAGES

All eating and drinking must stay in the break room. Water in bottles is allowed in the classroom during class. There is absolutely no food or beverages allowed in the classroom or in the salon areas (except water in bottles, which will be allowed in the classroom during class time.) A refrigerator is in the student break room for student use. Students are asked to label their name and a date on items stored in the refrigerator. Students are asked to please be responsible and clean up after themselves.

STUDENT PARKING

Students must park in the rear of The Academy and away from the building. Parking spaces next to the building are reserved for

staff and owners.

INTERRUPTIONS

When a class is being held in any room of The Academy, it is important that class not be interrupted and we ask that students

please stay in the classroom or salon area where they are assigned for the day. The Academy also asks that students not roam

about interrupting other students in training.

EDUCATOR IN CHARGE

When working on practical skills, either in the classroom or the salon area, students are asked to please work with the Educator

who has responsibility for that area.

REFUSAL OF A GUEST

Refusing a guest is unacceptable and the student will receive an automatic zero on all services that guest received from another

student after refusal and suspension. The Salon Professional Academy is a learning environment and everyone must learn skills

needed to serve every guest.

HARASSMENT

The Academy does not tolerate harassment of any kind (verbal, physical or sexual.) Students involved in any type of harassment

may be dropped from the program.

OBJECTIVES

Cosmetology, Esthetics, and Nail Tech instructional trainings at The Academy prepare the student to perform hair, skin, and nail services

on the public. The cosmetology, Esthetics, and Nail Tech programs offered by the Academy meet Alabama Board of Cosmetology and

Barbering standards.

Goals: The Academy provides education in all phases of the esthetics profession. The graduate will be a knowledgeable, skilled professional

esthetician. The graduate will have the background and skills to pass the state board exams and to work as an entry level esthetician.

Three-Day program 44 weeks – 24 hours/week Description: Manicurist (manicuring) training at The Academy includes theory and practical instruction that prepares the student to perform nail services on the public. The school offers a 750 clock-hour training program in Manicuring that meets Alabama state standards. Goals: The Academy provides education in all phases of the manicurist profession. The graduate will be a knowledgeable, skilled professional manicurist. The graduate will have the background and skills to pass the state board exams, and to work as an entry level manicurist.

Description: Student Instructor training at The Academy includes theory and practical instruction that prepares the student to instruct students in a school

of cosmetology, esthetics, or nails. The Academy offers a 650 clock-hour and a 1500 clock- hour training program that meets Alabama state standards. Goals: The Academy provides student instructor training that includes theory and practical instruction that prepares the student instructor-in-training to teach students in the school. The graduate will be a knowledgeable, skilled instructor. The graduate will have the background and skills to pass the state board exams and to work as an instructor.
