Sepsis Management, Surgical Safety,
Perinatal Safety, and Infection Prevention
• Select harm prevention strategies from peer
hospital presentations
• Apply six key strategies to enhance your
Patient Safety Culture

• 75% hospitals achieved < 5% early
elective delivery rate-
• 20% hospitals reached zero (0) cases for
all three device related HAI, and
28% reached zero for CAUTI
60% reached zero for CLABSI
69% reached zero for VAP
• Sepsis mortality rate aggregate relative
reduction of 28% from 20.24% to 14.48%
saving approximately 3576 lives

Phase 2 PSF Goals
• Reducing Sepsis Mortality by 10%
from 2012 baseline
• Achieving EED rate < = 3%
• Reaching an NTSV C-section rate
of 23.9%
• Reducing of HA C. Diff by 10%
from 2012 baseline
• Achieving a 0% rate for retained
sponge or towel
