In The Beginning....... Family Stabilization (FS) was implemented prior to OCAT – April 2014

Each GAIN office dedicated one specialized unit of six GAIN case managers and one supervisor to FS

Participants/families with destabilizing barriers were initially identified via the existing GAIN Appraisal

Created new FS activity on operating system with various session types to annotate identified barrier(s)

Care Coordinators • Care Coordinators (CCs) are Psychiatric Social Workers who are

employed by the Department of Mental Health

• CCs are embedded in the FS Unit

• CCs consult with FS GAIN case managers in the development of the FS/WtW Plan

• CCs collaborate and coordinate with FS GAIN case managers

• CCs assist FS participants/families to more effectively address their identified barrier(s) through Engagement, Education and Linkage to various community resources

• Provides support/education to FS GAIN case managers

Health Coordinators • Health Coordinators (HCs) are Public Health Nurses who are employed

by the Department of Public Health

• HCs are embedded in the FS Unit

• HCs consult with FS GAIN case managers in the development of the FS/WtW Plan

• HCs will help FS participants/families address health-related barrier(s) that are outside of the scope DPSS and provide a connection to community resources

• The HCs work with the FS Units in the GAIN offices to provide a team approach to FS case management 8

CFSC4Kids Success Story-Amber

• 3 year-old African-American female referred to address daily behavior tantrums.

• Tantrums consisted of screaming, throwing things and hitting.

• History of early trauma and loss.

• At 18 months, Amber and her mother were separated after her mother was arrested.

• Upon her mother's return, Amber and her mother resided in shelters and transitional housing until Section 8 housing became available.

• Initially, Amber was easily triggered and had tantrums in the therapist’s office.

• She had difficulty transitioning out of sessions as she would have a tantrum at end of each office visit.

• Amber learned how to communicate appropriately and self-regulate her mood and behavior.

• Amber successfully completed treatment after one year of working with the CFSC4Kids clinician.

• She was able to express positive emotions, no longer had tantrums, and was able to verbally communicate her needs.

• At school, Amber was able to engage in positive relationships with peers and is no longer having behavioral issues.

Care Coordinator Success Story

• 51 year-old Hispanic male disclosed that in the past 30 days he had thoughts of hurting himself and experienced recurring thoughts of suicide due to feelings of anxiety and depression.

• Onset began after becoming unemployed from a job he held for 23 years.

• This negatively impacted his ability to sleep, stay focused, or experience enjoyment in his life.

• Care Coordinator conducted a suicide risk assessment, validated the participant’s feelings, and provided psychoeducation on the symptoms of anxiety and depression.

• Care Coordinator also assisted the participant in identifying additional factors that continue to motivate him to persevere in the face of adversity.

• The participant attended a clinical assessment with a mental health provider later the same day.

• Care Coordinator provided suicide prevention, social support resources and personal steps to follow should he experience a reoccurrence of severe suicidal thoughts.

• Initially, the participant came to the appointment sad, hopeless, and tearful. By the end of the appointment, he was smiling and expressing hope and optimism about his future with mental health treatment.

• After six months of attending therapy regularly, the participant was ready to look for employment through the Los Angeles County CalWORKs Supported Employment Program offered by the mental health provider.
