Only Electronic billing is provided with all tariffs free of charge. Printed billing is available only as a supplement to an Electronic billing and

the subscriber is charged a fee for having it sent (see Appendix no. 1 Optional Services).

This Price List is constitutes an integral part of the General Terms & Conditions

Misuse of benefits arising from tariff or marketing event

Billing for services and payment methods

Rate for international calls - rate for national calls to fixed lines increased by an international surcharge. It applies to calls with a

national code different than +420.

Billing for services - it is issued in accordance with this Pricelist and prices are always without VAT; VAT is then added according to Act

no. 235/2004 of the Collective about Value Added Tax as subsequently amended.

I. GENERAL TERMS

Business customer - means a natural entity doing business under relevant legislature or a corporate entity. The price conditions and

services set out in this Price List are to be used for business activities or other activities being the reason for the establishment of the

corporate entity.

Price of call - price of all calls and SMS following relevant rates. The price for rates with time units is derived from defined minimum time

intervals. Within a call, each initiated interval is counted.

Misuse means among others:

(a) artificial or automatic generation of calls or messages,

(b) using of the advantage primarily for gaining financial or another benefit for the subscriber or a third party exceeding the legitimate

benefit at the cost of O2,

(c) using the benefit in an extent which exceeds the maximum of estimated generation by subscriber - which is the volume of 10,000 min.

of calls or 10,000 SMS/MMS per billing period (If the billing period of a specific benefit or price within a specific bundle is shorter than a

standard billing period the number of units will be proportionally reduced).

O2 has the right to apply standard charges depending on the tariff VARIO unless the subscriber satisfactorily proves to O2 that this

volume was generated by standard service usage. Any form of misuse is forbidden.

FUP - The volume limit of downloaded and uploaded data. It is counted in 30-day windows which start on the date of activation (date of

last change of the service). Once the window expires the data will be zeroed.

Roaming – is a service that enables using O2 services in the networks of foreign operators subject to roaming agreement. Roaming can

be used in any territory covered by the signal of the foreign operator.

Special rate - rate of outgoing call and SMS - equal for all tariffs. No free unites are applied.

Payment methods - by a collection from a bank account (with the option to set up a minimum amount limit of CZK 500 per one

subscribing number), by a money transfer, or a money order. Payment by a collection from a bank account can be set up for a period of

at least three subsequent billing periods. If a subscriber sets up or cancels a payment by a collection from a bank account, they have to

inform O2 at least 7 business days prior to the end of the billing period for which the payment is to be made for the first or last time. If a

payment by a collection from a bank account does not happen for reasons caused by the subscriber or the bank, it is understood that the

subscriber will cover the billing for provided services in a different way. O2 is then entitled to cancel subscriber’s payment by a collection

from a bank account.

Optional service - a service provided above the framework of the basic service. The optional service can be used based on a separate

order. Optional services, their current prices and methods of their ordering are specified in the Optional Services part, which makes the

first appendix of the Pricelist, and which is always issued on the 1st and 15th day of the calendar month with a maximum validity of 17

days. The Subscriber closes an individual contract with O2 for each use of an optional service; the contract is based on currently valid

terms and conditions.

Call forwarding - it enables forwarding of incoming calls to your Voice Mail or a different subscriber’s number in the Czech Republic

(national code +420). It is possible to set up immediate call forwarding when a call is not connected to the originally called number at all.

Forwarding if a call is not received (the call is not received on the originally called number) or when the called person is not available. For

Monthly flat fee - recurring charges of a relevant tariff.

Free minutes / toll-free calls - free calls up to the defined limit of minutes or specific destination and time of call applied during a given

billing period to national rate of calls subject to charges. This does not apply to calls with a special rate, see Appendix n.1 Optional

services. Unless otherwise specified, free minutes also apply to free calls and are forwarded to the next billing period. In the case a tariff

is used only in part of the billing period (activation, change of tariff or tariff composition during the billing period, account split/merge or

holiday disconnection) only a proportional part of the free minutes can be used.

Free minutes forwarded from a previous period get preference to free minutes of the same type in the current period. Forwarded free

minutes that can be used for Video-calls will be applied (if used for other calls than Video calls) as free minutes in the given billing period.

Basic service - it includes calls and text messages from an O2 mobile network to participating numbers in the Czech Republic as well as

other services included in the monthly fees of the chosen basic tariff or a recurrent package designed for national operation. The basic

service does not include services provided in networks of partner operators (Roaming) if they are not included in the basic tariff.

Connection to O2 - rate for connection to subscriber's numbers of O2 Mobile voice service, Bleskmobil and O2 Family services.

Free SMS / toll-free SMS : toll-free SMS applied in the given billing period to outgoing text messages to all Czech mobile networks.

Unless explicitly otherwise specified, free minutes are forwarded to the next billing period. In the case a tariff is used in a part of the

billing period (for activation, change of tariff, account split/merge or holiday disconnection etc.) it is only the proportional part that can

be used.

Fixed member – a customer’s phone connection in the O2 fixed-line network, integrated in the O2 Team Combi service.

VPN tariff – a basic tariff of the O2 Team Combi service. A Fixed member can choose a VPN tariff Team Profi or Team Nonstop

determining the price of advantageous calls to other Fixed members, Mobile members and the Team Fixed-Line Group. A mobile

member can choose a VPN tariff Team Profi/Team Nonstop determining the price of advantageous calls to other Mobile members, Fixed

members and the Team Mobile Group.

VPN - (Virtual Private Network) – customer’s virtual network created by a connection of customer’s fixed-line and mobile stations.O2 Team Combi (fixed part) – O2 Team Combi service set up at a customer’s phone connection in the O2 fixed-line network. O2 Team Combi (mobile part) – O2 Team Combi service set up at a customer’s phone connection in the O2 mobile network.

Mobile member – a customer’s active mobile station in the O2 mobile network, integrated in the O2 Team Combi service. Team Mobile Group: a group of external members (tariff or prepaid O2 mobile numbers that do not belong to the customer):

– 5 external members for customers with a signed mobile General Agreement with a minimum amount of up to CZK 24,000

– 10 external members for customers with a signed mobile General Agreement with a minimum amount of CZK 24,000 and above. Team Fixed-Line Group: a group of external members placed in a fixed-line network in the Czech Republic that do not belong to the

customer:

– 5 external members for customers with a signed mobile General Agreement with a minimum amount of up to CZK 24,000

– 10 external members for customers with a signed mobile General Agreement with a minimum amount of CZK 24,000 and above.

Additional VPN tariff – an additional tariff of the O2 Team Combi service. A fixed member can choose an additional VPN tariff Team

Fixed-Line determining the price of advantageous calls to the Team Fixed-Line Group.

▪ The Subscriber installing the O2 Business MAX tariffs after 1 July 2011 will be allowed to use the tariffs only as a Special offer of

▪ Tariff changes within the O2 BUSINESS MAX offer can be made at any time. The business customer must commit to using the agreed

services for the rest of the commitment period. Change of tariff to a tariff from a different line can be made after the expiry of 18 months

of committed contract only to a tariff which is offered within the Special offer (discounted tariff) or Special offer. In this case the business

customer must use the agreed services correctly for the period consisting of the remainder of the old and the new commitments.

▪ Free minutes/SMS are not applied to telephone number with O2 Tandem A, O2 Tandem B, O2 Tandem C.▪ The O2 Tandem enables the activation of a free function called Tandem Limit, which makes it possible to set an indicative limit from the

parent SIM card for the daughter SIM in the number of outgoing calls and SMS sent to the O2 network. After the exceeding of the limit,

outgoing calls / SMS will be blocked. The limit does not apply to additional services such as WAP, Infotext, MMS, Games, Logos, Icons,

Melodies or payment transactions (via Premium SMS, audiotex etc.). The checking of reaching the limit is carried out in periods, which

means that that the limit may be exceeded between two check intervals.

INTERNATIONAL CALLS

▪ Subscriber may activate up to maximum of 2 Tandem tariffs





▪ It is not allowed to activate "O2 Star" for "Tandem" cards and data communication tariffs. Neither the combination of O2 Star and O2

Duet is allowed. The total discount for the combination of O2 Tandem and O2 Star on a parent number will not be equal to the total of

both discounts. The bigger one will be applied.

▪ The offer can be used for the maximum of 4 (four) months from first activation. Unconsumed minutes will be forwarded to the next

billing period. Free minutes cannot be used for the following types of calls: roaming, international, NetCall*55, 1188 Information and

Assistance Services, Information and Operator Services, audiotex services, OLO operator services.

▪ The prices of all types of calls including sending of SMS and MMS (within the "O2 Star" service) are by 20% lower than the current price

of a relevant tariff. The lower rate comes into effect from the next day following the activation of the service.

The first service activation in a month is free; each next priced CZK 79,83 (CZK 96,59 incl. VAT).

▪ O2 Top packages cannot be newly activated with Profil and VARIO tariffs since 1st December 2013

▪ It is not allowed to use SMS for payment transactions.

▪ Free units can be applied solely in the territory of the Czech Republic.▪ Unconsumed SMS and MMS will not be forwarded to the next billing period.▪ TOP SMS and MMS will be applied after using free SMS and MMS of the tariff.

▪ Customers may purchase only one O2 Top of the given category per telephone number. It is allowed to combine O2 TOP services of

different categories.

▪ When the activation of the O2 Top service takes place in the middle of the billing period, the charges applied to the service reflect the

percentage part in which the service was in use including free minutes, SMS and/or MMS. ▪ O2 Top services can be activated to the following tariffs: O2 Tandem A, O2 Tandem B, O2 Tandem C (except O2 Top Relax) as well as to

IV. O2 MOBILE INTERNET CONNECTION

▪ The price of one SMS sent within the Mobile internet tariffs is CZK 1.60 VAT incl. (CZK 1.32 VAT excl.)▪ Each subscriber will activate the tariff option which best suits his/her type of technology in use. ▪ Mobile Internet tariffs are provided in the UMTS, CDMA, and Combo options. The Combo option makes it possible to use the service via

a combo modem, i.e. a modem that enables a connection to both CDMA and UMTS networks. Tariffs in the Combo option cannot be used

for an advantageous price.

▪ The service of a fixed-line internet connection is not included in the O2 RA service. The customer can get internet access via an O2

service or via an alternative ISP service. A fixed-line internet connection access needs to be set up prior to the installation of the O2 RA

service. The O2 RA service does not include the installation and running of an AAA server in a customer’s VPN (Radius, Tacacs+).

*2 Monthly per-user fee covers the operation and administration of authentication systems and applications for the administration of user

13) The activation of the “Internet v mobilu XXL”, “Internet v mobilu+ XXL” and “Mobile internet XXL” tariffs will not be allowed after 15

May 2013. The prices and conditions for these tariffs are specified in the Price list of Mobile voice and O2 Mobile Internet, applicable as

of 14 May 2013.

14) From December 1, 2015, the subscribers of archival tariffs will be gradually migrated to the current offered tariffs. The tariffs, where

the subscriber will be migrated, will be announced a subscriber at least one month before in a billing.

6) If the number of the remaining free minutes within the credit (tariff) is lower than the total duration of a call the call will be rated

proportionally based on the percentage of free minutes in the credit (tariff) vs. remaining free minutes.

10) The tariffs of the “O2 NEON” and “O2 Podnikání O2 Mobile voice” offered before 10 April 2013 can no longer be activated or altered

except the activation of “Internet in handset” and “Discount on O2 Internet/O2 TV”. The prices and conditions of the tariffs are shown in

the Price list for business customers of O2 Mobile Voice and O2 Mobile Internet valid as of 10 April 2013.

11) This price list applies to subscribers to “Internet in handset” and “Internet in handset+” (Internet v mobilu / Internet v mobilu+) who

agreed the tariffs offered in this price list before 10 April 2013. Changes to these tariffs made before 10 April 2013 are described in the

Price list for business customers of O2 Mobile Voice and O2 Mobile Internet valid as of 10 April 2013.

12) This price list applies to subscribers to “O2 Mobile internet” (O2 Mobilní internetové připojení) who agreed the tariffs offered in this

price list before 10 April 2013. Changes to these tariffs made before 10 April 2013 are described in the Price list for business customers

of O2 Mobile Voice and O2 Mobile Internet valid as of 10 April 2013.

7) All conditions mentioned in the price list apply to the concluded Framework Agreement (Contract).

9) The subscriber will not be eligible to agreed recurring credit or discount in the case the credit/discount exceed the total of costs

charged by O2 for post-paid services in the given month.

8) This Price List does not contain prices of services for the Closed User Groups. O2 reserves the right to communicate these offers solely

to the relevant groups of users. The price lists of CUG constitute an inseparable part of the General Terms and Conditions and the Price

List of O2 Mobile voice services and O2 Mobile internet.

VI. FINAL PROVISIONS

2) Subscribers asking for the installation of the Service may be asked by O2 to deposit a certain amount of cash for buying goods within

the Special offer or another discount event. This deposit will be automatically returned to the subscriber as long as: all the invoices for

services delivered in three (3) consecutive months following the payment of the deposit are paid in correct amount and time, the

subscriber has no outstanding debt and his/her service has not been reduced or suspended (due to payment delinquency) over the

recent 12 months.

5) All deposits will be returned before/on the expiry of the contract unless used for the settlement of the debt the subscriber may have

with O2.

1) In accordance with the Value Added Tax Act no. 235/2004 Coll. including later amendments, all prices displayed in this Price list are

subject to taxation, currently at the rate of 21%. All prices are displayed in CZK, VAT exclusive. The price list also includes indicative prices

after the addition of VAT.

3) The size of deposit required by O2 (General Terms and Conditions, Article 3.2 - a) and 5.4.8) may reach the maximum of 5 (five) totals

of all services provided in the running billing period + outstanding debt (if such exists). This does not apply when the invoice for services

in the running billing period drops below CZK 500 incl. VAT. The deposit of maximum amount of CZK 50.000 may be required in the case

O2 suspects that the service may be used in conflict with the General Terms and Conditions, in particular, if this may endanger the traffic

in the network, whole or part, or degrade the quality of services provided for other customers (Article 4.2 - c) hereof).

4) In the case there is reasonable suspicion that the Subscriber may not fulfil his commitments under the Contract and the General

Terms and Conditions (in particular when recorded in the SOLUS register or a similar register or misusing services), O2 shall have the

right to ask the subscriber to deposit a reasonable amount of cash up to CZK 10,000. The deposit will be returned based on a written

request of the subscriber at the earliest 12 months from the completion of the Specifications; all this is conditioned by the Subscriber’s

clean payment history (no overdue payments, no outstanding debt, no reduced / suspended service due to payment delinquency in the

recent 12-month period.

ROAMING - VOICE SERVICES

▪ An incoming call with information about roaming prices is free.

▪ Everybody who activates a Roaming service, including the Easy option, has the O2 Eurotarif set up as a standard unless they choose a

different roaming tariff.

▪ The Roaming service in the Easy option makes it possible to call the Czech Republic or in the country where the participant is. It is not

possible to call networks of third countries. Data services, receiving calls and receiving messages are without any limits.

Subscriber up to a defined limit per billing period. Once the data volume is exhausted, all downloaded data will be charged at a rate

based on downloaded data volume (including Data limit).▪ Monthly price applies to each initiated billing period. In the case of cancellation, data volumes that are not exhausted will be forfeited.

It is not allowed to have more than one package of a specific type per billing period. The billing unit is 1 kB. Unconsumed units will not be

forwarded to the next month.▪ The using of packages with SIM cards registered under a reference number is only allowed for SIM cards with active data service in the

Czech Republic and an active Roaming service. ▪ “Europe” data packages apply to all data transferred in the EU countries. The “EU zone” contains all EU member countries plus

Switzerland, Norway, Lichtenstein and Iceland. ▪ Activation and de-activation of packages is free of charge.

Shared roaming data packages

▪ On roaming activation, the Data limit is set up to all Subscribers as a default functionality. The Data limit includes data used in all zones.

The Data limit switches off data roaming when the Subscriber downloads data in a volume exceeding CZK 1554 incl. VAT (CZK 1284.30

excl. VAT). Since 1.7.2014 the limit volume changes to CZK 1650 incl. VAT (CZK1363.60 excl. VAT). When reaching of the Data limit is the

reason why data roaming has been switched off, the Data limit must be cancelled in order to download other data. In a single billing

period it is possible to repeatedly activate and deactivate the Data limit on a free number 720 720 721. The volume limit starts counting

anew with every new Data limit activation or at the beginning of another billing period.

O2 sends the Subscriber a message when the limit is reached 80% and 100%.

▪ The tariffs and packages can only be used in the Czech Republic in M2M devices using the technological platform of

UMTS/EDGE/GPRS/HSDPA/HSUPA over APN i.e. Oneport, Cma, Telemeters, Telemeter 1 or individual APN“.▪ The complementary packages can only be activated as an addition to the tariffs. The Subscriber may voluntarily combine the packages.

The maximum includes one package per type of package. The packages will be renovated for each billing period until deactivated. ▪ The Subscriber’s free units include both download and upload. Non-consumed free units will not be forwarded to the next billing

period.▪ If the tariff “O2 Machine Basic” or some of the complementary packages are activated in the middle of a billing period the whole

monthly fee will be applied.

▪ Once all free MB of data have been consumed (in the Czech Republic) all data traffic in this country will be charged CZK 0.04 incl. VAT

▪ The payment order is realized upon confirmation command in the Internet environment. The maximum price of one

transaction executed by the m-payment method amounts to CZK 1,500 (including VAT).▪ To enter payment orders via m-payment is not automatically set on the SIM card. 02 is authorized to set conditions

which allow the execution of payment transactions, especially the condition of continuous proper fulfillment of

obligation towards O2 for the previous six months.

▪ The number of the Audiotex line call has a form 90X AB CD ZZ (X indicates character of service, AB determines price

of service for the calling customer on a minute or call basis (including VAT), CD ZZ indicate Service Provider.) The total

amount transferred within a payment transaction via the Audiotex line with 900, 906 and 909 prefix is determined

by the call duration, per-minute call rate, minimal relevant call duration and time interval, and for 908 prefix fixed

rate of call regardless of its length.

▪ By sending Premium SMS MO to 90z AB, the Subscriber gives consent to the transfer of an amount/multiple

amounts to the Partner to be confirmed by receiving a "confirmation Premium SMS" (ConfirmSMS) received from

the 8-digit number. The amount transferred on the basis of the Subscriber's order is identical with the last three digits

of the ConfirmSMS received from the Partner. There is no limit to the number of ConfirmSMS, depending solely on

the Partner's offer. The subscriber may stop the transactions and the ConfirmSMS at any time by sending Premium

SMS MO to a number used by the Subscriber for giving a consent in format "STOP SLUŽBA". The termination of the

receiving of the ConfirmSMS and the sending of amounts for services delivered to the Subscriber over a specific

number will be carried out by sending a Premium SMS MO format "STOP ALL" or "STOP". The execution of the order

is free of charge. The sending of the Premium SMS is charged as a standard SMS under the subscriber's tariff.

▪ The command is entered through DMS sending SMS DMS HESLO to 87777, if the participant receives a confirmation

SMS from 87777. Transferred amount to partner is CZK 30 per DMS. A list of topics for individual humanitarian action

▪ Premium SMS are realized by sending offer Premium SMS MO on 7-digit (90z AB XY or 5-digit (90z AB) numbers,

z=type od PRSM service, AB=provider ID, XY=amount to pay in CZK.

V. PAYMENT TRANSACTIONS

▪ The option of executing payment transactions will be automatically activated for on the subscriber’s SIM card, on

their basis is possible pay for goods and services provided by Partners. List of Partners is available on

www.o2.cz/3partner and www.darcovskasms.cz. Tax document will be issued by the service provider at the request

of the user.

▪ Subscribers can activate/deactivate their payment transactions after dialling customer line no. 800 02 02 02 or *11

(from O2 mobile network) or by visiting any O2 brand store. ▪ Payment orders can be entered through Audiotex call, sending SMS (Premium SMS, SMS donation) or by

confirmation order on the Internet (m-payment).
