Personal Client Assistant – designated person to handle your current contract.

Individual Patient Counselor– each Premium package holder is assigned a Individual Patient Manager who organizes

research, medical consultation and take individual care of the patient during his stay in LUX MED facilities.

Telephone Medical Information – owing to this solution the patients calling the hotline with a health problem may receive

assistance any time of the day. The TMI Team is composed of experienced physicians, nurses and paramedics who collect

information from the caller and try to advise them what should be done in a given situation. They also have information

on the pharmacy and hospital duty hours;

Text message reminders of examinations and consultations – owing to them, patients may schedule appointments

without worrying about forgetting about them;

Patient Portal - access to an integrated IT platform (managing the Patient’s file, scheduling and cancelling appointments,

access to test and examination results, communication with a physician, etc.).

LUX MED Mobile application – free mobile application, which allows to: booking, viewing and removal of medical visits

and view schedules of doctors. It also contains information about the location of establishments owned by LUX MED Group.

The application is available for iPhone and iPad and other Android device. For owners of devices with other mobile operating

systems we have Patient Portal, available at: m.grupaluxmed.pl.

Scope of the Health Care Plan

Complex specialist consultations

The medical care package gives the patient unlimited

access to consultations of specialists in an event

of a disease, chronic illnesses, in an emergency setting

and in the case of an exacerbation of a chronic condition.

Specialist consultations consist of: medical history taking,

specialist advice and basic procedures necessary

for making a diagnosis, taking an appropriate therapeutic

decision and monitoring of treatment. Contractor assures

no-referral access to the specialization listed below:

Medical Centre, a Contractor physician can refer the

patient for a consultation to a specialist available in Poland,

who is not employed at Contractor.

Note:

This service does not include Proffesor consultations,

dentistry, rehabilitation or psychiatry consultations,

or consultations of other specialists, i.e. for example:

speech therapist, psychologist (also sex therapist).

Specialist consultations include: interview, specialist advice

and basic procedures necessary for diagnosis, make

The service is limited - You may use the 3 consultations

per calendar year (combined limit for all consultations).

Consultation with a speech therapist does not include

so-called. Subspecialisation speech therapy, including

surdologopedia, neurologopedia etc.

Professor degree physicians available

without referral

Within the medical care package that includes

the “professors’ consultations” service, Patients can benefit

from specialist consultations, focusing on a diagnostic

or therapeutic problem, given by an independent

academics.

A consultation consists of medical documentation analysis,

as well as subjective and objective examination.

Professors’ consultation does not require a prior referral

from

a specialist, and is not connected with any additional

payment. Professors’ consultations are not limited (except

for psychiatric and psychological consultations).

Consultation Outpatient Procedures

Services in the form of procedures that can be performed

on medical by a physician or a nurse during medical

consultation appropriate for the procedure or after medical

consultation in an outpatient setting. The scope

of consultation outpatient procedures depends on the

scope of specialist consultations to which you are entitled

for the subscription fee. With the service is available local

anesthesia (application with the drug): infiltration

or superficial if so required by the simultaneous treatment

with medical indications. If access to consultations with all

specialists is ensured, consultation outpatient procedures

The above procedures are performed within the framework

of a complementary consultation if they are available

at the centre. Medical materials and devices, i.e.: traditional

plaster, wound dressings, bandages, cannula, syringes,

cotton swabs, adhesive stripes, needles, disinfection

agents, surgical sutures and threads, used for the above

procedures, are free of charge. Other materials

and medicinal products used in the abovementioned

procedures need to be paid for by the patient

with a 10% discount.

Non-Consultation Outpatient Procedures

Services in the form of procedures that are not an integral

part of the medical consultation. They may be performed

under local anesthesia and do not require hospitalisation

or operating theatre regime. The scope of non-consultation

outpatient procedures depends on their availability

at the given centre of the contractor. With the service

is available local anesthesia (application with the drug):

infiltration or superficial if so required by the simultaneous

treatment with medical indications. Non-consultation

We offer flu vaccinations and tetanus vaccine as part

of infectious diseases prophylaxis.

Nursing service consisting of giving an injection.

On the flu vaccinations please visit our Medical Centre’s

and our Medical Partnership. We can organize flu

vaccinations in your corporation – for up of 30 employee.

Prevention without a referral

The patient can perform the service once a year without

having referral. The service is available at LUX MED Group

facilities and includes the following tests:

The service encompasses internal diseases, paediatrics

or family medicine exclusively in situations where the

patient is unable to get to the outpatient clinic indicated

by the Insurer, i.e. in cases of sudden onset of an illness

or sudden exacerbation, including exacerbation of chronic

conditions, excluding direct life-threatening situations.

Reasons making it impossible for the patient to get

to the clinic do not include inconvenient access to the clinic,

obtaining a prescription or a medical certificate. A home

visit is an emergency service provided solely on the visit

request day and aimed at making a diagnosis and starting

treatment whereas treatment continuation and follow-up

visits take place at the outpatient clinics indicated by the

Contractor.

It is not possible to freely choose a physician for a home

visit.

A home visit request is accepted or refused by a physician

based on the information provided. Home visits are

available within administrative limits of cities as indicated

for the service by the Contractor.

is possible in the case of sudden illnesses or accidents.

A sudden illness is a condition characterised by sudden

or imminent appearance of health deterioration symptoms

the direct consequence of which may be serious damage

to a body function or bodily injury or death, requiring

immediate medical rescue intervention and treatment.

The decision of dispatching the sanitary ambulance is

made each time by the Dispatcher after a conversation

with the Patient or his/her family. Depending

on the circumstances, the service may be provided

by an internal intervention team, a cooperating team,

or a team of the State Emergency Medical Service System.

The medical team, equipped with specialist means

of transport, medical equipment and drugs enabling

the initiation of medical emergency procedures in the case

of a health emergency, provides medical aid (depending

on the Agreement provisions) at the Client’s registered

office, at locations agreed with the Client or/and within

the area precisely described at www.luxmed.pl,

and if examinations or tests are necessary, transports

the Patient to a LUX MED centre or to the nearest hospital

in the case of a life-threatening condition. This service does

not replace the services provided within the State

Emergency Medical Service System, this service

is unlimited.
