State Of Illinois

Physical and Immunization

Requirements for Students

Physicals are required by the State of Illinois for students entering K, students 6th grade and 9th grade and all new students.

Students who are required to have physical ex-ams will not need a sports exam but sports ex-ams do not work for the required physical exam!

Kindergarten will need: physical form, dental form and eye exam.

All 2nd graders will need a dental exam.

All 5th graders going into 6th grade will need: physical exam; updated immunizations along with Meningitis vaccine; a dental exam.

If your child will need to take any medications during school hours, including over the counter medicine, we must have authorization from you and your child’s doctor on file. WE CANNOT GIVE OUT ACROSS THE COUNTER DRUGS OR ANY DRUG, UNLESS WE HAVE THAT AU-THORZATION ON FILE! These forms must be current, that is, filled out at the beginning of the year. We have these forms in the school office. If you need to fax any information, our fax num-ber is 815-627-8481.

The results from the LEO organized "Grade School Aluminum Can Drive" in support of the Bible

Church's outreach project were: 1st Place- Grade 2 (6,500 cans) and 2nd Place- Grade 5 -(3,600

cans). In total, over 14,000 can were brought to school by the students during April! As a re-

ward, Home Depot donated work aprons and a craft kit for each student in Grades 2 & 5.

Several HS Leos assisted the winning classes build their projects.

Paw Paw Schools Memorial Wall

A memorial wall is being established at the entrance of the High School. Its purpose is for families to remember loved ones

who have attended Paw Paw Schools and for individuals who have an attachment to the schools and want to leave a lasting

legacy. To remember loved ones or leave a legacy, paw prints with names can be purchased starting at $150. The paw prints

with the names and years of life or graduating class year, will be prominently displayed on the wall. The money collected for

the wall will be used for projects that are permanent. If you are interested in purchasing a paw print, contact 815-627-2841.
