The 2018 Summer Institute on the Teaching of Reading

Registration Instructions: Please review carefully – there are separate forms for each institute.

District Registration Form (2 pages): Acknowledges the number of participants who will be attending the

institute from the district and the amount owed the institute – to be completed by an administrator.

IMPORTANT: Please select one administrator as a district contact person with whom we can communicate

and who will be responsible for coordinating your district registrations.

Individual Registration Form: To be completed by the participant – must be approved by an administrator.

Forms are located on the district website: www.paramus.k12.nj.us (under “Programs”).

Please submit the District and Individual forms to reserve your spot(s) for the Institute(s) by: May 31, 2018

Purchase Order: A purchase order must be received by June 29, 2018, to complete registration. Participants will not be allowed to attend the institute(s) if the purchase order has not been issued.
