**** GUARANTEE AND LIMITATION OF LIABILITY **** *********THERE IS A $225.00 MINIMUM CHARGE FOR ANY AUTHORIZED WORK**********Omega Termite and Pest Control Inc. warrants all services and goods supplied hereunder for a period of one (1)year from the date of completion, except that plumbing, glass sealing, any other work undertaken to controlmoisture, and items or services which must be performed periodically shall be warranted for thirty (30) days only.Omega does not guarantee the work done by other contractors. Omega will provide, without further cost tocustomer, such additional services or chemicals as are necessary to maintain the effectiveness of the workspecifically performed for a period of one (1) year except as stated above. Fumigations for drywood termites andbeetles are warranted for a period of (2) years and subterranean termite treatments are warranted for a period oftwo (2) years. Unless otherwise agreed and set forth in writing, the materials, workmanship and equipment usedwill be those chosen by the contractor and will conform to the local building codes and local practice. Prices quotedare subject to acceptance within (30) thirty days. If additional work, beyond that specified in the report, is requiredby the City or County Building Inspector, such work will not be performed under this agreement. A separatequotation will be made if desired. OMEGA TERMITE AND PEST CONTROL INC. is hereby authorized to proceedwith work as noted above. Funds for work shall be disbursed upon issuance of Standard Notice of WorkCompleted or upon demand to the undersigned. Prior to starting any repairs, Omega Termite and Pest Control Inc.will determine the necessity and structure of progressive payments. Any job that totals more than $5000.00 of workwill require a $1000.00 or 10% whichever is less down payment. LIEN NOTICE UNDER THE MECHANICS LIENLAW. (California Civil Code Section 3110 et seq.) Any contractor, subcontractor,laborer, supplier or any otherperson who helps to improve your property but not paid for his work or supplies has a right to enforce a claimagainst your property. ITEM 11 FINDING There is a possibility that fungus and/or subterranean termite damage exists behindthe stucco.

RECOMMENDATION Install test holes through the exterior stucco at various areas around thestructure and inspect the condition of the internal framing. A supplemental report will thenbe issued covering additional findings and job costs, if any. Our charge for this item will be$250.00. Patching the holes made in the stucco is included in this quotation. The patchedstucco will not be an exact match of the existing texture. We will match the existing as closely aspracticable. Painting the stucco is not included. NOTE: Vibrations caused by installing test holescan sometimes knock loose items from the walls or shelves.
