During the meeting, make sure your instructor knows that he/she needs to COMPLETE all instructor sections of the form. It is your responsibility to review the form, make sure all sections are complete, and ensure it is returned to SAS Office. If we do not receive the form or if it is incomplete, we may not be able to provide the services. We will ONLY allow materials that the professor has specifically indicated on the Testing Proctor Form or in an email. We proctor exams in accordance with information provided by instructors. Therefore, if an announcement is made of a change/update in class, we need to be notified by the instructor in writing.

Testing Accommodations Agreement Prior to Testing1. Meet with each instructor for courses that you will be taking the exams in the Student Accessibility Office.Return a completed Testing Proctor Form as soon as possible but at least a week prior to your first test date.

2. Schedule your exams (if there is no conflict with other courses, it should be the same day/time as the rest of theclass.) If that is not possible, the exam date should be scheduled with SAS as soon as possible. Walk-ins MAY NOT beaccommodated. It is important to have a scheduled date/time. If you have not notified our office and arrive 20 minutes afterthe scheduled time, the test will not be administered until written approval has been received from your professor. If you arelate to an exam, you will be allowed to take it, but you will only be allowed the remaining amount of time to take it.

Our hours are from 8:00 am to 5:00 pm, Monday through Friday. Please make sure you have enough time (with your extended time) to be finished by 5:00 pm.

3. Make advance arrangements if you anticipate needing to speak to your instructor during the exam asstaff will not explain, clarify or express opinions concerning materials in the exam.

During the Exam

Cheating or Assisting to Cheat: Cheating, knowingly assisting another in committing an act of cheating, having knowledge of cheating by others and not reporting it, or other form of academic dishonesty. Cheating as used in the Student Academic Code of Conduct includes but is not limited to deceiving, tricking, defrauding, and/or otherwise taking unfair or unethical advantage of a situation to benefit someone’s grade, academic standing or status.
