dren with LUTS were more likely to have had a delayed
resolution of their nocturnal enuresis than fathers of
children without LUTS40.
Additional evidence supporting a potential genetic
component of OAB symptoms is provided by data from
three twin studies in women41–43; these data confirm the
existence of a genetic predisposition to urinary inconti-
nence. A genome-wide association (GWAS) study con-
ducted in women found that mutations in zinc finger
protein 521 (ZNF521), ADAM metallopeptidase with
thrombospondin type 1 motif 16 (ADAMTS16), and
citron rho-interacting serine/threonine kinase (CIT)
genes might have a role in urge urinary incontinence
in women44. A meta-analysis of published studies that
investigated potential genetic markers in men with
LUTS revealed that, although few putative genetic risk
variants have been reliably detected across the various
included studies, having the rs731236 variant of the gene
that encodes the vitamin D receptor reduces the risk of
LUT symptoms, with a moderate level of hetero geneity
observed for this conclusion. In men, vitamin D levels
were not associated with LUTS, but low sero tonin
levels and high adiponectin and glucose levels were
correlated with the occurrence of LUTS45.
Obesity in women is associated with an increased
risk of OAB13. Corticotropin-releasing factor (CRF) is a
known neurotransmitter that has modulatory effects on
signalling in the spinal cord and has been associated with
metabolic syndrome and psychiatric disorders. Mutations
in CRF signalling pathways might be a common factor in
patients with this pattern of comorbid symptoms. A simi-
lar association between obesity and an increased risk of
psychiatric problems also exists in children46,47.
Obesity in children has been linked with an increased
risk of OAB48–50 and faecal incontinence51. A population-
based study conducted in children (5–12 years of age) in
Taiwan revealed that 14.0% and 10.7% of the children
were overweight and obese, respectively, without any
gender disparity being observed48. Obese children had
higher urgency symptom scores than those of children
of a normal weight. Data from a multivariate analysis
showed that significant risk factors for OAB include
younger age and obesity, but not gender; being over-
weight; obstructive sleep apnoea-related symptoms; and
stressful events48. Some urologists in the adult field are of
the opinion that the effects of obesity on bladder func-
tion might be caused by modifications of body structure,
causing a change in the gravitational centre of the body
and in the compression of pelvic structures, similar to the
phenomenon observed in pregnant women.
An association exists between increased urinary
incontinence and obesity in children52,53. Furthermore,
faecal incontinence and constipation are also more likely
to be observed in children with obesity52. Data published
in 2004 revealed an increased incidence of obsessive-
compulsive behaviour in chronically obese boys and
girls, as well as an increased incidence of depression in
chronically obese boys54. This association between obes-
ity, incontinence and constipation might all be linked to
a possible problem with disinhibition in the frontal lobe,
which can, potentially, explain all three phenomena55.
Disorders such as depression, anxiety, ODD and various
other neuropsychiatric disorders, or at least the proclivity
towards such disorders, which then can be activated later
in life is widely known to have a genetic component56.
EpigeneticsCortisol modulates dopaminergic projections in the PFC
with a remarkable degree of lateralization. Dopaminergic
projections to the right hemisphere have an enhanced sen-
sitivity to stressors that are perceived as severe and uncon-
trollable, such as those that arise from social conflict57.
Thus, chronic psychological stress can disinhibit reflex-
ive circuits through a cortisol- induced dysfunction in the
right PFC that is caused by a prolonged activation of the
hypothalamic- pituitary-adrenal (HPA) axis. Wood et al.58
have published urodynamic data that confirm a signifi-
cantly greater frequency of nonmicturition contractions
(P <0.05) and greater bladder wall thickening (bladder-
to-body weight ratio 0.37 ± 0.01 versus 0.32 ± 0.01 in con-
trol animals; P = 0.0006) in socially stressed rats compared
with unstressed control animals. On the basis of these
findings, it can be postulated that the reduction in activity
of the PFC can result in an inability to inhibit the urge to
urinate, and might lead to urge incontinence, bladder-wall
thickening and faecal incontinence.
In a study by Romens et al.59 researchers demon-
strated that these effects of social stress are also relevant
in children, revealing that children who have experienced
physical maltreatment have epigenetic changes in the
NR3C1 gene, which encodes the glucocorticoid receptor
(GR) compared with those who have not experienced
such treatment. Specifically, the maltreated children had
greater methylation of several sites within exon 1F of the

DISEÑO METODOLOGICO
• ENSAYO CLINICO PROSPECTIVO.
• SE RECLUTARON 11 NIÑOS QUE MANTUVIERON URGENCIA URINARIA DESPUES DEL TRATAMIENTO CON OXIBUTININA:
• Inclusión
• CUESTIONARIO OAB-V8 CON UN PUNTAJE MAYOR A 8
• Exclusión
• Alteraciones neurológicas al examen físico
• alteraciones del desarrollo psicomotriz
• alteraciones ecográficas de las vías urinarias.
• Inicialmente, los pacientes fueron tratados con oxibutinina a dosis de 0,3mg/kg/dia durante 2 meses, uso continuo, asociado a uroterapia con medidas comportamentales y dietéticas padronizadas.
• Después de 2 meses de tratamiento los pacientes que aun presentaban urgencia y puntuación en el cuestionario OAB-V8 mayor a 8 puntos, fueron reclutados para iniciar tratamiento con electroestimulacion sacraltranscutanea (TENS)
• Todos los niños y los padres fueron capacitados para llenar los cuestionarios de síntomas urinarios cuestionario OAB-V8 y estreñimiento con la escala de bristol al inicio y al final del tratamiento así como diarios miccionales durante 2 días antes de iniciar el tratamiento y al final del tratamiento donde fueron recolectados frecuencia miccional, volumen promedio de las micciones capacidad mínima y capacidad máxima vesical.
• Además fueron solicitados a todos los niños examen general de orina y ecografía renal y de vías urinarias.
• Se ofrecieron consentimientos informados a los padres de familia y los niños para autorizar la realización de estimulación eléctrica transcutanea sacral(TENS).

procedimiento
• Aplicado con generador neurodyn evolution, FES ,TENS (Ibramed) usando electrodos de superficie reutilizables a nivel parasacral bilateral altura S3 con gel transductor, usando corriente bifásica con una frecuencia de 10Hz pulsos de 700 milisegundos durante 20 minutos por terapia.
• Se realizo TENS 3 veces por semana durante 7 semanas completando en total 20 sesiones.

CONCLUSIONES
• Tazas de curación de 64% en pacientes refractarios a la monoterapia con oxibutinina; y reducción en los síntomas en todos los pacientes, sin efectos adversos.
• La vejiga hiperactiva refractaria a oxibutinina, se presento principalmente en pacientes del genero femenino 73% vs 27%.
• En nuestro trabajo se presento un aumento en el volumen miccional promedio de mas o menos 30 ml.
• Disminución en el numero de micciones diarias de una media de 9 micciones por día al inicio del tratamiento reduciendo la media a 5,3 micciones por día al final del tratamiento, lo cual esta relacionado con la mejora en la urgencia urinaria valor p 0.05.
• Respecto de la enuresis solo 1 de los 3 pacientes que la presentaban mejoro.
• DEMOSTRAMOS QUE LA ELECTROESTIMULACION TRANSCUTANEA SACRAL ES UNA TERAPIA SEGURA, LIBRE DE EFECTOS ADVERSOS, Y QUE DEBE DE SER CONSIDERADA EN PACIENTES EN LOS QUE FALLA LA TERAPIA FARMACOLOGICA INICIAL (OXIBUTININA).
