• Most insurance companies request
the G modifier be used with the CPT
or HCPCS code – to indicate

Rules of the Tennessee State Board of Medical Examiners
Chapter 0880-02 – General Rules and Regulations Governing the Practice of Medicine
Rule Number – 0880-02-16 – Telemedicine Licensure and the Electronically-mediated Practice of Medicine
Definitions – Consultation using communication technology / Electronic-mediated practice of medicine / Established patient encounter / Facilitator / Initial patient encounter / Medical interpretation / Patient encounter / Referral using electronic or other technology
Eight Requirements
Initial patient encounter requires direct face-to-face
Every fourth encounter – face-to-face or once a year which ever comes first
Physician must have medical record prior to diagnosis, treatment or consultation
Distant site physician responsible for documentation of encounter in EHR / EMR
Originating site responsible for compliance with T.C.A. 63-1-117 (Identification)
Distant site physician responsible for determining appropriateness of TH encounter
Distant site provider must be ABMS board-certified or candidate May prescribe controlled substance BUT NOT schedule II
