1. Arrange the names of the students alphabetically regardless of sex.2. Keep your report neat and clean. Sign any and every erasure to indicate its authenticity.3. Complete the information asked for the upper portion of this form4. encircle all falling grades with red ink in each grading period and indicate reason of

failure under “Remarks” during the finals.5. Affix your signature just after the entry of grades for each grading period.6. If a student is marked “OFFICIALLY DROPPED” (OD), where the student officially

dropped at the registrar’s office, state exact date when he/she dropped the course. If astudent simply absents herself from classes and incurs more than the maximum allowable number of absences for the course, such student shall be given a FallingGrade and under ‘Remarks” should be indicated excess absences.

7. NE (No Exam) shall be given to students who failed to take the regular scheduled testduring the final exams. NE shall be entered in pencil. “NE” is only a temporary grade, The final grade may be submitted by the instructor later.

8. Print your name and affix your signature above it after the final grades.9. Submit this report for approval by the Dean not later than two weeks after the Pre-lim

and Mid-term and one week after the Finals.

SFACS Grading System is as Follows:

1.00 = 100 = Outstanding 2.25 = 85-87 = Good 5.00 = Below 74 = Failed1.25 = 98-99 = Excellent 2.50 = 83-84 = Fair NE = No Exam1.50 = 96-97 = Very Superior 2.75 = 80-82 = Satisfactory OD = Officially Dropped1.75 = 93-95 = Superior 3.00 = 75-79 = Passed INC = Incomplete2.00 = 88-92 = Very Good
