The aim of surgical fracture treatment is to reconstruct the bony anatomy and restore its function. According to the AO, internal fi xation is distinguished by precise reduction, stable fi xation, preservation of blood supply, and early, functional mobilization1, 2. Plate and screw osteosynthesis has been established and clinically recognized for some time.

Positioning holeThe majority of plates include positioning holes. They allow minor intra-operative corrections of the occlusion and bone segments and help positioning of the condylar heads.
Matrix LOCK Straight and Anatomic L-Plates, Maxillary Plates, Straight Sagittal Split Plates and SplitFix Plates include positioning holes for precise adjustment and positioning of intraoperative bone segments, to achieve correct occlusion.

Plate and Screw Color CodingThe color coding of implants (plates and screws) in the MatrixORTHOGNATHIC LOCK system helps to identify the level of strength. The color-coding scale for plates and screws is described below.
PlatesGold = 0.8 and 1.0 mm thick plates (maxillary and mandible)
Screws: Locking/Non-locking
Bronze = 1.5 mm self-tapping screws; LOCK and non-locking
Silver = 1.5 mm self-drilling screws; LOCK and non-locking

In the MatrixORTHOGNATHIC LOCK system the plate hole dimensions allow all the screws to be used with all the plates. This makes it a simple and intuitive system in which the screw strength (see color code) can be chosen based on the application (Le Fort I, Sagittal Split Osteotomy or Genio-plasty).

In 1958, the AO formulated four basic principles, which have become the guidelines for internal fixation1, 2. They are:
Anatomic reductionFracture reduction and fixation to restore anatomical rela-tionships. A comprehensive implant and instrument selection offers the ability to address most simple and complex fixation needs.
Stable fixationStability by rigid fixation or splintage, as the personality of the fracture and the injury requires. The MatrixORTHOGNA-THIC LOCK plates and screws are optimized to achieve stable bone fixation.
Preservation of blood supplyPreservation of the blood supply to soft tissue and bone by careful handling and gentle reduction techniques. Instrumen-tation and new gentle implant profiles and edges minimize disruption of soft tissue and preserve vascular blood flow bone healing.
Early, active mobilizationEarly and safe mobilization of the part and patient. The MatrixORTHOGNATHIC LOCK system implants, combined with AO technique, provide stable fixation enough to allow a functional aftercare.

Intended Use and Indications
Intended Use The MatrixORTHOGNATHIC LOCK plate and screw system is intended for use as a stable internal bone fi xation system in orthognathic surgery (surgical correction of dentofacial de-formities).
Indications The DePuy Synthes MatrixORTHOGNATHIC LOCK System is indicated for use in oral, craniofacial and maxillofacial sur-gery such as: trauma, reconstruction, orthognathic surgery (surgical correction of dentofacial deformities) of the cranio-facial skeleton, mandible and chin, and surgical treatment of obstructive sleep apnea.

The graphic case provides a slot with etched screw length markers to facilitate the confirmation of the correct screw length. Choose a screw for plate fixation. Pick up the screw using one of the self-holding screwdriver shafts. Place the screw in the slot as shown in the image. Make sure that the bottom of the screw head rests against the bottom of the counterbore at the lower end of the slot. Read the number adjacent to the screw tip to confirm screw length.
1Confrm length of the drill bits
The graphic case provides a slot with etched drill length markers to facilitate the confirmation of the correct drill bit length. Choose a drill bit. Place it in the slot as shown in the image. Make sure that the stop of the drill bit (A) (shoulder) rests against the inner edge at the lower end of the slot (B). Read the number adjacent to the drill bit tip to identify the drill bit's length.

After the osteotomy has been performed and the new posi-tion of the maxilla has been established, select the appropri-ate plate shape and thickness that best suits the bony anat-omy, treatment objective and the quantity and quality of bone.

Cut and contour the plate according to the bending tem-plate and bony anatomy using the plate cutter and the bending pliers, respectively. Bend the plate between the holes as necessary. Ensure that the plate is adapted to the bony anatomy.
When using locking screws, an exact match is not required. With locked screws, plate stability does not depend on the plate-to-bone contact.
Notes:– Etched lines, in 1 mm increments, to facilitate plate
bending.– When using non-locking screws take care to match the
anatomy.– Lateral portion of Matrix LOCK Maxillary Plates includes a
thinner bar section for adjustment according to the angle of the Le Fort I osteotomy.
Precaution: Confirm that plate positioning, drill bit and screw length allow for adequate clearance of nerves, tooth buds and/or tooth roots, and the edge of the bone.
Warnings: – Do not alter the bend of the prebent plates by more than
1 mm in either direction.– Do not excessively bend the plates as it may produce in-
ternal stresses which may become the focal point for eventual breakage of the implant.

4Optional: Confirm plate position on the bone using the positioning hole
If pilot hole is desired, select the appropriate 1.4 mm drill bit length to allow for the adequate clearance of nerves, tooth buds and/or tooth roots.
Insert the proper length Matrix screws (non-locking) into the positioning hole. Place the plate in the desired position . Insert the screw until seated. Do not fully tighten.
Drill a second hole on the opposite side of the osteotomy and use a locking or non-locking screw to fixate the plate to the bone . Tighten the screw until seated. Do not fully tighten.
Place a second plate symmetrically on the other side of the maxilla, repeating the steps described above , .
If necessary this procedure can be repeated and the plate repositioned.
Maxilla can be positioned as desired and held in place by tightening screws. Continue to step 5.
Notes:– The surgeon can determine whether a left or right plate
will be implanted first.– Self-drilling and self-tapping screws are available.
Precautions:– Confirm that plate positioning, drill bit and screw length
allow for adequate clearance of nerves, tooth buds and/or tooth roots, and the edge of the bone.
– Drilling speed should never exceed 1800 RPM. Higher rates can result in thermal generated necrosis of the bone and an increased hole diameter. 5Fixate plate to the boneIf pilot hole is desired, select the appropriate 1.4 mm drill bit length to allow for the adequate clearance of nerves, tooth buds and/or tooth roots.
Insert the remaining Matrix screws of appropriate length to fixate the plate to the underlying bone. Alternate between osteotomy sides , , , and maxilla sides , , , .
Note: Self-tapping and self-drilling screws are available.
Precautions: – Confirm that plate positioning, drill bit and screw length
allow for adequate clearance of nerves, tooth buds and/or tooth roots, and the edge of the bone.

Perform the sagittal split osteotomy and establish the position of the distal mandibular segment.
Select the appropriate plate shape and thickness that best suits bony anatomy, treatment objective and the quantity and quality of bone.
The straight and curved sagittal split plates, T-plates and strut plates are available for mono-cortical screw placement.
If intra-operative occlusal adjustments are necessary, straight sagittal split plates and SplitFix plates (straight and curved) with self-holding slider are available (see additional tech-nique steps on pages 20 to 25).
Notes:– For bicortical screw placement 1.85 mm Matrix screws
are available in lengths up to 28 mm (position screws). Pre-drill using a 1.4 mm drill bit without stop.
– The 10 – 28 mm length screws have a larger head dia-meter for better visibility and force transmission.

Fixate the curved Sagittal Split plate to the underlying bone by drilling and inserting the proper length 1.85 mm Matrix locking or non-locking screws.
Note: Self-tapping and self-drilling screws are available.
Precautions:– Confirm that plate positioning, drill bit and screw length
allow for adequate clearance of nerves, tooth buds and/or tooth roots, and the edge of the bone.
– Drilling speed should never exceed 1800 RPM. Higher rates can result in thermal generated necrosis of the bone and an increased hole diameter. The detriments of an oversized hole include reduced pullout force, increased ease of screws stripping in bone, and/or suboptimal fixa-tion. Always irrigate during drilling.

Confirm plate position on the bone using the positioning hole
If pilot hole is desired, select the appropriate 1.4 mm drill bit length to allow for the adequate clearance of nerves, tooth buds and/or tooth roots.
Insert the appropriate length 1.85 Matrix screw (non-lock-ing) in the positioning hole in the proximal segment (contain-ing the condyle). Place the plate in the desired position . Insert the screw until seated. Do not fully tighten.
Repeat the procedure for the positioning hole in the distal segment .
Check and position the condyle by adjusting the position of the proximal segment. Tighten the screws once desired posi-tion is obtained.
Insert the remaining screws of the appropriate length by alternating between the osteotomy sites , , , , starting from the side with positioning hole in the proximal segment.
Note: Self-tapping and self-drilling screws are available.
Precautions– Confirm that plate positioning and screw length allow for
adequate clearance of nerves, tooth buds and/or tooth roots, and the edge of the bone.

Contour the plate according to the bending template and bony anatomy using the bending pliers. Bend the plate between the holes as necessary. Ensure that the plate is adapted to the bony anatomy.

Notes:– Etched lines, in 1 mm increments, facilitate bending and
placement.– When using non-locking screws take care to match the

4Primary plate fixation
If pilot hole is desired, select the appropriate 1.4 mm drill bit length to allow for the adequate clearance of nerves, tooth buds and/or tooth roots.
Fixate the SplitFix plate to the bone by drilling and inserting the proper length 1.85 mm Matrix screws in the specified sequence as shown ,,. Screws should be placed mono-cortically.
Note: Self-tapping and self-drilling screws are available.
Precautions:– Confirm that plate positioning, drill bit and screw length
allow for adequate clearance of nerves, tooth buds and/or tooth roots, and the edge of the bone.

5Intraoperative correction of occlusion
Release the intermaxillary fixation and inspect the occlusion.If the occlusion needs to be adjusted, loosen screw 3 in the slider plate as shown.
The distal bone segment can now be shifted horizontally and vertically until the occlusion has been corrected.
Retighten screw 3 in the slider. The process can be repeated as many times as necessary.
Note: Ensure that the desired condylar position has been achieved.
6Final plate fixation
Using a 1.4 mm drill bit to pre-drill, insert the remaining 1.85 mm Matrix screws of the appropriate length in holes 4 to 7.
Remove screw 3 and the slider plate component. Repeat this step on contralateral side. Ensure fixation of the mandible is adequate to withstand the sagittal forces.
