TECH TALK New Training keeps our Anesthesia Techs on top of their game “We are approaching Joint Commission Time. We have a great team and are joint commission ready 99.9% of the time, but we want to make sure we are definitely ready. We are always getting new updates as to what they are looking for and want to make sure we are not deficient areas and that we don’t get pinged on anything.” - Tom Malboeuf While we hear a lot about how terrific our clinicians are, we don’t hear enough about the unsung heroes of our ORs – our wonderful team of Anesthesia Techs led by Clinical Manager Thomas Malboeuf. The Joint Commission conducts periodic visits to hospitals to check that we are in compliance with all regulations and our facilities are in proper order to conduct medical care. The Commission shows up without notice, but there is usually a window of time, beginning in spring, when they show up, so we are entering a period when a visit could happen at any time. Tom Malboeuf believes in keeping his team prepared every day 24/7, so on May 11 he gave a series of trainings to the techs to update them on the tools they use to keep clinical areas, ORS, patients, and fellow staff safe. Tom invited the communications team to the training and we outline below how our techs keep everything running. Firstly there is Materials, Safety, and Data (MSD) for chemicals that details the purpose, proper handling and storage and first aid precautions of each compound. MSDs can be found on the hospital portal as well as on paper at the OR desks on both campuses. It is crucial that Anesthesia Techs are aware of the possible hazards of all the materials, equipment and chemicals that they use on their jobs. The National Fire Protection Association (NFPA) offers a color-coded graphic and instructions identifying flammability, health risks, reactivity and other considerations such as electrical and construction codes. Finally there are the Instructions for Use (IFUs) that list exactly how to use and maintain products. Another consideration is the physical conditions of the medical space, including cleanliness, placement, and organization. The list below includes some of things that techs need to remember to keep our spaces safe and in compliance. 1. Keep all equipment and supplies off the floor. 2. No cardboard boxes allowed in workrooms except interior boxes. 3. Checking for outdates. 4. Keep equipment and supplies low from the ceiling. 18 Inch Rule. 5. Make sure items are organized in a neat manner. 6. Cover extra towels with a sheet. Don’t place on heaters or windowsills. We’ve not sure when the Joint Commission will pop in for a visit, but it’s clear that Tom is keeping the techs on their toes and alert so we will pass with flying colors whenever they show up! Dr. Oriol, Associate Professor of Anaesthesia at HMS, and Associate Dean of Community Engagement at HMS, is a longtime advocate for improving care for underserved communities and reducing medical disparities for people of color. She is one of the founders of the MEDScience Program, an HMS class that introduces local high school students to medicine and anatomy using medical simulation techniques. She is also one of the founders of the HMS Family Van that travels to local underserved communities and provides basic primary and preventive care to residents. Congratula- tions Dr. Oriol for this much deserved recognition for your efforts to provide better care to disadvantaged populations. Dr. Ed Clune receives 2021 Nicholas M. Green, MD Award for Outstanding Humanitarian Contribution Congratulations to Dr. Ed Clune, recipient of the 2021 Nicholas M. Greene, MD, Award for Outstanding Humanitarian Contribution. This award is given annually by the American Society of Anesthesiologists to a member whose career has supported international anesthetic education, care and volunteerism. Dr. Clune joined the BIDMC Dept. of Anesthesia, Critical Care and Pain Medicine in 2017. With department support he moved to Botswana that year as part of the Botswana Global Health Initiative. This program was launched in 2011 to provide education and QI support at Scottish Livingstone Hospital (SLH) in Molepolole, Botswana. Dr. Clune’s goals were to build up critical care capacity at the hospital, bring American residents in for rotations, and help train local clinicians. He quickly got to work, becoming Chief of Anesthesia at SLH and joining the faculty of the University of Botswana, School of Medicine, the only medical school in this country of 2.3 million. Dr. Clune has worked tirelessly to improve the clinical and educational infrastructure in Botswana. At SLH, he supervised visiting residents, set up an ICU with 24-hour coverage, provided anesthesia and critical care services, and filled in where there were gaps in clinical care at the hospital. Botswana has limited anesthesiology services and most providers are foreign nationals. To address this need, Dr. Clune teamed up with BIDMC OB/GYN physician Dr. Rebecca Luckett, who is also in Botswana, to create an anesthesia residency in Botswana that will graduate its’ first group of locally educated doctors in 2023. When COVID-19 hit in spring of 2020, Dr. Clune worked with colleagues to convert the Sir Ketumile Masire Teaching Hospital, a large hospital in Gaborone that was previously empty, into a COVID only hospital. Despite COVID restrictions, Dr. Clune continues to teach, practice, and strengthen the residency program that he helped create. His vision is a self-sustaining program that relies mostly on local initiative with some support from BIDMC and other medical institutions. He spends most of each year in Botswana with his family and has come to love the country and its’ people. Dr. Clune is modest about his own extraordinary efforts and proud of Botswana trainees, clinicians and faculty who have worked so hard to improve medical care for their people. In his own words- “I’d like to spend enough time here to create a program where we help support critical care and anesthesia train- ing but it doesn’t depend on our presence. Overall, Botswana has done an incredible job of setting up a universal healthcare system and impressive access to educational opportunities. The concept of “Botho”, or very simply put “a respect for humanity”, is shared throughout the Southern African region and no more so anywhere than in Botswana. The medical clinicians have a deep commitment to the care of their patients and this is on display everyday on the wards throughout the country.” “Ed is an exceptional humanitarian, but I would like to think that he represents an ethos that all of us in the department share, which is to do our part to make the world a better place. We should all be proud of the way he represents our Department. Please join us in congratulating Dr. Clune for his exceptional work on behalf of global health equity and his unwavering commitment the health of the people of Botswana.” -Danny Talmor Donnell Carter wins Humanitarian Award from American Association of Nurse Anesthetists Congratulations to Donnell Carter, CRNA who was awarded the 2021 Janice Drake Humanitarian Award from the American Association of Nurse Anesthetists (AANA) for his work on behalf of the AANA Foundation and other charitable organizations. Donnell’s efforts to make the world a better place literally extend far and wide – from his charitable work in Africa, to mentoring kids interested in science and helping people lose weight for better health in the US. He has been a huge supporter of the Boston Africa Anesthesia Collaborative run by Eileen Stuart-Shor, RN, PhD, both as an active volunteer and a fundraiser. Donnell has traveled to Liberia to help the nurse anesthetist program at Phebe Hospital and worked with the Liberian CRNAs who came to BIDMC for training. He also used his social media platform to raise money during our holiday fundraisers for the pro- gram and beyond. (He raised over $10,000 for the program over a four-day period!). Here in the US, Donnell works with the Biomedical Science Career Program as a mentor for young men and women interested in science-based careers and also mentors new nurses entering the ICUs and PACUs and stu- dents as part of a Diversity in Nurse Anesthesia program. As a certified master nutritionist and certified personal fitness trainer, Donnell is committed to helping people live healthier and more active lives. He led 44 individuals to lose a combined 644 pounds by creating a “challenge’ and regularly speaks to groups about using healthy lifestyle, wellness, and self-care as a way to preventative health. Donnell says “I’m pretty passionate about this. My goal is to combine 30 years of healthcare experience with my certifications to help people live healthier lives.” A statement from the AANA lauds Donnell and his work: “Our colleagues consider you to be a role model and tireless volunteer, believe you have made this world a better place by being an optimist and viewing challenges as opportunities, and praise your dedication and hard-working nature. New intern inspired by Dr. Nancy Oriol’s work creates medical simulation program for students Dr. Stephanie Wahab, a 4 th year medical student at Medical College of Georgia who joins our department as an intern this summer, got some real inspiration from her recruitment interview with Dr. Nancy Oriol. She created a program in Georgia called the Medical Outreach Curriculum and Simulation (MOCAS) class that uses simulation to help high school students learn subjects such as anatomy and how to respond in medical crises. The program is modeled on Dr. Oriol’s MEDScience Program at HMS that uses simulation to introduce local high school students to medicine and stimulate their interest in medical and science careers. The Georgia program made quite a splash with the local media (see the links below) when they did a session with students that simulated injuries suffered in a car crash and allowed the students to observe as medical personnel responded to the injuries. Dr. Wahab cited Dr. Oriol’s work as the inspiration for the program. She is excited to join our department and recently sent an email to Dr. Oriol to share the news about this program and thank her for the important work she has done over the years: “Much like yourself, I have a special place in my heart for underserved populations - I feel so grateful to have had the chance to speak with you and be inspired by your perspective. Dr. Shahla Siddiqui presents results of study detailing effects of pandemic on frontline workers Dr. Shahla Siddiqui gave an online presentation of her study “Compassion and humanism in the ICU” for the journal ICU: Management and Practices that measured the impact of the COVID-19 pandemic on front line clinical staff. The study used a generalized anxiety and depression scale and some open ended questions to detail the types of stresses these clinicians experienced. In addition to the stress, fear of transmission, and burnout that clinicians suffered, Dr. Siddiqui also discovered that some expressed positive emotions related to their strong sense of duty as clinicians and commitment to helping patients and relieving their suffering. Dr. Siddiqui says: “Empathy and compassion came out on top of people’s concerns and what they thought was important. TECH TALKNew Training keeps our Anesthesia Techs on top of their game

“We are approaching Joint Commission Time. We have a great team and are joint commission ready 99.9% of the time, but we want to make sure we are definitely ready. We are always getting new updates as to what they are looking for and want to make sure we are not deficient areas and that we don’t get pinged on anything.” - Tom Malboeuf

While we hear a lot about how terrific our clinicians are, we don’t hear enough about the unsung heroes of our ORs – our wonderful team of Anesthesia Techs led by Clinical Manager Thomas Malboeuf. The Joint Commission conducts periodic visits to hospitals to check that we are in compliance with all regulations and our facilities are

in proper order to conduct medical care. The Commission shows up without notice, but there is usually a window of time, beginning in spring, when they show up, so we are entering a period when a visit could happen at any time.

Tom Malboeuf believes in keeping his team prepared every day 24/7, so on May 11 he gave a series of trainings to the techs to update them on the tools they use to keep clinical areas, ORS, patients, and fellow staff safe. Tom invited the communications team to the training and we outline below how our techs keep everything running.

Firstly there is Materials, Safety, and Data (MSD) for chemicals that details the purpose, proper handling and storage and first aid precautions of each compound. MSDs can be found on the hospital portal as well as on paper at the OR desks on both campuses. It is crucial that Anesthesia Techs are aware of the possible hazards of all the materials, equipment and chemicals that they use on their jobs. The National Fire Protection Association (NFPA) offers a color-coded graphic and instructions identifying flammability, health risks, reactivity and other considerations such as electrical and construction codes. Finally there are the Instructions for Use (IFUs) that list exactly how to use and maintain products.

Another consideration is the physical conditions of the medical space, including cleanliness, placement, and organization. The list below includes some of things that techs need to remember to keep our spaces safe and in compliance.

1. Keep all equipment and supplies off the floor.

2. No cardboard boxes allowed in workrooms except interior boxes.

3. Checking for outdates.

4. Keep equipment and supplies low from the ceiling. 18 Inch Rule.

5. Make sure items are organized in a neat manner.

6. Cover extra towels with a sheet. Don’t place on heaters or windowsills.

We’ve not sure when the Joint Commission will pop in for a visit, but it’s clear that Tom is keeping the techs on their toes and alert so we will pass with flying colors whenever they show up!

for her work in health disparitiesDr. Nancy Oriol is the recipient of the 2021 Massachusetts Medical Society Reducing Health Disparities Award. Dr. Oriol, Associate Professor of Anaesthesia at HMS, and Associate Dean of Community Engagement at HMS, is a longtime advocate for improving care for underserved communities and reducing medical disparities for people of color. She is one of the founders of the MEDScience Program, an HMS class that introduces local high school students to medicine and anatomy using medical simulation techniques. She is also one of the founders of the HMS Family Van that travels to local underserved communities and provides basic primary and preventive care to residents. Congratula-tions Dr. Oriol for this much deserved recognition for your efforts to provide better care to disadvantaged populations.

Dr. Ed Clune receives 2021 Nicholas M. Green, MD Award for Outstanding Humanitarian Contribution

Congratulations to Dr. Ed Clune, recipient of the 2021 Nicholas M. Greene, MD, Award for Outstanding Humanitarian Contribution. This award is given annually by the American Society of Anesthesiologists to a member whose career has supported international anesthetic education, care and volunteerism.

Dr. Clune joined the BIDMC Dept. of Anesthesia, Critical Care and Pain Medicine in 2017. With department support he moved to Botswana that year as part of the Botswana Global Health Initiative. This program was launched in 2011 to provide education and QI support at Scottish Livingstone Hospital (SLH) in Molepolole, Botswana. Dr. Clune’s goals were to build up critical care capacity at the hospital, bring American residents in for rotations, and help train local clinicians. He quickly got to work, becoming Chief of Anesthesia at SLH

and joining the faculty of the University of Botswana, School of Medicine, the only medical school in this country of 2.3 million.

Dr. Clune has worked tirelessly to improve the clinical and educational infrastructure in Botswana. At SLH, he supervised visiting residents, set up an ICU with 24-hour coverage, provided anesthesia and critical care services, and filled in where there were gaps in clinical care at the hospital. Botswana has limited anesthesiology services and most providers are foreign nationals. To address this need, Dr. Clune teamed up with BIDMC OB/GYN physician Dr. Rebecca Luckett, who is also in Botswana, to create an anesthesia residency in Botswana that will graduate its’ first group of locally educated doctors in 2023. When COVID-19 hit in spring of 2020, Dr. Clune worked with colleagues to convert the Sir Ketumile Masire Teaching Hospital, a large hospital in Gaborone that was previously empty, into a COVID only hospital.

Despite COVID restrictions, Dr. Clune continues to teach, practice, and strengthen the residency program that he helped create. His vision is a self-sustaining program that relies mostly on local initiative with some support from BIDMC and other medical institutions. He spends most of each year in Botswana with his family and has come to love the country and its’ people. Dr. Clune is modest about his own extraordinary efforts and proud of Botswana trainees, clinicians and faculty who have worked so hard to improve medical care for their people. In his own words-

“I’d like to spend enough time here to create a program where we help support critical care and anesthesia train-ing but it doesn’t depend on our presence. Overall, Botswana has done an incredible job of setting up a universal healthcare system and impressive access to educational opportunities. The concept of “Botho”, or very simply put “a respect for humanity”, is shared throughout the Southern African region and no more so anywhere than in Botswana. The medical clinicians have a deep commitment to the care of their patients and this is on display everyday on the wards throughout the country.”

“Ed is an exceptional humanitarian, but I would like to think that he represents an ethos that all of us in the department share, which is to do our part to make the world a better place. We should all be proud of the way he represents our Department. Please join us in congratulating Dr. Clune for his exceptional work on behalf of global health equity and his unwavering commitment the health of the people of Botswana.” -Danny Talmor

Donnell Carter wins Humanitarian Award from American Association of Nurse AnesthetistsCongratulations to Donnell Carter, CRNA who was awarded the 2021 Janice Drake Humanitarian Award from the American Association of Nurse Anesthetists (AANA) for his work on behalf of the AANA Foundation and other charitable organizations. Donnell’s efforts to make the world a better place literally extend far and wide – from his charitable work in Africa, to mentoring kids interested in science and helping people lose weight for better health in the US. He has been a huge supporter of the Boston Africa Anesthesia Collaborative run by Eileen Stuart-Shor, RN, PhD, both as an active volunteer and a fundraiser. Donnell has traveled to Liberia to help the nurse anesthetist program at Phebe Hospital and worked with the Liberian CRNAs who came to BIDMC for training. He also used his social media platform to raise money during our holiday fundraisers for the pro-gram and beyond. (He raised over $10,000 for the program over a four-day period!).

Here in the US, Donnell works with the Biomedical Science Career Program as a mentor for young men and women interested in science-based careers and also mentors new nurses entering the ICUs and PACUs and stu-dents as part of a Diversity in Nurse Anesthesia program. As a certified master nutritionist and certified personal fitness trainer, Donnell is committed to helping people live healthier and more active lives. He led 44 individuals to lose a combined 644 pounds by creating a “challenge’ and regularly speaks to groups about using healthy lifestyle, wellness, and self-care as a way to preventative health. Donnell says “I’m pretty passionate about this. My goal is to combine 30 years of healthcare experience with my certifications to help people live healthier lives.”

A statement from the AANA lauds Donnell and his work:

“Our colleagues consider you to be a role model and tireless volunteer, believe you have made this world a better place by being an optimist and viewing challenges as opportunities, and praise your dedication and hard-working nature. New intern inspired by Dr. Nancy Oriol’s work creates medical simulation program for students

Dr. Stephanie Wahab, a 4th year medical student at Medical College of Georgia who joins our department as an intern this summer, got some real inspiration from her recruitment interview with Dr. Nancy Oriol. She created a program in Georgia called the Medical Outreach Curriculum and Simulation (MOCAS) class that uses simulation to help high school students learn subjects such as anatomy and how to respond in medical crises. The program is modeled on Dr. Oriol’s MEDScience Program at HMS that uses simulation to introduce local high school students to medicine and stimulate their interest in medical and science careers. The Georgia program made quite a splash with the local media (see the links below) when they did a session with students that simulated injuries suffered in a car crash and allowed the students to observe as medical personnel responded to the injuries. Dr. Wahab cited Dr. Oriol’s

work as the inspiration for the program. She is excited to join our department and recently sent an email to Dr. Oriol to share the news about this program and thank her for the important work she has done over the years:

Dr. Sheila Barnett, anesthesiologist and Chief Medical Office at BID-Milton, is active with the American Society of Anesthesiologists (ASA) Committee on Performance and Outcomes Measurement. Dr. Barnett and Dr. Victor Davila from Ohio State Medical Center spoke to Dr. Adam Striker on the ASA’s podcast “Central Line” about their work on the committee that measures anesthesiologists performance and quality in a systematic and objective way. Dr. Shahla Siddiqui presents results of study detailing effects of pandemic on frontline workers

Dr. Shahla Siddiqui gave an online presentation of her study “Compassion and humanism in the ICU” for the journal ICU: Management and Practices that measured the impact of the COVID-19 pandemic on front line clinical staff. The study used a generalized anxiety and depression scale and some open ended questions to detail the types of stresses these clinicians experienced. In addition to the stress, fear of transmission, and burnout that clinicians suffered, Dr. Siddiqui also discovered that some expressed positive emotions related to their strong sense of duty as clinicians and commitment to helping patients and relieving their suffering. Dr. Siddiqui says:

“Empathy and compassion came out on top of people’s concerns and what they thought was important. If people felt there was someone they could speak to and who would under-stand them it was so helpful.”
