EDUCATIONAL PURPOSE OF THE FIELD TRIP Attend Archdiocesan event for Middle School students which serves as Retreat I for Confirmation Preparation. Featuring speakers, comedy, live music, Mass, Adoration, prayer and worship. Please note the following: X Students may wear the school colors or
Knights gear which are church-appropriate and in accord with school dress code for out-of-uniform days.
X Students will bring their lunches labeled with name.
X Students will bring a refillable water bottle and 2 snacks. All reusable items should be labeled with student’s name.
Cost: DUE MONDAY, SEPTEMBER 30 Students not being Confirmed: $40.00 Students preparing to be Confirmed: no additional cost (ticket included in $125 Confirmation fee)
Transportation: Bus – provided by Pilar Transportation, Inc.

I/We understand that this is an educational trip and a valid extension of the classroom experience. In consideration of the making of arrangements by the school, I/we release and save harmless the school, parish, NFCYM, Credit Union 1 Arena, UIC, the Catholic Bishop of Chicago, Pilar Transportation and any and all school or parish personnel or volunteers from any and all liability for any injuries, loss, or other claims arising or resulting from the trip. Note: Teachers will have each student’s Medical & Emergency Notification Authorization for Medical Treatment on the trip. I will abide by the Youth Code of Conduct (see back) as well as school rules while I participate.

Youth participants will:
• Respect other persons and/or property • Attend all scheduled activities, arriving promptly,
and staying for the entire event • Project an image of Christian consideration, sensitivity, and respect to everyone and to the property
around them through language, dress, and behavior • Refrain from inappropriate touching, verbal harassment, bullying and mean-spirited activity • Refrain from actions that could result in injury and/or damage to property • Wear appropriate credentials in order to gain & maintain access to NFCYM events & activities • Keep their personal belongings with them at all times • Be aware of noise levels in lobbies, hallways, and concourses • Maintain the spirit of the event • Report problems of any kind to a trusted adult • Make appropriate gestures of praise, blessing, prayer and/or assistance to other participants
Youth participants will not: • Possess weapons of any kind • Purchase, possess, consume, or distribute alcohol or illegal drugs • Engage in any form of sexual activity or peer sexual harassment • Purchase, download, possess, or distribute pornography • Touch another participant inappropriately or in a secluded area
No minor student may leave UIC Pavilion unaccompanied. In case of illness or a documented request for early dismissal, student must be picked up at the door by a parent/guardian.
