Overview

This section details the purpose and process of induction and in itself is not part of any Induction.

Before working on site all employees and subcontractors are to complete the on line induction. Records of all

inductions are automatically forwarded to and held by TRH Services.

Policy

TRH Services is committed to providing a healthy and safe work environment through a process of

continuous improvement to their safety management systems. Every practicable step will be taken to

safeguard employees, subcontractors, and the public affected by our activities.

A Safety Management System (SMS) is in place to enable compliance with the Health and Safety in

Employment Act, the Electricity Act, relevant Standards, and our commitment to safety in all our activities.

An Operations Manual which incorporates the requirements of the SMS has been prepared for this site. This

covers all of TRH Services procedures and protocols relating to health and safety, training, compliance with

legislation and documentation related to our work activities and infrastructure. The induction forms part of

this system.

It is the responsibility of all employees and subcontractors to look after their own safety, to report hazards, incidents and accidents, and ensure that their actions do not harm others. Employees are to participate in and abide with the Health and Safety System.

This induction is split into different levels based on the following categories.

1. General – Delivery drivers, waste removal, cleaners, regular visitors, farming

2. Windfarm asset work – Road repairs, electrical reticulation, site building work

3. Turbine work – Any person required to work on a turbine including the tower

4. Principle employees – All full, part time or temporary employees of NWL or TRH Services

Each category includes the requirements from each preceding category.

Aim of this Induction

This induction is designed to:

• Detail the site rules and understanding required to be obtained from the on line induction.

• Induct employees, subcontractors, farmers, suppliers and site visitors into the Safety and Environmental requirements of this site.

Expected Outcome of This Induction

• The promotion of safety at work.

• The promotion of zero accidents in the workplace.

• Protection of the environment.

1. General

All employees, contractors, delivery drivers, farm workers or any other non-escorted person must complete

this section.

1.1 Objectives of this Induction

• To ensure a safe and healthy workplace for all employees, contractors and the public on the Te Rere Hau Windfarm

• To ensure we identify hazards and associated risks present in the workplace.

• Specify behaviour that will eliminate or minimise the risks for you, your workmates and the public.

• Specify behaviour which reduces the risk of damage to plant, equipment and the environment.

• To ensure we protect the environment.

1.2 Windfarm Overview

The Te Rere Hau Site consists of 97 wind turbines, with a maximum output capacity of 48.5 MW, sufficient to

power 18,000 homes.

The power produced by the turbines is exported by underground cables to the neighbouring substation for

final connection to the national grid.

1.2.1 Windfarm Operations

The turbines are operated and maintained by TRH Services Ltd. This operation is generally supervised by

the Site Manager or his nominee.

The farm at all times has a designated Site Controller which may be off site outside of normal hours. The

Site Controller has overall control of all activities on site and is responsible for site safety. All turbine control

and active will be coordinated through them.

1.2.3 Health and Safety Representatives

A list of site Health and Safety Representatives is available in the Site Control Room and/or Health and

Safety Notice Board.

1.3 Site Entry Procedures

Everyone must undergo this site induction before working on the site. This induction is valid for 12 months

only.

The only access to site is typically via North Range Road. All individuals must sign in when they arrive before

starting any work activities and out when they depart site. A sign in board is mounted in the site office for use

by permanent personnel and regular attendees to site. A sign in book is available for all others.

With instruction it is acceptable for non-inducted persons to drive from the main gate to the office once all

signage is read.

Inducted farmers and inducted farm workers may sign in remotely using phone with the on-duty Site

Controller with prior arrangement. Inducted farmers and farm workers are not required to sign in and out

when the site is not attended by TRH Services staff, Power Co staff or their contractors.

Non-inducted visitors must be escorted around the site and advised of potential hazards they may be

exposed to.

Any unauthorised entry to site, reported by a third party or witnessed, must be reported to site management

who will fill in an ‘Incident, Accident or Hazard ID report’ as found in the TRH Operations Manual. These will

be investigated by TRH Services staff to ensure staff, contractor and public safety is maintained.

1.4 Safety

1.4.1 Introduction

The personal commitment and involvement of all employees and contractors is essential to establish and

maintain a safe and healthy working environment. All Workers (as defined under the Health and Safety Act

2016) must comply with the laws of the land and are required to observe the site safety rules at all times.

It is a requirement that you must:

1. Take reasonable care of your own health and safety

2. Ensure no act or omission affects others

3. Comply with all reasonable instruction

4. Co-operate with all policies and procedures

5. Stop any dangerous work

6. Be deemed competent and current by your PCBU to perform the work required of you

7. Have all licences necessary to perform the work required of you up to date

The Health and Safety System is reviewed on a regular basis by management and we welcome your

feedback.

1.4.2 Traffic Management Rules

1. The maximum operating speed on site is 30km/h.

2. All vehicles are required to have hazard and/or head lights on when driving on site with the

exception of farmers use quad bikes.

3. Keep as far left as practical at all times.

4. Seatbelts are to be worn at all times when operating vehicles.

5. If you are unfamiliar with the site check the terrain before proceeding.

6. There are no barriers on the corners. Keep your speed down to a minimum when descending to

maintain control.

7. If the tracks are wet, check before proceeding.

8. You are responsible for the safety of yourself, your vehicle, your load and the safety of others on

site. If you are unsure of what you are doing stop and seek help before proceeding.

9. Do not drive or park on the grass unless unavoidable.

10. Park into wind when around site and in exposed areas. Be cautious of tail winds when getting out of vehicles to open gates.

11. Headlights should be turned on in low visibility.

12. North Range Rd is a public road and not controlled by TRH Services. It is recommended you treat this as part of site and note the legal speed restriction of 30kph.

1.4.3 General Site Safety Rules

The personal commitment and involvement of all is essential to establish and maintain a safe and healthy

working environment.

1. Remain 30m clear of running turbines at all times, with the exception of driving past a turbine or where specific procedures allow and have designated safety procedures and exclusion zones.

2. Remain 30m clear of any turbine, running or not, unless authorised and trained to work on and around the turbines and/or associated electrical equipment.

3. When wind speeds reach the mean high wind speed set for site for either the location where work is being under taken or the access road to the area where work is being undertaken work shall cease and all affected personal shall proceed to Windfarm Site Office. The Site Manager and/or Site Controller have responsibilities for determining the need to vacate site if required.

4. Dangerous chemicals are to be handled only by authorised employees and stored appropriately.

5. No unauthorised persons are allowed on site. Notice of a minimum 48 hours is to be given to TRH Services if bringing authorised visitors to site.

6. No animals may be brought onto site with the exception of stock and working animals that are working on site.

7. All persons must be trained and competent for the work being undertaken or under the direct supervision of a trained and competent person

8. All persons must comply with warning signs

9. Children are prohibited on site or in work related vehicles while undertaking normal work duties

10. Smoking in prohibited within 20 metres of any building, vehicle, turbine or other structure and within 20 metres of any flammable materials which may include vegetation at some times of year.

11. All rubbish shall be disposed of appropriately

12. Work and amenity areas are to be kept clean and tidy

13. No working under the influence of alcohol or illegal drugs

14. The consumption of alcohol is strictly forbidden on site at any time

15. No horseplay or practical jokes on work sites

16. No firearms or air rifles are permitted on site without the explicit approval, on a case by case basis, of the General Manager of TRH Services or CEO of NZ Windfarms.

1.4.4 Personal Safety Rules

1. All injuries, accidents or incidents must be reported to the Acting Controller or Site Manager as soon as possible. Necessary first aid or medical treatment is to be obtained. An incident report must be completed and a copy provided to the H&S Rep.

2. Personal protective equipment provided must be worn when required. Safety footwear and high visibility clothing must be worn all times with the exception of the site office area and moving between the office car park and the office. This includes drivers when around site or delivering.

3. All work teams must carry some form of communication either cell phone or radio in case of emergency and be able to contact the Site Controller.

1.4.5 Personal Protective Equipment

Safety footwear and high visibility clothing must be worn by all personnel, subcontractors and site visitors

when outside the site building and car park area or a vehicle, at all times.

Other P.P.E. must be used as required and includes for identified site work the following:

1.4.6 Location of the Hazard Whiteboard

The Site Hazard Board is located adjacent to the Sign-in Board and outlines the temporary events,

associated risks and controls related to site and working on the turbines. It must be read before any work

commences. The Hazard Board has an area for specific updates as they arise. New hazards should be

reported using an Accident, Incident and Hazard ID form.

1.4.7 Site Evacuations

A site evacuation may be required due to High Winds, Snow or Electrical Storms and will be initiated by the

Site Manager or acting Site Controller.

It will be communicated by cell phone or TRH Services staff member driving around to the various work sites

and advising personnel directly. Thus, it is critical we know where all staff, contractors and visitors are at all

times. You will be advised at this time of the required action or assembly area but you must ensure your

work site is safe before leaving including:

• Turning off all equipment

• Close all turbine doors

• Close all covers on exposed electrical equipment

• Secure loose equipment in case of high winds.

• Do not block vehicle access

1.5 Security and Public Safety

Security on the Te Rere Hau Windfarm site is necessary to protect the power station, the general public as

well as those people working on the site. Collectively these procedures contribute to a safe site.

1. All boundary gates onto the farm shall be left shut at all times. The aim is to exclude the public from

driving through an open gate and putting themselves and others at risk on the Windfarm.

2. The person opening the gate is responsible for ensuring the gate is closed. Gates inside the farm

are controlled by the farmer. All gates inside the farm are to be left as they are found.

3. Turbine access is restricted to authorised personnel only.

4. Assess to all electrical equipment is restricted to authorised personnel only.

1.6 Environmental Protection

It is important that we are all aware of the restrictions placed on the site to protect the environment and act

responsibly to protect the environment by:

1. Acting responsibly

2. Not littering or leaving debris on site

3. Minimising damage to grass, vegetation and land

4. Preventing fires, smoking in designated areas only

• Respirators • Overalls

• Wet Weather Clothing • Gloves

• Sunscreen/sunhats • Warm windproof clothing

5. Recycling

6. Ensure that lose objects in vehicles cannot be blown out when doors are open. Open one door at a time

7. Ensure all waste oil and waste items are disposed of correctly

8. Not clearing any vegetation without authorisation

1.7 Public Feedback and Confidentiality

A Public Feedback record is in operation by NZWL. This includes complaints and positive remarks.

All requests for information about any activities on site or about any company involved with the Windfarm

shall be referred to site management.

You are not to make direct statements to the media or divulge any information to any other person about the

activities on site or the companies involved with the Windfarm. Statements are made by specific personnel

within NZWL or TRH Services.

1.8 Drug and Alcohol testing

TRH Windfarm is owned by NZ Windfarms Ltd and operates under their umbrella of policies. NWF is committed to providing a healthy and safe work environment by properly managing the risks to safety, productivity, and NWF’s business reputation, arising from drug and alcohol affected individuals. NWF will provide training, assistance, and rehabilitation to employees where appropriate. Drug and alcohol testing may be conducted on TRH Windfarm as follows:

• Post incident

• Justifiable cause

• Random

Anyone with concerns or wanting further detail should read the NZWF Drug and Alcohol Policy and / or

discuss with TRH Services management.

2. Windfarm Asset Work

Any employee or contractor working on windfarm assets such as roading, buildings, electrical reticulation

equipment, turbines or other windfarm assets must complete this section.

2.1 Additional Site Safety Rules

1. A hard hat must be worn within 30m of the turbines or any working crane.

2. Remain 15m clear of any turbine if maintenance crews are working outside the turbine nacelle.

3. Enough communication devices must be available to allow communication between personnel on and off site and enable communications with the Site Controller.

4. No hot work is permitted on site without an authorised Hot Work Permit unless in a designated Hot Work Area. See the TRH Operations Manual.

5. Keep a minimum of 2 metre away from transformers at all times where at all possible, exemptions

may need to be made for crane setup due to crane pad sizes.

6. Riding on mobile plant unless in a proper seat or correct operating position is forbidden. No

employee shall operate any item of plant or machinery without the appropriate licence, certificate or

training.

7. If you are involved in any crane work, including truck mounted cranes, all lifting equipment used is

required to be appropriate for the task and rated for the weight being lifted. As the operator you are

responsible for what is on the end of your hook.

8. There must be at least two trained first aider on site at all times. All TRH Services staff are trained

first aiders.

9. Take note of your surroundings and the location of the turbine tower and transformer when manoeuvring vehicles near them. Note: If using a mobile work platform or similar in tight quarters ensure a second person acts as a spotter

2.2 Road Repair, Digging and Excavation

1. Any road repair work which involves the removal of material below the original level of the road

surface requires the express permission of TRH Services and PTS to ensure underground services

within the vicinity are located and isolated if necessary.

2. No digging work on site shall take place without the express permission of TRH Services and PTS to

ensure underground services within the vicinity are located and isolated if necessary.

3. All work under this heading must be recorded in MEX history.

4. For any incident involving the PTS network follow the Fault Callout Protocol

2.3 Electrical and Communications Network Contractors

The electrical and communications network owner require any person working on these networks follow the

internal policies of the owner. Refer to Appendix A for a copy of the Power Co Electricity Safety Non-

Negotiables.

2.4 Fires

No fire shall be lit on site without the express permission of the Windfarm Site Manager or their nominee.

2.5 Emergency Procedures

2.5.1 Injured or trapped person(s)

Action by person(s) injured or about to assist injured person(s):

1. Call the Site Controller in all cases and inform them of:

a. Your location.

b. Nature of the emergency.

c. Actions you are about to take.

2. Stabilise the person(s) if the situation allows.

3. If trained to do so and required to preserve life, initiate rescue procedures, otherwise stabilise and

wait for help. Contact on site first aider(s) if the Site Controller is not doing so.

Note: The Site Controller should already be aware of your location, work you are performing, number of

persons, skills, potential hazards and any access issues.

Action by those aiding assist and that are trained to do so:

1. Pick up the Site Emergency Kit from inside the Site Building main door.

2. Pick up Defibrillator and First Aid Kit.

3. Attend accident / incident and be guided by those already in attendance.

4. Your own safety is the first priority.

5. Once all emergency actions are complete and personnel are safe:

a. Inform senior management of the incident

b. Make the site safe.

c. If a notifiable event, do not disturb or clean up the accident scene unless required to extract injured people or prevent further injury. This is a requirement of Worksafe as they have the right to investigate any notifiable event.

2.5.2 High Voltage Emergencies

If you are involved in an emergency involving the high voltage network or including transformers;

1. Keep yourself clear of any exposed cabling and equipment, do not attempt to move or readjust.

2. If a transformer is leaking oil do not attempt to stem the flow. Get as far back as possible.

3. Follow the Te Rere Hau Windfarm Fault and Planned Callout Protocol as published by Powerco Transmission Services.

2.5.3 Site Specific Safety Plans

A site specific safety plan must be completed by all companies working on site before work commences if they will not be under direct supervision of TRH Services staff at all times.

A template is available for use by companies that do not have standard safety plan templates if required.

A company may elect to provide the safety plan using their own internal standards. This is an acceptable option provided it meets or exceeds the requirements of the TRH Services template.

2.6 Risk Management

The management of risks is via the following hierarchy of control:

1. Eliminate - the hazard from the workplace.

2. Minimise - the risk of harm from the hazard.

The process used to eliminate or minimise risk is:

1. Identify hazards

2. Assess these hazards

3. Implement a hierarchy of control:

a. Substituting

b. Isolating

c. Engineering controls

d. Administrative controls

e. PPE

4. Review and maintain effectiveness

Staff and subcontractors are expected to monitor the hazard board and attend regular safety meetings.

Note: It is the responsibility of everyone on site, no matter whom, to control or report any hazard, so

as to prevent damage or harm to people, plant and equipment or to the environment.

2.7 Farm Operations

The TRH Windfarm covers five separate properties which are in use as operational farms, these farms co-

exists beside all our activities. We must ensure that our impact is minimised.

Respect their property and their stock:

• Do not drop or leave rubbish lying around.

• Do not touch any fence without prior approval.

• Do not drive on pasture areas unnecessarily, stay on the tracks

• Gates shall be left as they are found.

• If stock are obstructing the road move forward at a low speed. Do not use your vehicle horn in an

attempt to move the stock

Report any farm related problems immediately to the Site Controller who will liaise with the Farmer.

2.8 Work hours

Generally hours of work will be between the hours of 7:00am to 4:00pm on week days. If scheduled work is

planned outside these hours, the express permission must be obtained from TRH Services prior to

commencement of work.

Any unscheduled work outside of normal working hours must be communicated to the site manager before

attending site.

2.9 Archaeological Site Accidental Discovery Protocol

NZ Windfarms Limited aims to preserve both known and unknown archaeological sites from damage or loss

at the TRH wind farm site.

As required by the TRH wind farm resource consent, an Accidental Discovery Protocol is in place. All

contractors should be aware of the Protocol when undertaking the Site induction process.

If evidence of Maori artefacts (taonga), archaeological evidence e.g. pre 1900 structures, food storage or

preparation areas or Human remains (koiwi) are found do not disturb the site but report the find to

management. There is a specific reporting procedure in place for dealing with this type of discovery.

3. Turbine work

3.1 Work at Heights

Everyone climbing a turbine to perform any work must be trained and competent or be accompanied by

trained climbers in accordance with Workplace Safety of the Windfarm Operations Manual.

For accompanied work the following safety rules should be noted in addition to all other site rules:

1. All non-Routine turbine work as defined in Workplace Safety of the Windfarm Operations Manual

must have a Work Order and associated Procedures. The Work Order being the Permit To Work.

‘No Work Order – No Work’

2. Lock Out / Tag Out procedures as defined in Lock Out & Tag Out of the Windfarm Operations

Manual must be followed.

3. No one must remain in the tower base while another is climbing or the tower top trap door is open or

any equipment is being lifted or lowered.

4. All loose items must be secure before climbing, e.g. cell phones, keys, wallet, etc.

5. Appropriate PPE must be worn for the work to be undertaken

6. The safety bag must the first item lifted after the first climber

7. All tower base signs must be read before climbing

8. Only trained personnel may open external nacelle hatches

3.2 Additional Site Safety Rules

1. No person shall remain in a turbine during an electrical storm or imminent threat of such conditions.

2. Ensure tower doors are kept shut whenever the turbines are unattended. Check the lock is secure

when closed.

3.3 Hot Work

No hot work is permitted inside an erected Nacelle in any circumstances.

Hot work may be performed outside of the Nacelle with an authorised Hot Work Permit, see TRH Operations

Manual.

4. Employees

This section is for all TRH Services staff and monitoring staff only. New Zealand Windfarms staff that are

involved in operational activities must also be inducted to this section.

4.1 Site safety

It is expected that all employees lead safety on site and report and stop any unsafe activities.

All TRH Services employees will be expected to have a good working knowledge of the Windfarm

Operations Manual.

Employees must be trained as documented in the Windfarm Operations Manual before performing

unsupervised work on turbines.

Employees will be trained and signed off to documented competencies and appropriate training sheets will

be issued.

4.2 Security and Public Safety

The site building has an alarm fitted with the aim to protect the control room, servers and SCADA system.

The control room, workshop and outside doors must be locked each night.

• Each employee shall be issued with a key and individual alarm code for the site building if

appropriate. These are to be kept confidential.

• Entry into the site office and workshop during the weekends requires that the alarm monitoring

company is rung when the alarm is unset and voice code is given. The approximate time on site

(when alarm will be reset) should also be given to alarm company and mobile phone contact ,so any

extended period on farm will result in a “check up“ call.

• The alarm should be set by the last person leaving site each day using their personal number code.

• The alarm must be unset at the start of each day. Entry to the site prior to 6.00am in the morning will

require a call to the security company on 0800 111 238 to verify who you are. A voice code will be

requested. The voice code will be changed on a regular basis.

4.3 Daily Planning Meetings

Daily Planning Meetings are held by the Site Manager or other nominated person. These cover the work

activities planned for the day and who is the job leader. They are also conducted to stimulate awareness of

all matters concerning safety, work practices, plant and method improvements. Safety issues raised in these

meetings shall be investigated and actioned as appropriate. A Hazard ID form shall be completed for any

new Hazards identified which shall be documented on the Site Hazard Register.

Staff are encouraged to submit hazard forms to the Site Manager or H&S Rep so that hazards and unsafe

conditions can be investigated and appropriate controls put in place.

The Site Manager or other nominated person is responsible for updating anyone arriving to site of any

warnings or hazards which that person may be exposed to if this meeting has concluded. The responsibility

for updating new arrivals can be handed from the nominated person to the Site Manager if the Site Manager

arrives to site after the conclusion of the meeting.

4.4 Toolbox Meetings

Toolbox meetings and minutes will be completed for any work requiring more than a 2 person team. All other

turbine work must have a Workorder which includes the tool box meeting checklist. The Job Leader is

responsible for controlling access to the work site and updating anyone arriving to the work site of any

warnings or hazards which that person may be exposed to if this meeting has concluded.

4.5 Emergency Procedures

In the event of an emergency an inducted person should contact the Site Controller as per section 2.5 of this

induction. Is this event:

1. Call 111 and ask for ambulance in all situations, they will require as much information as possible

and will determine what other services are required and will organise all other services. They will

need the following information:

a. Nature of the emergency.

b. Which Windfarm the incident is on and you will need to state it is Te Rere Hau on 376 North

Range Road via Pahiatua Track.

c. If requested, the GPS co-ordinates of:

i. The Main Entrance : S40° 23’ 44”, E175° 44’ 02”

ii. The Site Office : S40° 23’ 33”, E175° 43’ 54”

d. Type of accident.

e. How serious.

f. Number of people.

g. Type of injuries (do not mention names).

h. Nature of access.

i. What you are planning to do next in regards rescue, stabilising or moving the injured

person(s).

2. Organise assistance from any on-site personnel, which may include you, but be mindful of the need

to remain in communicate with and co-ordinate emergency services.

3. Outside of normal hours text all staff to attend site urgently using the SCADA Global Command –

Emergency Call Out facility.

4. Stop all turbines. (If a helicopter is called we need to ensure there is no danger to them from rotating

turbines.)

5. Be guided by emergency services and may include:

a. Meeting them or organising someone to meet emergency services at the main gate.

b. Giving turbine Lat/Longs.

c. Organising additional TRH Services staff to site if outside normal hours.
