Contract for the 2021-2022 School year
Materials required to start the first day of school:
1. Lunch box and reusable drinking bottle and containers (used for hot lunches, If applicable) with
child’s first and last name written on it with a permanent marker.
2. Two complete outfits (pants, shirts, underwear, and socks) in a bag with your child’s first and last
name on it, in case they have bathroom accidents or get their clothes dirty.
4. Velcro or slip-on shoes: Children are responsible for taking off and putting on their own shoes.
Please provide them with shoes they can independently put on themselves. Students must always
wear socks.
5. Backpacks : Please provide your child with a child-size backpack in which they may store their papers or
school supplies, as well as a lunch bag/lunch box. Please provide a folder for their work in the backpack.
6. The completed application packet, signed contract, and copy of the child’s immunization record.
Lunch snacks
Parents are responsible for providing lunch and snacks for their children each day. Healthy, well-balanced
snacks, meals, and beverages should be sent each day, like nutrition and good choices are an important part of
our educational program and your child’s development. Be sure to offer a variety of choices in case they do not
like certain foods and to ensure they get enough to eat.
Late fees:
Late fees of $15 will incur for every fraction of (1-15 minutes late). You will receive an invoice for late fees each
day. Late fees are due no later than 7 days in cash from the day in which you receive the invoice, and the due
date is indicated at the top.
Termination of agreement
- This contract may be terminated by either parent guardian or provider by giving one month notice
in advance of the ending date
- Payment by parent /guardian is due for the notice period whether or not the child is brought to the
provider for care
- The provider may terminate the contract without giving any notice if the parent guardian does not
make payments when due
The provider may terminate the contract without giving any notice without a refund of their
monthly payment.
Absence policy
- Regular fees are still due regardless of child absence due to illness or vacation
- Parents must call the provider proper to the usual start time if the child is to be absent
- If a four week written notice is given prior to vacation, only half of the regular fees will be charged for
no more than one week. This benefit is only offered once per the calendar year.
Payment prices might vary each semester.
