    Secure valves on all pipelines. Flooding of tank with l iquid. Opening valves. Secure valves and hydraulic pumps.

    2

    Check tank atmosphere. Lack of oxygen, Hazardous

    atmosphere.

    Poor venting, Faulty gas detectors. Sufficient ventilation, Calibrate gas detectors.

    Prepare rescue equipment. Unexpected emergency situation. No rescue equipment. Prepare rescue equipment, Standby personnel,

    Communication.

    Enclosed space entry permit. Unawareness of procedures. Failure to follow procedures.

    Complacency.

    Follow standard operating procedures. Copy of

    Permit OPS-66 available to VMT, OOW, EOW

    and posted in Notice Board on A-Deck.

    Initial inspection. Slips trips and falls/pockets of gas. Improper climbing, No gas detector,

    No communication, No ELSA and

    No Emergency Light.

    Safe climbing, Communication, Personnel gas

    detectors. ELSA and Intrinsically safe

    Emergency Light available. Continuos

    monitoring of Atmosphere during Tank Entry.

    6 Tank Entry and

    Inspection of pipes and bottom.

    Gas pockets, sludge removal

    generating gas and Oxidation, slips

    Trips and Falls. Fatigue. Improper

    Head Count.

    Tripping, on slippery surface and

    dark corners. Not following

    procedures like: Tank Entry on hour

    of Darkness in heavy weather. No

    Tank Entry Cards.

    Move slowly, watch steps, use light. Safe

    climbing, Communication, Personnel gas

    detectors. Follow procedures: No tank Entry

    during darkness and heavy weather. Report to

    supervisor if not feeling well and come-out from

    the Enclosed Space. Use Tank Entry Card.

    7 Stepping-Out Enclosed Space. Eye Injury- Flying of Air Borne

    Particles (saw dust and falling rust)

    Removing PPE before actually out

    of Enclosed space. Not Removing

    Keeping PPE at all times.

    Situational Awareness. Inform OOW when

    OPS-55 (2-02)Required by WPR-01-02
