TEXAS ACADEMY OF FAMILY PHYSYCIANS MINUTES OF THE MEMBER ASSEMBLY LUNCH MEETING

FRIDAY, NOVEMBER 4, 2016 WESTIN GALLERIA DALLAS – DALLAS, TEXAS The meeting was called to order by Ajay Gupta, MD, President and Chair. Parliamentarian Rebecca Hart, MD reported that there was a quorum. The minutes of the last meeting on Friday, November 13, 2015 in The Woodlands were approved as written. Dr. Janet Hurley provided an update on TAFP including membership, advocacy and educational initiatives.

ELECTION OF LEADERS Dr. Gupta introduced the slate of candidates for officer, Board of Directors and AAFP delegation positions and made a call for additional nominations from the floor. The Assembly heard speeches from Dr. Emily Briggs, Dr. Grant Fowler and Dr. Oscar Garza, candidates for the At Large seats on the Board of Directors and Dr. Angela Cade and Dr. Ike Okwuwa for the New Physician seat on the Board of Directors. After another call for nominations, delegates voted via a paper ballot for all positions. Ballots were collected and results were tabulated under the supervision of monitors from the TAFP Nominating Committee. The following members were elected for terms to being on Saturday, November 5, 2016.

Standing Rules for TAFP Member Assembly

Adopted November 2016

These Standing Rules of the TAFP Member Assembly can be amended by a majority vote at the start of each meeting of the Member Assembly.

1. The TAFP Member Assembly shall be convened once a year during the TAFP Annual Session and Primary Care Summit at a time and place to be determined by the TAFP CEO. Admission to the sessions shall be limited to members and other CME attendees who display a badge issued by TAFP.

2. TAFP’s President shall serve as Chair of the Member Assembly. 3. The order of business shall be outlined in an agenda for the meeting, which shall be distributed

electronically prior to the session. The chair may change the order of business. 4. The American Institute of Parliamentarians Standard Code of Parliamentary Procedure (most recent

edition) shall govern the Member Assembly except when this code is in conflict with the TAFP Bylaws or the Standing Rules of the Member Assembly.

5. The Member Assembly shall elect officers, Delegates to the AAFP, and members of the TAFP Board of Directors, and it shall act on business specifically referred by the Board of Directors.

6. The delegates to the Assembly may, by majority vote, approve a referendum for submission to the members of TAFP on questions affecting the policy or recommendations of the Academy. The option to bring a referendum to the Member Assembly for consideration should be used as the last resort for a member wishing to raise concerns. TAFP members wishing to have a concern addressed are encouraged to first contact the chair of the relevant committee or commission. TAFP’s committees and commissions are open to guests unless they are in executive session. If a member is not satisfied with the response of a committee, that member should then address his or her concerns with the Board of Directors. If the Member Assembly approves a referendum, the item must be submitted to the entire membership for a vote.

7. Each local chapter shall have delegates and alternates in proportion to the number of members in their chapter as of June 1 each year. Chapters with 1-49 members shall have one delegate and one alternate, chapters with 50-99 members shall have two delegates and two alternates, chapters with 100 to 199 members shall have three delegates and three alternates, chapters with 200 to 299 members shall have four delegates and four alternates and so on.

8. Local chapters will be given an opportunity to select delegates and alternates. If a chapter has not submitted the names of members of their delegates and alternates to TAFP staff at least 60 days prior to the start of the Annual Session, TAFP staff will contact members and assign delegates on a first-come, first-served basis.

9. In addition to chapter delegates, the Section on Special Constituencies, the Section on Resident Physicians, and the Section on Medical Students shall each elect two delegates and two alternates at their Interim Session meeting to serve at the subsequent Member Assembly. The current TAFP Board of Directors will also serve as voting members of the Member Assembly.

10. The term of office for all delegates and alternate delegates is one year but they may succeed themselves without limit.

11. To be seated, a delegate must be a member in good standing with TAFP and AAFP. 12. A quorum shall be 30 voting members of the Member Assembly. 13. Each delegate to the Member Assembly shall have one vote and the privilege of speaking. Should a

TAFP member in good standing not serving on the Assembly wish to have the privilege of the floor, they must petition the presiding officer and obtain permission to address the Assembly but they shall have no right to vote.

14. Delegates must be credentialed prior to the Member Assembly. They will be given ballots for elections and for run-off elections if necessary. If a delegate has not checked in 15 minutes prior to the start of the Member Assembly, the first alternate from their chapter or constituency shall be credentialed as a delegate.

15. Nomination process for elected positions: a. TAFP’s Nominating Committee will propose a slate of one or more names for the positions of

President-Elect, Vice President, Treasurer, Parliamentarian, Delegate to the AAFP, Alternate Delegate to the AAFP, At-Large Director on the TAFP Board of Directors and New Physician Director on the TAFP Board of Directors. Members interested in one of these positions should contact the Nominating Committee chair or staff prior to Interim Session.

b. In addition to the Nominating Committee slate, local or component chapters may nominate members for any elected position. This nomination must be submitted in writing to the president at least 90 days prior to the beginning of the Annual Session. If a component chapter is inactive, five members in good standing of that component chapter may submit a nomination. The president will notify the Board of Directors of all nominations for all elected positions at least 60 days prior to the beginning of the Annual Session.

c. Members can also be nominated from the floor during the Member Assembly. d. The Section on Special Constituencies, Section on Resident Physicians and Section on Medical

Students shall each elect a qualified member for the respective positions on the TAFP Board of Directors at their Interim Session meeting. Those names will be sent forth to the Member Assembly on the slate with the candidates for the other positions. There may be nominations from the floor for those positions.

e. The names of candidates nominated for positions in one of the methods outlined above shall be read and placed in nomination during the Member Assembly. The floor shall be open for additional nominations at that time.

f. Each candidate is required to give a speech, except in cases in which a candidate is running unopposed for election or re-election. Speeches shall not exceed five minutes and the limit shall be determined by the presiding officer prior to the Annual Session and Primary Care Summit.

16. Voting procedures: a. Delegates and members of the Board of Directors will be given ballots when they are

credentialed. There shall be one ballot for the initial election and another to use in case of a run-off. The ballots shall be different colors to avoid voting irregularities. The presiding officer of the Member Assembly will not vote unless there is a tie.

b. Candidates must receive a majority vote to be elected. c. If no candidate for a position receives a majority of the votes on the first ballot, a run-off will

occur between the two individuals with the highest number of votes on the first ballot. Voting members will already have a second ballot and will be instructed which individuals are in the run-off and will have 30 minutes to turn in their run-off ballot.

d. If the votes are not tabulated before the Member Assembly is adjourned, a run-off can still occur as long as the TAFP Annual Session and Primary Care Summit is still in session.

e. If the Member Assembly has adjourned before the votes are tabulated, the results will be announced in the general session and other meetings or CME events of TAFP’s Annual Session and Primary Care Summit.

17. Elected officers and delegates shall take office at the conclusion of the TAFP Annual Session and Primary Care Summit.

Member Assembly Item 7. Elections

November 2017

All current members of the TAFP Board of Directors, all chapter and constituency delegates, and those who have been seated as a delegate may vote in the election. Printed below are the nominees from the Nominating Committee and the appropriate section meetings. Nominations may be made from the floor for all positions. For all, the candidate who receives a majority of the votes will be elected. If you are interested in serving on the Board in the future, please contact TAFP staff for more information.

New Physician Director (elect one)

LAWRENCE M. GIBBS, MD, FAAFP - An alumnus of the University of Pittsburgh Medical School, Lawrence Gibbs, MD, FAAFP, completed his residency at St. Louis University’s Scott Air Force Base Family Medicine Residency program in Belleville, Illinois. After residency he remained on staff as faculty and element chief of the Belleville Family Medicine Clinic. After separating from the United States Air Force, he returned to his home state of Texas in 2016. He continues to serve as a reservist at Carswell Joint Reserve Base. He is a faculty physician at the Methodist-Charlton Family Medicine Residency Program in Dallas, and he serves as Director of Inpatient Medicine and Medical Student Clerkships. He has served as a member of the Uniformed Services Academy of Family Physicians Clinical Investigations and Education Committees and is a current member of TAFP’s Commission on Academic Affairs.

MARY ANNE SNYDER, DO – A graduate of the University of Dallas, Mary Anne Snyder, DO, received a Bachelor of Science in biology with a Spanish concentration. She received her Master of Arts in bioethics and Doctor of Osteopathic degrees from Kansas City University of Medicine and Biosciences. She completed residency at the University of Texas Health Science Center in San Antonio, where she was an administrative chief resident and was involved in many research and quality improvement projects. She enjoys volunteering her time at health fairs, women’s shelters, and refugee clinics. Snyder is currently practicing outpatient medicine at the Toyota Family Health Center in San Antonio. She enjoys staying active in TAFP and she recently attended the AAFP Advocacy Summit. She loves participating in international medical outreach and is excited to be a member of the founding faculty at the new Osteopathic School of Medicine at the University of Incarnate Word.

IRVIN SULAPAS, MD, FAAFP – An assistant professor at Baylor College of Medicine, Irvin Sulapas, MD, FAAFP, is a Houston native who received his Bachelor of Science in biomedical engineering from the University of Texas at Austin and his medical degree from Ross University. Sulapas completed both his family medicine residency and sports medicine fellowship at Texas Tech University Health Sciences Center at Lubbock. He is active in the Texas Medical Association, where he’s a current member on the Council of Medical Education. He is a current member of TAFP’s Commission on Academic Affairs and serves locally as one of the Harris County Medical Society delegates to TMA. Sulapas is a team physician for Texas Southern University and volunteers as a staff physician for the Houston Marathon. In his free time, enjoys exercising and advocating for healthy living to the general public.

TERRANCE HINES, MD, FAAFP – A graduate of Texas A&M for both undergraduate and medical school, Terrance Hines, MD, FAAFP, completed his residency at John Peter Smith Hospital in Fort Worth. He joined Baylor Scott & White in 2009 and is medical director for several clinics with emphasis on recruitment and development of new physicians. He is also a clinical assistant professor at Texas A&M College of Medicine. He’s served on several TAFP commissions and committees, served as chair of Bylaws, the Section on Special Constituencies, and is a former member of the Board of Directors. Hines enjoys all aspects of primary care with a particular interest in LGBT health, including transgender hormone therapy and PrEP for HIV prevention. He believes family medicine must seize the opportunity to be a stabilizing force in the evolving health care market and that TAFP must strongly advocate for all patients and physicians. He is married, has two sons, and loves Aggie football.

LOREN S. LASATER, MD, FAAFP - A native Texan hailing from Houston, Loren Laster, MD, FAAFP, received his undergraduate degree from TCU and completed medical school at UTMB. After completing a family medicine residency, he practiced in multiple settings including private practice, single-specialty family practice, multispecialty practices, and academic medicine. He currently practices in Grapevine, Texas. Motivated by the September 11 terrorist attacks, he volunteered for the Army reserve and currently serves as a Colonel with 14 years of service. He has three active-duty mobilizations under his belt, two in the United States and one in Germany. He considers it a great privilege to serve our troops and their families. During his career in medicine, he says he has seen many changes, stresses, and challenges. His breadth of practice experiences within health care gives him a unique perspective to empathize and relate with his colleagues.

ADRIAN BILLINGS, MD, PHD, FAAFP – He received his medical education at Texas A&M, the University of Texas Medical Branch, and John Peter Smith Hospital. He was awarded a scholarship to medical school by the National Health Service Corps and went on to complete his service obligation in Alpine, where he continues to practice. He’s served on the National Advisory Council to the National Health Service Corps, on TAFP’s Board of Directors, and as physician advisor for TMA’s Hard Hats for Little Heads program. Billings is an Associate Professor of Family and Community Medicine at Texas Tech University Health Sciences Center. Medical student and resident education continues to be central to his practice, and he has been recognized for his contributions to teaching, receiving several awards. He serves as the director of the Texas Family Medicine Preceptorship Program. He is also the immediate past chief of staff of Big Bend Regional Medical Center and was recently elected to the Alpine Independent School District Board of Trustees.

MARY NGUYEN, MD, FAAFP – She shares a family medicine practice with her husband in the rural town of Castroville and counts special needs advocacy as near and dear to her heart. She is currently a Member Constituency Delegate to the AAFP Congress of Delegates and previously held the position of Minority Co-convener for the 2017 National Conference of Constituency Leaders. Nguyen has served on several TAFP commissions and committees, including the Executive Committee and she currently serves on the TAFP Board of Directors as the Special Constituency member. She is the current Alamo Chapter Treasurer and was recently selected to serve on AAFP’s Reference Committee on Advocacy. She’s an avid supporter of the arts and she enjoys attending the symphony and theater with her husband and spending time with their 11-year-old son.

REBECCA HART, MD – After many years teaching full scope family medicine including obstetrics, Dr. Hart joined a small family medicine group and practices in League City. She has been actively involved with TAFP for many years and served as Chair of TAFP’s Commission on Academic Affairs and Commission on Continuing Professional Development. She also serves on AAFP’s Commission on Continuing Professional Development. She has been actively involved in TAFP’s CME efforts serving as program chair, faculty, and planning committee member. She served as an At-Large Director on the TAFP Board and is finishing her year as Treasurer.

JAVIER D. “JAKE” MARGO, JR., MD – Dr. Margo practices full scope family medicine in his hometown of Rio Grande City. He earned his medical degree from Texas A&M Health Science Center College of Medicine and completed his postgraduate training with John Peter Smith Family Medicine Residency. He has been involved with TAFP since medical school serving on numerous committees and commissions over the years. He is currently the President of the TAFP Valley Chapter, President of the Hidalgo Starr County Medical Society, and previously served as chair of the Section on Leadership Development. He served as an At-Large Director for two years and as Vice President for two years.

AMER SHAKIL, MD, MBA, FAAFP – Dr. Shakil is a professor in the Department of Family Medicine at UT Southwestern. After graduating from Rawalpindi Med College, Punjab University in Pakistan, he completed a residency and fellowship at University of Illinois College of Medicine at Chicago/Christ Hospital in Research and Faculty Development. In 2015, he earned an MBA in Healthcare Organization Leadership. He has been an active member of TAFP for many years serving on and chairing the TAFP Commission on Academic Affairs, serving on the Executive Committee, Nominating Committee and more. He has also been involved in leadership for the Dallas Chapter of TAFP and he serves on the AAFP Commission on Education. He is completing his year as Parliamentarian

Delegate to AAFP

LINDA SIY, MD – Dr. Siy is a TAFP Past President and completed her second term as Delegate to the AAFP Congress of Delegates this year. She previously served as Alternate Delegate and was a Women’s Constituency Delegate in 2000 and 2001. She has served on numerous TAFP committees and commissions and is the current President of the TAFP Foundation. She previously served as member and chair of the AAFP Committee on Special Constituencies. She is eligible to serve another term as Delegate and the Nominating Committee put forth her name for the position.

Alternate Delegate to AAFP

ASHOK KUMAR, MD, FAAFP – Dr. Kumar is a TAFP Past President and completed a partial term as Alternate Delegate this year. He is eligible to serve three full terms as Alternate Delegate and the Nominating Committee put forth his name for the position. Dr. Kumar has served on multiple committees before serving as an officer, and he was elected the first International Medical Graduate Delegate to AAFP’s Congress of Delegates in 2001. He has served on the AAFP Commission on Membership and Member Services, the AAFP Commission on Health of the Public and Science, and as President of Bexar County Medical Society.

Special Constituency Director

LESCA HADLEY, MD, FAAFP – After earning her medical degree at Texas Tech University Health Sciences Center in Lubbock, Dr. Hadley completed residency and a geriatrics fellowship at John Peter Smith Hospital. She is currently faculty at JPS practicing full-scope family medicine including inpatient and maternity care in an academic setting. She is also the Geriatrics Fellowship Program Director and teaches medical students, residents, nurse practitioner students, and geriatric fellows. She is passionate about global health and has presented at medical education conferences around the world. She has remained actively involved with TAFP and her local chapter since residency. She previously served as New Physician Delegate to the AAFP Congress of Delegates and has chaired several TAFP committees and commissions. She is also involved with the American Geriatrics Society and currently serves as President of the Texas Geriatrics Society. She is the nominee from the Section on Special Constituencies for the Special Constituency Director position on the Board of Directors for 2018.

Resident Director

SAMUEL MATHIS, MD – After completing medical school at UTMB Galveston, Dr. Mathis completed residency at Memorial Family Medicine Residency Program in Houston. He is now completing a fellowship in Integrative and Behavioral Medicine at UTMB Galveston. He has been actively involved with TAFP and AAFP as a student and resident. He has served as a member of the TAFP committee on Academic Affairs since he was a student and is the current resident member of the TAFP board. He served as student representative from AAFP to the AMA-MSS and is currently the AAFP Resident Delegate to the AMA-RFS. He also is active in the Texas Medical Association as part of the TMA-RFS executive committee and an Alternate Resident Delegate to TMA’s House of Delegates as well as numerous boards and committees. He is the nominee from the Section on Resident Physicians for the Resident Director position on the Board of Directors for 2018.

JASON JOHNSTON – Currently a third year medical student at McGovern Medical School in Houston, Jason is a leader among the students. He has been involved with the Section on Medical Students and is serving as Chair for 2017-18. He is the nominee from the Section on Medical Students for the Student Director position on the Board of Directors for 2018.
