    The tool can be used wherever you have a

    web browser. Therefore, it can be used at

    multiple service sites, at an outreach

    gathering, on a PC or laptop and even atablet or phone

    The tool uses open source free technology:

    The Affordable Care Act provided an

    opportunity and challenge for Community

    Health Center Outreach and Enrollment

    staff. In Texas, 67 health centers received

    funding from the Health Resources &

    Services Administration (HRSA) to hire and

    train Certified Application Counselors(CACs) to educate consumers about health

    coverage options and to assist them in

    applying to enroll in the Marketplace,

    Medicaid or CHIP and to register to vote.

    The Texas Association of Community Health

    Centers (TACHC) developed an Online

    Reporting & Tracking Tool to assist CACs to

    fulfill their HRSA reporting requirements

    and to enable health centers and TACHC

    better analyze consumer interest in health

    coverage options.

    We love the tool because it helps us keep

    up with inf ormation about all the in-house

    patients and con sumers that we have seen

    and provided information to.

    The tool helps in not having to do

    separate reports for every person/client we

    see. I would defin itely recommend this tool

    to other health centers.

    I do recommend the tool to other cli nics.

    Its well organized and its very easy to

    use and easy to find patient inf ormation.

    The CAC tool helps metrack down

    patients and see where they are in th e

    enrol lment process, which I believe helps

    eligibility.
