IHC delivers on an approach to wellness that is truly unique. With roots in a systems biologyapproach using the Functional Medicine model, along with incorporating structural integrity, we offer a ‘whole-being’ approach that goes well beyond the structural and biomolecular level of care. In other words, your emotional, mental, and physical all are enveloped in your care path. This model we call functional medicine, or FxMed for short, uses a systems biology approach to explore imbalances that cause the underlying symptoms and conditions you are experiencing. This includes chronic pain and inflammation, which more often than not, is the result ofmetabolic imbalance. The care received at IHC is not the typical impersonal and hurried kind you might beaccustomed to receiving; but involves a significant investment of time for both the patient andcare team. For this reason, the patient handbook has been compiled to provide details on what can be expected throughout the relationship. We ask that each patient sign anacknowledgement form that the handbook has been reviewed and understood. We look forward to partnering with you. Functional Medicine

Functional Medicine, or FxMd for short, is a model of health care where you the patient, are the center of an individualized, science based approach for offering only natural therapies which include diet, life-style, herbs and nutrients. With a FxMd approach to wellness you work with Dr. Bump directly to discover your underlying imbalances, and causes for your health concerns. In FxMd we help you restore balance to you life in each and every area, which can include not only the physical body, but also emotional, mental and spiritual. By discovering the root causes of your symptoms, we will be able to better understand all thecomplex ways it may express, whether it be fatigue, autoimmunity, gastrointestinal, chronic pain and inflammation, hormonal issues, and every other condition known to us! We might find that one condition has numerous causes, and conversely we might learn that one cause might lead to many conditions. In FxMd you the patient is the center of the wheel rolling towards wellness. Structural Integrity/Chiropractic At IHC, we refer to chiropractic care as Structural Integrity, as our physical structure dovetailsintimately with how our bodies function. In other words Structure affects Function, and Function also affects Structure. Our approach to working with patients often incorporates therapies that address underlying imbalances from a structural perspective. This not only includes chronic mus-cle tension and inflammation patterns, but also the joints. It also includes addressing the deeper supportive tissues, often overlooked in a typical chiropractic or massage experience. Using our Functional Medicine approach with evaluating and treating the “structural integrity” of your body we will address everything from your joints, muscles, ligaments, to the fascia andconnective tissue right down to the cellular level. This truly becomes a whole person healingexperience! We also utilize the most current treatment modalities and correct chronic pain andinflammation syndromes Low-Level Laser Therapy (LLLT) Commonly known as “cold Laser” therapy, as the light emitted does not heat the tissue the way some lasers do. Therefore you are comfortable throughout the treatment process. LLLT emits low levels of light, of various frequencies, that are absorbed by your cells, which then promote in-creased cellular energy and ultimately help heal surrounding tissues. LLLT helps regenerate tissue, reduce inflammation, decrease pain, and increase immunity, and research has shown its efficacy in treating many conditions, including and not limited to the following common ailments: pain, chronic or acute, injury, sprain, strains or muscle tension, acne, autoimmune disorders, including Hashimoto’s, microbial infection, energy and fatigue syndromes.

Her expertise in food planning, shopping and preparation is deep and servesas a platform for your program. She will assist your in setting your goals and help you take thesmall measurable steps along your path to achieve them. She will empower you to make theneeded changes that align with your values of good health while providing ongoingaccountability to keep you moving in a positive forward direction. Dawn, stands apart from others as she integrates PSYCH-K® into her work as your coach,when appropriate. Because your decisions are based on your belief systems, which are learnedin the first early formative years, they often become the obstacles which interfere with changeand growth. Dr. Christopher Bump Dr Christopher Bump has created a unique model of patient centered health care by integrating Functional Medicine and Structural Integrity. His practice philosophy includes integrating science- based alternative and integrative therapeutics, applied with a systems biology approach. He has over 35 years of experience in clinical nutrition and has earned numerous certifications and degrees along the way. He is a certified practitioner by the Institute for Functional Medicine and he holds a Masters Degree from Columbia University in Human Nutrition, where he has served as guest lecturer. He also holds certifications in clinical nutrition with the AMA College of Nutrition and he is a diplomat on the Chiropractic Board of Clinical Nutrition. He has also chaired the Council for Nutritional Education for the Association of New Jersey Chiropractors for over 6 years. Dr Bump continues in private practice in Northern New Jersey, as well as lecturing internationally on clini-cal nutrition offering unique insight from a functional and structural perspective.

Dawn Bump Dawn Bump is a Certified Functional Medicine Health Coach who has been with Integrated Health Care for 29 years. Her focus is treating the mind, body and spirit to help patients achieve their wellness goals. As a FM Health Coach, Dawn concentrates on the ‘roots’ of healing which starts with lifestyle changes. Elements like sleep, exercise, nutrition, stress reduction, health relationships and environment are all a part of getting on a healthy path. When approaching limiting beliefs that may hold one back in their health, relationships, or career, Dawn often

The in depthknowledge of the mind-body-spirit connection that Dawn brings comes from an extensive fusionof education. Dominion Herbal College, One Spirit Interfaith Seminary, Functional MedicineCoaching Academy, PSYCH-K® Certified Facilitator and a Certification in Spiritual Intelligence.Dawn and Dr. Bump just recently celebrated their 25th wedding anniversary and together have5 children and 4 grandchildren.

Diana Palmieri Diana has been working for Dr. Bump for 12 years, yet her journey with the office started much earlier than that as a patient. Diana found her own expe rience as a functional medicine patient so profound, that she wanted to come and be a part of the team. She knows so much about the supplements Dr. Bump prescribes to help patients with their questions, and ensures there is enough for all of our patients! Her passion for functional medicine and encour aging patients on their health journey’s is supported by her loving husband of 37 years, Tony, her 3 boys, and her 2 grandchildren that she loves to spoil!

Cassie Cuppek Cassie Cuppek, Dawn and Dr. Bump’s daughter and Office Manager graduated from Montclair State University with a Bachelor of Art in Psychology, and Fairleigh Dickinson University with a Master of Arts in Organizational Behavior and Leadership Studies. Cassie brings 10+ years of experience in corporate america working with cross functional teams and executive leadership to our office to marry her passion for health and wellness and organization effectiveness together. Having grown up in the functional medicine/chiropractic model created by Dr. Bump, she is passionate about bringing the model to life for more patients where it is simple, accessible, and cost effective. Cassie follows her parents passion for teaching and wellness as a certified Yoga Alliance RYT-200 teacher, Chair Yoga instructor, corporate facilitator, and a motorcycle riding instructor. Above all, Cassie will always say that her favorite role in life is her job as Aunt Cassie.

Bernadine Barany Bernardine was introduced to Dr. Bump in 1988 as a patient. She began working at the office in 1991 in a variety of capacities, includ ing Chiropractic Assistant and front desk administrator both of which she still does today. While working with Dr. Bump, Bernardine obtained certifications from Sussex Community College and the Institute for Therapeutic Massage, graduating with honors. Bernar dine’s contributions extend beyond Dr. Bump’s office with her own business that offers massage and reflexology. When she is not work-ing with or on patients she is mother to 3 children and grandmother to 4 grandchildren.

What conditions and diseases do you treat?

At Integrated Health Care (IHC) we like to say that we do not treat disease, we treat all of you.The focus is on you and how you developed a condition or disease. We clarify what isn’tworking well and how to restore balance to those areas for your optimal health. Using thisapproach, we discover the causes of “disease” rather than treating symptoms. One symptommight have many causes, and one cause might create many symptoms.

With that said, patients come to us with any condition imaginable, but some of the morecommon expressions of imbalances are:

What can I expect from the initial Fx Med visit?

It will be unlike any doctoring experience you have had, as you will actually engage in dialogueand be “heard”. The Integrated Health Care (IHC) motto is: “If we don’t know who you are, thenhow can we help you?” This is a lengthy meeting, (often 2 hours) where patients sits with Dr.Bump and our health coach to share your story. IHC will want to know detail how it is you cameto be sitting in our office. This initial meeting is wonderfully intense and often very revealing.In preparation for this first office visit you will need to complete Living Matrix/HealthHistory/Patient Intake forms online and have it completed at least 7 days prior to your visit. Thiswill enable us to perform a records review and prepare for our first meeting.

How many visits will I need?

The frequency and length of your follow-up appointments will depend on the nature of yourillness and your success with our dietary, lifestyle and nutrient recommendations. We find thatmost people see significant improvement after 3 - 4 visits, or over 6 months. That said, if youhave a complex condition, your healing could take longer and may require more visits to steadily‘peel back the layers’ of imbalance and “dis-ease”.

What’s the difference between a consultation and coaching?

In a consultation we are gathering, looking for and sharing information specific to your case.ÊHere, the doctor is educating you and making recommendations.

Coaching is a vehicle for helping people to achieve a higher level of well-being andÊperformance, particularly when change is difficult. Coaching is a partnership of patient focusedÊself discovery that elicits autonomous motivation, increases the capacity to change, andÊfacilitates a change process which leads to sustainable change for the good. Do you accept health insurance?

Integrated Health Care chooses to invest time in our patient’s total health by opting out of allhealth insurance plans, including Medicare/Medicaid. The level of care provided at our officecannot be delivered within the restrictions imposed by insurance companies. We will provide areceipt for service at check out, that includes diagnostic and procedures codes if desired. Youare able to use that to submit as you wish with your insurance arrangements. You may be ableto use Flexible Spending Account (FSA) or Health Savings Account (HSA) dollars to pay forfees and visits.We do not provide reports beyond the diagnosis and tests codes and receipt given at check out.

Are supplements included?

Integrated Health Care (IHC) doctors and coaches will make supplement and nutraceuticalrecommendations as part of a patient’s treatment plan. Because these formulas are available only through licensed practitioners IHC has an apothecary with many of these in the office forpurchase, or they can be ordered online via Dr. Bump’s Fullscript retail store. The costs of thesupplements are not included in serviced visits. If requesting the office to ship products to you,additional costs will apply.

Can I use my own supplements? While sometimes the nutrients you might buy on the internet and in health food stores may beappropriate for our recommendations, usually they are not. There are typically issues withquality and specificity in these consumer available products. The companies and formulas usedat IHC are the highest standard in manufacturing and are often offered only to licensedprofessionals. Are labs included? Integrated Health Care (IHC) will make diagnostic testing recommendations as part of apatient’s assessment and treatment plan and will provide scripts for our patients. Manyinsurance plans will cover labs and/or radiology and the patient can file forpayment/reimbursement. It is the patient’s responsibility to understand their insurance coveragefor lab work. Our office also utilizes specialized lab testing which you will need to pay directlyfor, as many insurances do not cover these.We do not provide written reports to insurance companies for reimbursement beyond thediagnosis and tests codes and receipt given at check out. Do you have payment plans? The payment terms are listed in the Fees and Policies section. Should unforeseen financialcircumstances arise during your care with IHC, please reach out to our staff to discuss yourneeds to discuss a payment options that is agreeable for both you and IHC. Do you do telemedicine with Skype, Google Hangout, Zoom, or Facetime?

Yes. This always us to reach patients across the United States and the globe! We dorecommend meeting face to face at least one time in your partnership with us, if possible. Can I email questions and or concerns? The use of email must be pre-arranged by agreeing to the fee schedule. (See Fee Schedule).Email communications are considered consultations. You might be wondering why. When anemail comes in we need to take time to pull your patient data and review your files to ensureany responses would be personalized to you and your partnership. Then responses areformulated and sent off often resulting in a follow up email from patients with additionalquestions or clarifications. All of this takes time for the IHC care team while tending to patientsthat are in the office. Are you available for emergencies? In the case of an emergency, Dr. Bump’s cell phone is available. Call and leave a message.Text messages will not be accepted and or responded to.
