wife Nagma Khatoon had born baby boy in your

hostipal on 10.06.2005. In the Neonatial

Discharge Summary you mentioned my name of I.e.

S.A.MOHD and my wife name I.e. Nagma Khatoon.

The Municipal Corporation of Delhi issued birth

certificate mentioned my name as S.A. MOHD

instead my name mentioned above of SYED ASAD

MAHMOOD and my wife name as NAGMA instead of

mentioned NAGMA KHATOON. You are therefore

requested to either rectify it your goodself for

MCD record or provide/reissue a copy of some

duly authorized for such purpose to me so that I

can correct it at MCD record my self.
