About Coleman County
Medical Center
CCMC is a Critical Access Hospital that has served Coleman County and surrounding areas since the 1920s. The hospital is licensed for 25 acute care beds, all of which may also be used as swing beds. Unlike many small hospitals, CCMC continues to offer surgical and obstetrical services, along with a full complement of inpatient and outpatient laboratory, imaging, physical therapy and rehabilitation services. CCMC has been designated a Level IV Trauma Center, while its nursery meets all of the latest requirements to receive Level I Neonatal designation.

SWING BED PROGRAM
The Swing Bed Program at Coleman County Medical Center (CCMC) is for patients discharged from CCMC or any other hospital following an acute illness, injury or surgery, who still need time and physical therapy to heal and strengthen before going home.
The CCMC Swing Bed Program eases the transition from acute care to home by providing patients with access to highly trained and caring medical professionals right here in Van Horn!
Who is eligible for the Swing Bed Program?
The CCMC Swing Bed Program is for Medicare patients who require:
Teaching and training to care for a new health problem or condition
IV Therapy
What are the advantages of the Swing Bed Program for family and other caregivers?
Eases the burden of caring for a loved one
Helps educate the caregiver to provide care
Allows family, friends and neighbors to visit more often
If you or a family member are discharged from an acute care hospital following hip or knee surgery, general surgery, a stroke or other surgical or medical condition and still need time to strengthen and heal before going home, contact CCMC to see if you are eligible for the Swing Bed Program.
