Background

• Sustainable home care and support is critical in helping vulnerable adults to

remain living independently at home.

• In 2017/18 the council spent around £37m on homecare and is a vital service

for customers, providers and the Council

• By working closely with the care sector we have ensured a good

understanding of what is important to ensuring the delivery of safe, quality

and sustainable care across Suffolk.

• The market engagement events have enabled the care sector to influence the

design of the proposed new model

Update on Continuing Health Care (CHC)

• The following Clinical Commissioning Groups (CCG) operate in Suffolk and are responsible for CHC;

• Ipswich and East Suffolk

• West Suffolk

• Great Yarmouth and Waveney

• Ipswich & East Suffolk and West Suffolk share a joint CHC team and have decided to jointly commission home care with SCC using the Locality service.

• Great Yarmouth & Waveney have yet to make a final decision.

Following all the feedback and market engagement so far this is the final consultation via our website to hear your views on;

• The proposed Pricing for each of the 5 Levels

• The Levels applicable for each parish as shown on the County map

• The proposal for the allocation of new care

• The proposed KPI’s and Performance Management

Pricing Methodology

Through the previous market engagement there was no consensus as to whether SCC and the CCG

should be a price setter or taker.

The model has to be affordable within Councils and the CCG’s budget constraints

In order to recognise the differences of delivering care across Suffolk there will be 5 price bands across the

county set at parish level, the bandings take account of factors such as:

• Availability of care staff

• Deprivation

• Rurality

Therefore this recognises the varying difficulty in providing care in some areas compared to others.

The pricing methodology has been approved by SCC and the CCG (not Great Yarmouth & Waveney)

Proposed Price Levels

The proposed prices for 2019/20 would be as below subject to any inflationary uplift for that

financial year:

Level 1 £16.49ph

Level 2 £17.07ph

Level 3 £18.03ph

Level 4 £19.46ph

Level 5 £22.24ph

Thereafter, the Council will notify providers of the rates for each new financial year with due regard

to inflationary pressures in the market and the funding position of the Council / CCG.

Proposal on how work will be allocated to successful providers within the CONNECT/Locality areas:

• Step 1: The requirements for care will be circulated to all the successful providers in each

connect / locality area with a date when we require the care to start. Providers will be

required to respond within a set timescale and the provider that can start on the required

start date will be offered the work.

• Step 2: If more than one provider makes an offer for the required start date then we will use

the ranking to allocate the care.

• CHC patients - all care will be allocated by contacting providers in each Connect area by

their ranking position until a Provider is able to pick up the work. This is due to the nature of

the patients needs and many requiring end of life care at home which requires care to start

within a shorter timeframe. This applies to Ipswich & East and West Suffolk CCG.

Proposed Ranking of Providers

• Initially, Providers will be ranked by the score they obtain from their tender submission.

• To ensure there is an equitable process in place to allocate care to providers, a proposed ranking system will be set up.

• Following the first quarter, SCC and the CCG will use factual information available to us to determine the ranking position of providers going forward. This will be undertaken on a quarterly basis:

Proposed Performance ManagementProviders will be asked to submit information on key performance areas which align to the CQC

domains:

• Staff training

• Medication Errors from other professional services (i.e. pharmacies)

• Outcome Based Care planning

• Staff numbers and Care hours

• Customer Feed back Survey

• Staff Survey

• Complaints and Compliments received directly by the provider

This performance information will be requested on a regular basis but will be proportionate and

although initially will not be used to determine the ranking of providers, it is our intention to

introduce this within the life of the contract.
