PROJECT: H2050
SUBPROJECT 2: Comprehensive traceability systems of patients and resources.
Thematic Area: Safe Hospital. Efficient Hospital.
1. Total number of proposals received and participating companies:
2. Project Scope/Functional Analysis
Development of a traceability system to improve the quality and efficiency of services provided to patients and professionals. Three areas of activity within this project have been identified:
• Patients: Identification, localization, and real-time flow control of patients.
• Resources: Adequate inventory control and equipment location avoiding inefficient use.
• Professionals: Improved management activity, location and physical and logical security.

The objectives of this integrated system are:
• Increase patient security
• Improve hospital processing times
• Improve management and control resource usage
• Improve the management of professional activity
3. Identification of innovative elements to be developed.
Development of an integrated system that facilitates professionals to know, regardless of the time, the
physical location of patients and professionals, as well as relevant information for the development of
programmed activities that are performed on the patient.
For this purpose, it is necessary to:
• Identify and parameterize different care processes and events.
• Integrate a system of identification, control, status, tracing and tracking systems based on real
time location in the healthcare network in agreement with the different scenarios that may arise.
• Integrate all of the events in the various subsystems into a concentrator platform, identifying and
relating them with managers and times.
• Activity control of professionals, integrating physical and logical security.
In terms of components, the system can be described in the following scheme.
The project identifies several key processes within the hospital, to be taken as reference for the building of
the system, but which must be expanded during project implementation. The processes sho
various events that will be obtained from existing information systems or through new traceability
elements that are incorporated into the project. The information on these events will be added into a
system thatwill register the information and
that information.
EXAMPLE OF AN IDENTIFIED EVENT

• Atomization of entrance to a
• Arrival to the hospital unit
2. In relation to tests:
• Date and time of the request
• Date and time of realization
• Date and time of results

• Arrival time
5.In relation to the transfusion process
• Request date
• Cross-match extraction date
• Time of request and blood sample
• Time request for blood or blood unit products
• Departure time from the blood bank
• Registration and identification of patient and professionals involved in the process
6.In relation to medication administered
• Prescription date and time
• Date and time of pharmacy dispensing service
• Date and time for administration
• Registration and identification of patient and professionals involved in the process
7.In relation to the administration to Cytostatic treatment
• Identification of the product on entry to the pharmacy
• Time of prescription
• Time of pharmacy validation
• Time of preparation/dispensing detailing the medication and registering the verification of the
dose
• Time of administering
• Registration and identification of patient and professionals involved in the process
8.In relation to the taking of vital signs
• Date and time the vital signs are taken
• Inputting the patients Vital signs including the professional involved

9. In relation to the univocal mother-child relationship
• Issuing Mother-child wristbands in the admissions services
• Arrival to delivery room
• Time of wristband collocation
• Date and time of request for a second wristband with information related to the child
• Date and time of issue of a second wristband by the admission services
• Date and time of collocation of the second wristband on the child
The following is a diagram of the functioning of the system applied to a specific situation in which
information is collected from events and is distributed to multiple application consumers of information.

Overview
The complexity of the flow of patients and resources within a hospital is
very high and is a major cost in time and resources in their coordination.
Incorrect identification of patients and procedures that are subjected cause
errors in medication, in testing, in invasive or surgical procedures.
System Traceability that Improves the Quality And efficiency of Theservices provided to patients and professionals .
Identified needs
• Patients:
Identification, Location and flow control in real time of the patient .
• Resources :Control inventory and location of equipment properly, avoiding inefficiencies in its use.
