1 Sub-Award to Strengthening capacity of TB providers in HIV counseling USAID Funded Project No. AID-OAA-A-14-00029 Request for Proposal (RFP) No: RFP/CTB/I-011/2017 RFA issuance date June 8, 2017 Proposal due date and time June 16, 2017 at 5 p.m. KNCV/Challenge TB, on behalf of United States Agency for International Development (USAID), is seeking organizations to train and strengthen the capacity of TB providers in offering HIV testing to TB patients. The RFP has objectives as follows: 1. To develop a brief PITC workshop curriculum for TB staff with appropriate job aids suitable for TB staff at all Health Facilities (based on existing material) 2. To train the trainers in 6 provinces / 16 districts 3. To train TB staff in the 16 CTB supported districts (768 PKM with surrounding public and private primary care level health providers) in effective communication to ensure uptake of HIV testing 4.     INTRODUCTIONKNCVTuberculosisFoundationKNCVTuberculosisFoundation(KNCV)isaninternationalnon-profitorganizationdedicatedtothefightagainst tuberculosis (TB), still the second most deadly infectious disease in the world. KNCV is aninternational center of expertise for TB control that promotes effective, efficient, innovative andsustainable TB control strategies in a national and international context. We are an organization ofpassionateTBprofessionals,includingdoctors,researchers,trainingexperts,nursesandepidemiologists.WeaimtostopthespreadoftheworldwideepidemicofTBandtopreventthefurtherspreadofdrug-resistantTB.Over thepast centurywehavebuilt up awealthof knowledge andexpertise, initially by successfullycontrollingTBintheNetherlands.Sincethe1970s,wehavealsosharedourknowledgeandexpertisewiththerestoftheworld.WeoperatefromacentralofficeinTheHagueintheNetherlands,aregionalofficeinCentralAsiaandcountryofficesworldwide.KNCVraisesfundsfromprivate, institutional,corporate,andgovernmentdonors.ChallengeTBKNCVistheleadpartnerinChallengeTB(CTB),theongoingfive-yearglobalprojectfundedbytheUnitedStates Agency for International Development (USAID) to decrease TBmortality andmorbidity in highburdened countries.We lead an international consortiumwith eight partner organizations: AmericanThoracicSociety(ATS),FamilyHealthInternational(FHI360),InteractiveResearch&Development(IRD),Japanese Anti Tuberculosis Foundation (JATA), Management Sciences for Health (MSH), Program forAppropriateTechnologyinHealth(PATH),TheInternationalUnionAgainstTuberculosisandLungDisease(TheUnion),andtheWorldHealthOrganization(WHO).TheoverarchingstrategicobjectivesofCTBaretoimproveaccesstoqualitypatient-centeredcareforTB,TB/HIV,andMDR-TB;topreventtransmissionanddiseaseprogression;andtostrengthenTBplatforms.CTBprojectincludesTBcontrolactivitiesin23countriesandseveraloverarchingcoreprojectsinmultiplecountries.Formoreinformation,pleasevisitwww.challengetb.org.ChallengeTBinIndonesiaIndonesia is among top30highest TB/HIVburden countries in theworld. TheHIVepidemic is largelyconcentratedinkeypopulations,althoughalowlevelepidemicexistsinsomeprovincesandageneralizedepidemicintwoothers(PapuaandWestPapua).TheoverallcoverageofHIVpreventionservicesremainslow,resulting inhigh levelsofunsafebehaviorand lowawareness.StigmatobothHIVandTBamongcommunities,patientsandhealthprovidersexists.ProviderinitiatedHIVtestingandcounselling(PITC)isadoptedasanationalstrategyforTBpatientssince2013(MinisterialDecreeno.21/2013).However,HIVtestingservicesremainconcentratedathigher-level

    healthfacilities.DistrictTBofficersreceivedtrainingonPITC,butdidnotpracticeitduetolackofclearinstructionsfromtheNTPandNAP.StaffsattheTBtreatmentsites,whicharehighlydecentralized,lackanytrainingoncounselingforHIVtesting.In2016,only15%ofthenotifiedTBpatientshadaknownHIVstatus.Onlyoneprovince(Bali)achievedmorethan50%coverage,whilecoveragewaslessthan40%eveninPapuaandWestPapua,and20outof34provincesachievedlessthan10%coverage.Itwasunclearwhetherthisisduetolowimplementation,orareportinggap,orboth.

    Thereareatotalof3,204HIVtestingsitesoutof12,242healthfacilitiesin514districtsinIndonesia,andthereislimitedavailabilityoftestingservicesatthePuskesmaslevelwherethemajorityofTBpatientsreceivetreatment.TBpatientsarenotofferedHIVtestroutinelyasprovidersanticipatestigmaandofferan HIV test only to those perceived to have a high risk. The TB training curriculum lacks operationalguidanceon implementationofPITCandstaffprovidingTBservicesdonotreceiveanytraining inHIVcounseling.ThereforethereisagenerallackconfidenceinofferingHIVtestingamongspecialists,doctorsandnurses.ThequalityofcounselingtooremainssuboptimalindicatedbythelowacceptancerateandlinkagetoARTinTBpatients.ThiscallsforincorporatingappropriateHIVcounsellingcontentinbasicTBtrainingandtheprovisionofcatch-uptrainingformostTBstaff.Inthecurrentpracticeclients/patientsneedtoprovidesignedconsent(bothtooptinoroutofHIVtest)furtheraffectinguptake.

    Challenge TB in Indonesia is a USAID funded project supporting the National TB Program in makingstrategicchoicesforsustainableTBservices,ensuringimplementationofthehighestimpactmeasurewithlimited resources. Challenge TB Indonesia is led by KNCV, collaboratingwith two in-country coalitionpartnersFHI360andWHO.KNCVisassistedthroughshort-termtechnicalassistancefromtwoexternalcoalitionpartners;AmericanThoracicSociety(ATS)andInteractiveResearchandDevelopment(IRD).Theoverallaimoftheprojectistoimplementabroadrangeof(distinctfinancialandtechnicalsustainableinterventions.AtnationallevelChallengeTBprovidesTechnicalAssistance(TA)totheDirectorateofCDCwithintheMinistryofHealthasthemainpartner.AdditionalrecipientsofTAincludeotherGovernmentInstitutes (suchasMedicalServices,LaboratoryandPharmaceuticalServices), theMinistryofLawandHumanRights,professionalsocieties,communityorganizationsandotherlocalpartnersinallsupportedprovinces.Atdecentralizedlevel,CTBprovidesTAtotheProvinceandDistrictHealthOfficesin16districtsin6provinces:NorthSumatra –MedanandDeli Serdang,DKI Jakarta JakartaUtara, Jakarta Selatan,JakartaBarat,JakartaPusatandJakartaTimur,WestJava-BandungKotaandBogorKabupaten,CentralJava - Semarang Kota and Surakarta, East Java – Jember and Tulun Agung, Papua – Jayapura Kota,JayawijayaKabupatenandMimikaKabupaten.OneofCTBIndonesia’spriorityactivitiesistoensurethatTBpatientshaveaccesstoqualitytreatmentandcareforTBHIV,frombothpublicandprivatecareproviders.

    PURPOSEOFREQUESTFORPROPOSALS

    ThisrequestforproposalsisintendedtoselecttheinstitutionwithstrongcapacityandhumanresourcestotrainandstrengthenthecapacityofTBprovidersinofferingHIVtestingtoTBpatients.

    SCOPEOFWORKObjectives:

    APPLICATIONPROCESSTheproposalmustbepreparedinaccordancewiththeinstructionsprovidedinthesectionbelow.Eachorganizationshallsubmitonlyoneproposal.

    IssuanceofthisRFPdoesnotinanywayconstituteanawardorcommitmentonthepartoftheCTBnordoesitcommittopayforcostsincurredinthepreparationandsubmissionofproposal.

    GENERALGUIDELINESFORDEVELOPINGREQUESTSFORPROPOSALS

    Applicantsshallconsiderthefollowingguidelinesfordevelopingtheproposal:

    ● Theproposalshouldaddressallactivitiesundersection‘ScopeofWork’.● Proposedactivitiesdescribed in theproposal shouldbe for theproposedgeographicarea, i.e.

    Indonesia.● Applicants can receive funding for similar activities from other donors. However, proposed

    activitiesmustnotbeduplicationofactivitiesthatarecoveredorplannedundertheotherfundingsource,andtheorganizationmustmakeclearhowthedifferentfundingsourceswillbeusedfordistinctobjectivesordistinctgeographicareas.

    ELIGIBILITYCRITERIATheorganizationsubmittingtheproposalshouldmeetthefollowingeligibilitycriteria:

    • Legalorganization;• Organizationwithdistrictandnationalnetworksmayimplementactivitiesatcommunitylevel;• Organizationworkswithspecifictargets,especiallyman,elderlyandchildren;• Organizationexperiencedinworkingwithinternationalgrant.

    BUDGETANDFUNDINGPERIODAbudgetplanshouldbeaddedintheattachedformat.Thebudgetshouldspecifyalllevelsandnumbersof staff needed both working in full-time and part-time (%), equipment, maintenance, consumables,communications,transportation,etc.Thefollowingcategoriesofcostsshouldbeused:salaryandwages,fringebenefits, travelandtransportation,equipment,supplies,contractual,otherdirectcosts, indirectcosts.1Allitemsshouldbespecifiedandjustified.The funding period of the sub-award is threemonths, depending on the signature date. FundingwilldependonUSAIDsupportforthisprojectandthereforefundingcommitmentscanonlybegivenforoneyearperiodatthemaximum.

    1 For an indirect cost rate: In accordance with the provisional negotiated indirect cost rate agreement (NICRA), 1) allowable indirect costs shall be reimbursed on the basis of the negotiated provisional or predetermined indirect cost rates and the appropriate bases (NICRA) or 2) Sub-Recipient can budget an indirect cost rate which can be independent from KNCV’s indirect cost rate provided the following is in place: a) Job cost or activity based accounting system which accumulates indirect costs by pool (e.g. fringe benefits, overhead, indirect, general and administrative); b) Timekeeping system which supports direct and indirect costs; c) Cost policy statement which identifies base and pool, direct and indirect functions/costs; d) Timely (monthly) closing of books and records; and e) Financial statement audit and/or independent review of rates annually to attest to compliance the relevant USAID cost principles. This indirect cost rate needs to be approved by KNCV. For budget purposes, Sub-Recipient shall submit a substantiated indirect rate calculation and a confirmation that the above is in place to KNCV. Reimbursements will be made based on actual rates determined by an audit, if budgets allow.
