3. Ask a teacher to complete the Teacher Evaluation and return it to you in an envelope.
4. Choose three pieces of Student Work - Choose one piece that represents your best work from most of your current classes. “Best work” does not necessarily mean you received a high grade, rather it is work that you feel best represents your ability, passion and/or growth in the subject. You should have a total of 3 pieces: English - Language Arts, Math- Science, Your Choice .
The Your Choice option can be an artistic work, another academic work or any piece of work that you feel represents you as a student. If your work sample is a piece of visual art, please make sure it is a photo copy. If it is an audio recording , please make sure it is on an audio CD. (WORK WILL NOT BE RETURNED)
Examples of appropriate work many include, but are not limited to: written assignments, projects, assessments, or in-class work. All work should fit within a manila file folder. Please use photocopies. If you have a large project you would like to include, please include photos of your work. Student Work samples will not be returned.

4. When you finish, your application should include:

SAMI
MISSION STATEMENTThe mission of SOTA AND SAMI is to utilize our community resources to offer a creative path of learning which emphasizes human expression through the sciences, mathematics and arts as central elements in academic achievement and lifelong endeavors.
The Tacoma School of the Arts and the Science and Math Institute will:• Stimulate higher level thinking and problem solving skills• Provide advanced training in the arts• Prepare students to meet the challenges of the 21st century• Incorporate experiential and innovative approaches to learning• Use both public and private facilities• Connect students to the larger community• Build this creative learning community using the resources of our whole city
In light of the SOTA AND SAMi mission statement and principles:SOTA and SAMi staff, students, and parents will:
• Act as agents of change in our urban community setting• Encourage positive personal connections among ALL members of the SOTA and SAMi community• Collaborate in creating an integrated curriculum• Share responsibility for a safe, ethical and focused place to learn• Respect and protect school and community property and equipment
SOTA and SAMi students agree to:• Behave and dress appropriately at all SOTA and SAMi activities• Respect requests and directions given by adults• Respect others regardless of class, disabilities, gender, race, religion, sexual orientation, and any other differences that may exist• Participate fully in mentor groups and all school functions• Attend, engage, and perform in all academic courses and artistic endeavors• Respect and protect school and community property, equipment, and resources in our school and community
SOTA and SAMi staff agrees to support students by addressing any breach of contract with the following interventions:• Communicate with other staff• Conference between mentor and student to clarify and discuss incidents• Assist students to creatively solve the problem• Assign RASH (Required After-school Study Hour)or alternative resources to get back on track academically• Involve families in problem solving
Consequences for serious or repeated infractions may include:• Formal conference between family, staff, mentor, and student • Required study skills class the following semester if a D or E is earned in any class at semester’s end• Academic probation status (Required Study Skills class, loss of privileges such as participation in showcase, etc.)• Administrative interventions including suspension or expulsion• Formal counseling as to a more appropriate site placement for the student
In addition to the information stated above, all students will receive a SOTA / SAMi Student Handbook In it are the Student’s Right, Responsibilities and Regulations. Please make sure you are all familiar with this information.
Because our day is longer and we have more periods of study, sports can be taken during the last period of the day without impacting a student’s graduation credits. All students must be enrolled in a minimum of six classes.
