History(Excerptedfrom“TheHistoryofPacificUniversity”onthePacificUniversitywebpage-http://www.pacificu.edu/about/history/index.cfm)Oregon'sPacificUniversityistheoldestchartereduniversityintheWest.TheOregonTerritorialLegislaturegranteditsoriginalcharterastheTualatinAcademyonSept.26,1849.Thecharterpredatesstatehoodby10years,andwasthefirstformalactoftheterritorialgovernment.Pacificfounderswerealsoinstrumentalinthe1843voteatChampoeg,whichresultedintheformationoftheOregonProvisionalGovernment,thefirstAmericangovernmentonthePacificCoast.TheschooltracesitsrootstoalogcabinmeetinghouseinForestGrovewheretheRev.HarveyClark,aCongregationalistminister,andTabithaBrown,aformerteacherfromMassachusetts,caredforandeducatedorphansoftheOregonTrail.TheUniversityissuedthefirstbaccalaureatedegreeintheregionin1863toHarveyW.Scott,latertheeditorofTheOregonian.Pacific'sOldCollegeHall,builtin1850,housedtheoriginalacademyandistheoldesteducationalbuildingintheWest.In1945,theUniversityexpandedintothehealthprofessionsthroughamergerwiththePacificNorthwestCollegeofOptometry.Today,Pacificisacomprehensiveuniversitywithundergraduate,graduateandprofessionalprogramsintheliberalartsandsciences,optometry,education,healthcareandbusiness.Pacificservesmorethan3,400studentsoncampusesinForestGrove,Eugene,HillsboroandWoodburn.Otherhealthprofessionsprogramswereaddedlater:PhysicalTherapyin1975,OccupationalTherapyin1984,ProfessionalPsychologyin1985,andPhysicianAssistantStudiesprogramin1996.TheCollegeofHealthProfessionswasestablishedin2005tohousethehealthprofessionalprograms.TheSchoolofPharmacyandtheSchoolofDentalHygieneStudiesjoinedtheCollegeofHealthProfessionsin2005.TheCHPhasexpandedtoincludetheSchoolofHealthcareAdministrationandLeadershipin2008,Audiologyin2011.Today,Pacific’sfocusonteachingandlearninginaclosenurturingenvironmentleadstogenuinetransformationinstudents’lives.Distinguishedbyitsinquiry-basedcurriculaandinterdisciplinarycollaboration,Pacificoffersstudentsexceptionalopportunitiesforexperientiallearning,leadershipandcivicengagement.

OverviewTheSchoolofPharmacy(“School”)islocatedinHillsboro,Oregon,andsharesphysicalfacilitieswithnineotherprofessionalprogramsintheCollegeofHealthProfessions(CHP)ontheHillsboroCampus:DentalHygieneStudies,OccupationalTherapy,PhysicalTherapy,PhysicianAssistantStudies,ProfessionalPsychology,HealthcareAdministrationandLeadership,andAudiology.ThoughtheSchoolofPharmacyisnotlocatedontheForestGrovecampus,itsharesresourceswith,andhasstrongtiesto,thePacificUniversitycampusinForestGrove.TheSchooloccupiesthefourthfloorofCreightonHall(HPC1),whichincludestwolargeclassrooms,multiplebreakoutrooms,astudentloungearea,astudentlaboratorythatisfullyequippedasacompoundinglab,andfacultyoffices.Inaddition,thereareadditionalofficesfortheSchool’sfacultymembersinHPC1onthefifthfloorandinHPC2onthethirdfloor.HPC2alsoprovidesastate-of-the-artscienceresearchfacilitytoaccommodatethescholarshipneedsoftheSchool’ssciencefaculty,aswellassuppliesadditionalspaceforstudents,faculty,staffandadministratorstostudy,socializeandcollaborate.TheSchooloffersathree-yearprofessionalcurriculumleadingtoconferraloftheDoctorofPharmacydegree.Thecurriculumiscomposedoftwodidactic/experientialyearsfollowedbyoneyearofAdvancedPharmacyPracticeExperiences(APPE).Duringthefirsttwoacademicyears,studentsspendtimeoff-siteinvarioushealthcaresettingsaspartoftheIntroductoryPharmacyPracticeExperiences(IPPE).TheIPPE1andIPPE4coursesoccurduringthedidacticyear.Inaddition,studentsparticipateinIPPE2andIPPE3coursesduringthesummerfollowingtheconclusionofthefirstprofessionalyear.TheseIPPEexperiencesassistinprovidinghands-onexperiencesthatsupportandreinforceclassroominstruction.Thecurriculumisbasedonamodified-blockdesignthatallowsthesequentialdeliveryofblocks(pluslongitudinalcourses)ratherthanthemoretraditionalmethodofteachingmultiplesubjectsatthesametime.Studentsarenotassignedlettergradesinthecurriculumbutinsteadreceiveeithera“Pass”or“NoPass”basedonachievementof90%ofstatedcompetencies.Duringblockcourses,studentsareassessedeverytwoweeksduringthefirsttwoyearsofthecurriculum.Studentswhodonotachievethenecessarylevelofcompetencearegivenopportunitiesforre-examinationandextendedlearning,ifnecessary.Re-examinationsareconductedtheweekfollowingtheblockexams,whileextendedlearningblocksareconductedduringadedicatedperiodafterthefallandspringterms.Therearealsolongitudinalblocksinthecurriculumthatoccuroverthecourseoftheentireterm,withclassesmeetingapproximatelyeverytwoweeks.Longitudinalblocksplaceanemphasisonintegrationofknowledge,criticalthinkingandtheutilizationofevidencebasedprinciples.

AccreditationProcessandStatus
PacificUniversityTheUniversity’sauthorityforconferringdegreeswasestablishedintheCharteroftheUniversitybytheLegislativeAssemblyoftheTerritoryofOregonin1849.In1863,thefirstbaccalaureatedegreewaspresentedtoHarveyW.Scott,wholaterbecameeditorofTheOregonian,thestate'slargestnewspaper.PacificUniversitysoughtandreceivedregionalaccreditationfromtheNorthwestAssociationofSchoolsandofCollegesandUniversities(NASC),CommissiononCollegesandUniversitiesin1929.In1945,theUniversityrequestedpermissionfromNASCtoofferthedoctoraldegree.TheDoctorofOptometry(OD)wasthefirstdoctoraldegreetobeconferredbytheUniversity.PacificUniversityisfullyaccreditedbytheNorthwestCommissiononCollegesandUniversities(NWCCU),whichuntiltheyear2003waspartofNASC.TheUniversitywasfullyreviewedbytheCommissionin2007andreceivedfullaccreditationforthenext10-yearcycle.In2010,NWCCUrevisedtheaccreditationcycletoaseven-yearcycle.Pacific’syearsevenreportandvisitwillbeinfall2016.PacificUniversityisamemberoftheNationalAssociationofIndependentCollegesandUniversities,theOregonIndependentCollegesFoundation,theOregonIndependentCollegesAssociation,theAmericanAssociationofUniversityWomen,theCouncilforHigherEducationoftheUnitedChurchofChrist,theCollegeScholarshipService,theNorthwestConferenceofIndependentColleges,theNationalCollegiateAthleticsAssociation,andtheAssociationofIndependentLiberalArtsCollegesforTeacherEducation.

ProfessionalAppearanceTheSchooliscommittedtopreparingstudentsforprofessionalcareers.Partoftheeducationalprocessislearningwhatprofessionaldressattireisandunderstandingtheexpectationsoffutureemployers.Studentsshallmaintainaphysicalappearanceandpersonalhygienethatisconducivetodevelopingeffectivepharmacist-patientrelationships.Studentswhodonotappearwellgroomedandappropriatelyattiredwheninteractingwithapatientareatriskforjeopardizingpatientrespectandforcreatingabarriertoeffectivecommunication.ThefollowingstandardsrepresenttheSchoolexpectations:

ViolationoftheStandardsofProfessionalConductAdefiningqualityofaprofessionisitsabilitytopoliceitself.Tothatend,itisthedutyandresponsibilityofeachmemberoftheacademiccommunitytousehis/herprofessionaljudgmenttotakeappropriateactionwhenanapparentviolationoccurs.Dependingonthesituation,theindividualmayattempttoresolvethesituationonhis/herown,orifthatremedyisunsuccessfuloriftheviolationisdeemedserious,theindividualwitnessingtheviolationshouldreporttheincidenttotheclassroominstructororexperientialrotationpreceptor.Ifthereappearstobeanadequatecause,theindividualreceivingthereportwillcommunicateittotheappropriateadministrativefaculty(mattersconcerningacademicintegrityshouldbereferredtotheADSA).Anystudentfoundtoreportanotherstudentintentionallyandmaliciouslyforthemerepurposeofharassmentshallbesubjecttodisciplinarysanctions.Theappropriateadministrativefacultymembershallfirstmakeapreliminaryinquiryintotheallegation(s)todetermineifthereissufficientbasistoconductaninvestigation.Ifaninvestigationisdeemedwarranted,theadministrativefacultymember,ortheirdesignee,maymeetwiththestudenttoinformhim/heroftheallegationandallowthestudentopportunitytorespond.Theadministrativefacultymemberwilldetermineifthereissufficientinformationavailabletomediateandresolvethecomplaint.Iftheinformationappearstobeindispute,theadministrativefacultymemberhastheoptiontoreferthemattertotheSPCfortheirreview.IfareferralismadetotheSPC,theadministrativefacultymemberwillprepareawrittenreportregardingtheissue,baseduponallknowninformationandforwardtotheChairoftheSPC.ConsequencesofUnsatisfactoryAchievementofAcademicorProfessional,andorStandardsTheStudentProgressionCommittee(SPC)andtheDeanwillhavethefollowingsanctionsattheirdisposalwhenreviewinganystudentissuerelatedtoacademic,professional,and/orexperientialissues.Duringprobation,astudentmayberequiredtomeetregularlywiththeirfacultyadvisor,theappropriateADand/ortheSPC,ifdirected.

SanctionsTherangeofpossiblesanctionsthatmaybeimposedininstancesofacademicdishonestyincludes,butisnotlimitedto,thoselistedbelow;itisgeneralPacificUniversitypolicythatsanctionsareimposedthatareconsistentwiththeseverityoftheviolation.
1 Incaseofviolationsofthe“AcceptableUsePolicy,”sanctionsrangefrombeingbarredfromthecampuselectronicnetworktosuspensionfromPacificUniversity.
2 InparticularlyseriousbroachesoftheHonorCode(seeHonorCodesection)orinrepeatoffenses,suspensionordismissalfromPacificUniversitymaybeimposed,aswellasotherappropriatesanctions.
Inmostfirstoffenses,nopermanentrecordoftheviolationwillbemade.Shouldthestudentnotbeinvolvedinanysubsequentincidentsofacademicdishonesty,thentherecordoftheviolationwillbedestroyeduponthestudent’sgraduation.Onlythosefacultymembersdirectlyinvolvedintheincidentandthoseservingonpanelsthatmighthearastudent’sappealwillbenotified.Seriousorsecondviolationswill,however,benotedonthepermanentrecord.
AcademicProbationSchoolofPharmacystudentswillbeplacedonacademicprobationforany“NoPass”(N)duringanyextendedlearningblock(failureofELre-examination)orbyreceivinganNforanyAPPErotation.Studentsonacademicprobationarerequiredtosatisfyandcomplywiththetermsand/orconditionsoftheirprobation.TheStudentProgressionCommitteeorADSA(inconsultationwiththeADAA)mayrecommendsuspensionofstudentswhohavenotfulfilledthetermsandconditionsoftheiracademicprobation,asstipulatedintheirofficialprobationletter.TheADAAwillacknowledgethestudent’splacementonacademicprobationandtheADSAwillreferthestudenttotheSPCforameeting.Forprobationresultingfroma“NoPass”duringanExtendedLearning,theSPC(inconjunctionwiththeADAAandADSA)willrecommendtermsofprobationtotheDean.Forprobationresultingfroma“NoPass”foranExperientialBlock,theSPCwillrecommendtermsofprobation,basedoninputfromtheDirectorofExperientialEducation,theADAA,andtheADSA,totheDean.UponapprovalbytheDean,theSPCwillspecifythetermsofprobationtothestudentinawrittendocument.Includedinthisdocumentwillbe:
1 astatementinformingthestudentoftheircurrentacademicstandingandthatthefollowingwillresultindismissalfromtheprogram“notingoodacademicstanding”:
• notpassingtwoAPPEblocks,• notpassingthreeExtendedLearningsessionsinonesemester,or• notpassingacourseforasecondtime
2 astatementreiteratingtheSchool’sWithdrawal/DismissalPolicy.3 informationregardingthestudent’srighttotheappealprocess.4 Thisdocumentwillbeprovidedtothestudenttosignandreturnacopyoftheletter.A

Undernormalcircumstances,astudentwillberemovedfromacademicprobationoncehe/shehassuccessfullypassedallextendedlearningblocksandmetallotherprobationrequirements.Foracademicprobationresultingfroma“NoPass”inanexperientialblock,theADAAwillcommunicatethetermsofprobationinawrittendocumentforthestudenttosignandreturnacopy.AcopyoftheletterwillbesubmittedtotheDean,thestudent’sacademicadvisor,andothernecessaryUniversityoffices,suchastheRegistrar.Studentswillremainonacademicprobationuntilsuccessfulcompletionofallprogramrequirements(i.e.thedurationoftheirP3year).ThisletterwillalsobecommunicatedwiththeSPC,whowillmeetwiththestudenttoprovideadditionalfeedbackanddetermineanyadditionalprobationaryterms.
ProbationforProfessionalorPersonalMisconductSchoolofPharmacystudentswhoexhibitinappropriateprofessionalorpersonalbehaviormaybeplacedonprobationforProfessionalorPersonalMisconduct.Inappropriateprofessionalconductexhibitedwhileinenrolledintheprogram(e.g.on-campus,atSOPeventsoroffcampusprofessionalevents,oratanyIPPEorAPPErotationsite)shouldbereportedtotheADSAforevaluationandtriage.Inappropriateprofessionalorpersonalbehaviorincludes,butisnotbelimitedtothefollowing:excessiveattendanceissues(unexcusedabsence;tardiness);disruptivebehaviorinclass;inappropriateordisrespectfulbehaviortowardfellowstudents,faculty,staff,preceptors,orstaff/employeesatpharmacypracticesites;inappropriateordisrespectfulinteractionwithpatients;andunprofessionaldress,language,orinappropriateconductasdefinedbytheSchoolortherespectivepharmacypracticesites.TheADSAwillworkwiththeappropriateADandfacultyadvisortoreviewtheincident.Ameetingwiththestudentmaybeheld.TheADswillresolvetheincident,orrefertotheSPC.IfprobationisrecommendedbytheSPCandapprovedbytheDean,theADSAwillacknowledgethestudent’splacementonProbationforProfessionalorPersonalMisconduct,andtheStudentProgressionCommitteewillspecifythetermsofprobationinawrittendocument,includingfurtherdisciplinaryactiontobetakenshouldthetermsofprobationnotbemetwithinthespecifiedtime.AllrecommendationswillbesubmittedtotheDeanforfinalapproval.Thisdocumentwillbedeliveredtothestudent,theDeanandfacultyadvisor.
SuspensionSuspensionisafullyeardisciplinaryleavethatdoesnotallowthestudenttoprogressintheprogram.Suspensionofastudentisaseriousactionandisonlyconsideredinsituationsofacademicdifficulties,orforprofessionalorpersonalmisconduct.TheStudentProgressionCommittee,afterappropriatereview,mayrecommendsuspensionofastudentfromtheprogramtotheDean.IftheDeansupportstheSPC’srecommendationforsuspension,theADSAwillnotifythestudentofhis/hersuspensioninwriting,includingthetermsandconditionsofthesuspension.Ameetingwiththestudentandhis/heradvisorwillbeheldinareasonabletimeframe.This

decisionwillalsoincludethelengthoftimeforwhichthesuspensionwillbeenforced.Duringtheimposedsuspension,thestudentisprohibitedfromattendingorparticipatinginanyinstructionalsessions(eitherintheclassroomoronexperientialrotations)oranyPacificUniversityorSchoolofPharmacyeventsthatarenotopentothegeneralpublic,includingeventsinvolvingstudentorganizations.Thestudent’sstatuswillbecommunicatedtotheORandWABoardsofPharmacy(BOP)bytheADSA.ThestateBOPswilldeterminethestatusoftheinternlicenses.Followingthesuspensionperiod,thestudentshallrequest,inwriting,permissiontoreturntotheprogramtotheADSA.TheDean(inconsultationwiththeADSAandADAA)shallconsidertherequestandshallprovideadetailedrecommendationtotheDeanforapproval.
Dismissal/TerminationTheSchoolreservestherighttoterminatetheenrollmentofastudentatanytimeinordertosafeguarditsstandardsofscholarship,professionalandpersonalconduct,andorderlyoperation.Actionswhichinanywaythreatenorendangerthepersonalsafetyand/orwell-beingofselforothers,orwhichdisruptorinterferewiththeorderlyoperationoftheSchoolarecauseforimmediateterminationofthestudent.AstudentwhohastheirenrollmentterminatedmaynotbereinstatedtotheSchoolunderanycircumstances.
WithdrawalandDismissalProceduresAttendanceattheSchoolisaprivilegegrantedinconsiderationofspecifiedlevelsofperformanceandofmaintainingtheestablishedstandardsofscholarshipandpersonalandprofessionalconduct.TheSchoolreservestherighttorequireastudenttowithdraworbedismissedatanytimeitdeemsnecessarytosafeguarditsstandardsofscholarship,conduct,andorderlyoperation.
VoluntaryWithdrawalApplicationforvoluntarywithdrawalfromtheSchoolmustbemadeinwritingtotheADAA.Exceptinrareandspecialcircumstances,theapplicationwillbeaccompaniedbyapersonalinterviewwiththeADAA(andtheADSA,asappropriate).Everyeffortshouldbemadebythestudenttoassurethatnomisunderstandingsorerrorsoccurinthewithdrawalprocess.Followingnotificationbythestudentandthepersonalinterview,theADAAwillnotifytheDeanandprovidethestudentwiththeformsnecessarytoprocesstheofficialwithdrawal.Withdrawalisnotcompleteuntiltherequiredformsaresignedbythestudent,anadministrativefacultymember,andtheDean.TheprocedureforVoluntaryWithdrawalisasfollows:
1 Thestudentmustfirstconsultwithhisorherfacultyadvisor.2 ThestudentmakesawrittenrequesttotheADAAtovoluntarilywithdrawfromthe
School.3 TheADAAnotifiestheDean,preparesthenecessaryformsforwithdrawalandmay

5 ThestudentmustturnintheirBoxerIDcardtotheADAA.6 Thecompletedwithdrawalformandtermsofwithdrawalaresignedbythestudentand
returnedforsignaturetotheADAAwhoshallforwardthemtotheDeanforsignature.7 TheORandWABoardsofPharmacywillbeinformedofthewithdrawal.Thestudent
shouldfollowtheirrespectivepoliciestosurrendertheirinternlicense.8 Onceallformsaresignedanddated,thewithdrawalprocessiscomplete.9 Inasituationthatastudentdoesnotcomplytheabovewithdrawalprocess,the

MandatoryWithdrawalAsstatedinthePacificUniversityCatalog(http://www.pacificu.edu/about-us/offices/registrar/academic-catalog),underAcademicandRegistrationPolicies,studentswhomustwithdrawforhealthemergenciesorotheremergencyreasonsmaybegrantedaWithdrawal.AstatementfromahealthcareproviderorotherdocumentationmayberequiredforaWithdrawal.TheDeanmaywithdrawastudentfromtheSchoolifthestudenthasmentalorphysicalhealthproblemsand/orengagesinorthreatenstoengageinbehaviorthat(a)posesadangerofcausingphysicalharmtothestudentortoothersor(b)directlyandsubstantiallyimpedestheactivitiesofothermembersofthecampuscommunity.TheprocedureforthesewithdrawalsiscontainedintheUniversityStudentHandbookandisinaccordwiththeguidelinesoftheAmericanPsychiatricAssociationandSection504ofTheRehabilitationActof1973.Re-enrollmentisatthediscretionoftheVicePresidentofStudentLife,inconsultationwithappropriatehealthprofessionals.

6 Iftherequestisgranted,thestudentshallsignanddatetheappropriatedocument.ThestudentshallthenreturnthecompleteddocumenttotheADSA.
7 ThestudentmustmeetwiththeDirectorofFinancialAidtoverifythatthestudentdoesnothaveanyoutstandingfinancialobligationstotheSchool.

Leavesofabsencerequestedforafullacademicyearwillbeforoneyearonlywithexpectedreinstatementatregistrationforthefollowingyear.LeavesofAbsencerequestedmorethanonemonthafterregistrationforanygivenacademicyearwillbegrantedforaperiodnottoexceedthenumberofmonthsremaininguntiltheregistrationdateforthenextacademicyear.UnlesscircumstancesdeterminedbytheDeanwarrant,studentswhoaregrantedleavesofabsencere-enteratthebeginningofthenextacademicyearandregisterfortheentireacademicyear,includingallblockspreviouslycompletedandpassed.Leavesofabsencemaybeextendedtoamaximumof24monthstotal(eitherconsecutivemonthsorcumulativetime).Studentsareencouragedtofocusonaddressingtheirhealthand/orpersonalissuesthatresultedintheLOA.Therefore,theyarediscouragedfromauditingregularSOPclasses,enrollinginelectivesorparticipatinginstudentorganizationactivities.TheDeanwilldeterminethetermsandconditionsoftheleave.Astudent,whoisgrantedaLOAforanentireacademicyear,mustsubmitaletterofintenttoreturntoclassestotheADSAatleastthreemonthspriortotherequesteddateofreturn.Itisthestudent’sresponsibilitytokeeptheADSAinformedofanychangeofaddresswhileonaLOA.Ifthestudenthasnotpaid100%ofthetuitionduringtheyearinwhichtheleaveisgranted,thebalanceofthetuitionplusanyincreaseintuitionorfeeswillbepayableinthenextyearofattendance.IndependentStudyContract(ISC)AnISCmaybeanoptionforstudentswhoareonatemporaryleavefromtheprogram.TheISCisaprogramwherestudentsworkwithanassignedfacultymemberonatopicofinterestforaperiodofatleast9weeks.TheISCtopicmayalsobeanareathatthestudentwouldliketoimproveonduringtheirtimeawayfromtheprogram.ISCsmaybetakenforupto6creditsa

StudentAppealProcesses
AstudentmayappealSchooldecisionsregardingacademicprogression(including,butnotlimitedtosuspensionanddismissal)totheCollegeofHealthProfessionsStandardsandAppealsCommittee.AllrequestsforappealsofrulingsbytheDeanoftheSOPshallbesubmittedinwritingtotheVice-Provost/ExecutiveDeanoftheCollegeofHealthProfessionswithinten(10)UniversityworkdaysaftertherulingfromtheSchoolisreceivedbythestudent.
Studentsarepermittedtoparticipateindidacticcurricularactivities(i.e.attendclassoncampus)andAPPErotationsduringtheappealprocess,butarenotallowedtoparticipateinintroductoryexperientialeducationalactivities(i.e.IPPErotationsuntiltheappealprocessisover.Appealsmustbeexplicitlyjustifiedforatleastoneofthefollowingreasons:1)Evidenceofsubstantial,materialerrorinprocedurebytheSchoolofPharmacy.Theerrormustbeofsufficientgravitytoconstitute:
(i)atotaldeparturefromprocedure
(ii)adefectthatpreventedthegivingorreceivingofnecessaryandrelevantinformation,or
(iii)alackofneutralityonthepartofthehearingauthority
2)Newevidencethatisboth(i)unavailableatthetimeoftheoriginalhearing,and(ii)thatissufficienttoalterthedecision.(Bothmustbemettoinvokethisbasisforappeal)

GeneralStudentsareinaprofessionalprogramandexpectationsforparticipationandattendancearesimilartoemploymentresponsibilities.Additionally,activeparticipation,whichisvitaltoteamdynamicsandsuccess,isexpected.Attendanceisrequiredatallscheduledexaminations,re-examinations,extendedlearningperiodsandexperientialactivities.Absencefrominstructionalperiodsforanyreasondoesnotrelievethestudentfromresponsibilityforthematerialcoveredduringthoseperiods.Attendancerequirementsduringinstructional/didacticsessionsareatthediscretionoftheblockcoordinatorandmayvarybetweenblocks.Studentsshouldconsulttherespectivefacultyandcoursesyllabifordetailsrelatedtoattendancerequirementsforeachblock.MorethanTwoConsecutiveAbsences:Expectationsforthecommunicationofabsencesortardinessshouldfollowdirectionsprovidedineachindividualblocksyllabus.Ifastudentis/willbeabsentformorethantwoconsecutivedaysfromanexperientialactivity,oradidacticblockwhichrequiresattendance,he/sheisrequiredtocontacttheADAA.
ExcusedAbsenceAbsencefromscheduledexaminations,re-examinations,orextendedlearningblocks,issubjecttoapprovalbytheADAA(whomayconsultwiththeCourseCoordinator)whoshallconsidertherequestanddeterminewhetheranexcusedabsenceiswarranted,andmaybepermittedonlyunderthefollowingconditions*:

Ifanabsencefromascheduledexaminationorre-examinationisexcused,thestudentmaybeassessedusingadifferentexaminationinstrumentatatimesetbytheblockcoordinator.Studentswithexcusedabsenceswillbegiventhesameexaminationopportunitiesasstudentswhowerepresentattheexaminationorre-examination.TheADAAwilldetermineifthestudentwillhavetheopportunitytoearngrouppointsmissedduetoanexcusedabsence,uponconsultationwiththeCourseCoordinator.Thisisassumingthestudent’sgroupreceivedthepoints.Pointsmaybeearnedbycompletionofanassignment/activity,asassignedbytheADAA.Ifthestudentreceivedgrouppoints,eitherbyparticipatingintheoriginalexamorbyalternativemethods,thosepointswillcarryovertoamakeupre-examthatthestudentmissedduetoanexcusedabsence.Theblockcoordinatorwillworkwiththestudentandscheduleamakeupexaminationassoonaspossiblefollowingthestudent’sreturntoschool.ThedateandtimeofthemakeupexaminationwillbecommunicatedtotheADAAandmayormaynotbescheduledduringregularSchoolhours.Foranyunexcusedabsencefromascheduledexamination,thestudentwillberequiredtoattendthescheduledre-examination.Thestudentwillnotbeentitledtoreceivegrouppoints.Thestudent’sperformancewillleadtoeitherpassingthatexaminationorattendingascheduledextendedlearningblock.Foranyunexcusedabsencefromascheduledre-examination,thestudentwillberequiredtoattendthescheduledextendedlearningperiodandbeassessedonthosecompetenciesatthattime.Morethanoneunexcusedabsenceisnotpermitted.Followingasecondunexcusedabsence,thestudentwillnotbeallowedtositforthere-examinationandwillbereferredtotheADAA.StudentswhocannotsitforanexaminationorothercurricularrequirementsbecauseofreligiousobservancesareresponsibletonotifytheADAAinwritingnolaterthanthefirstweekofclassesofeachsemester.ExtendedExcusedAbsencePolicyIfaparticularlifeexperiencehappenstoastudentduringtheprogram,includingbutnotlimitedtoanaccident,severeinjury,illness,pregnancy,etc.,thestudentmaybeeligibletoreceiveanexcusedabsenceofuptothreeconsecutiveweeks(i.e.atotalof15consecutiveUniversityworkdays)duringanacademicyear.Inordertoreceiveanexcusedabsence,astatementfromthestudentand,dependinguponthelifeexperience,astatementfromthestudent’shealthcareprovidermustbeprovidedtotheADSAidentifyingtheneedforanabsenceofupto15consecutiveUniversityworkdays.Ifthetimeframeislongerthan15consecutiveUniversityworkdays,thestudent’srequestwillbeconsideredarequestforLeaveofAbsence(LeaveofAbsencesection).

ClarificationsDuringExamsInterpretationofexamquestionsispartofthestudentlearningassessmentprocess.Studentsarenotallowedtoaskanyexam-relatedquestionsduringtheindividualorgroupexaminations.Studentsareonlyallowedtoraisetheirhandduringanexaminationifastudenthasacomputerproblem,orhasanemergencyrelatedissue.Studentsareexpectedtoreadthequestioncarefullyandtrytheirbesttoanswerit.
PersonalPropertyOn/AboutStudentsDuringExamsOnlytheitemsclearlydesignatedbytheinstructor(e.g.pencil,calculator,charts,andprovidedscratchpaper)arepermittedatthestudent’sdesk.Books,notebooks,papers,handouts,andclass-relatedmaterials,aswellasanypersonaltechnologicalitemssuchascellphoneplayers,iPodorothermp3players,handbags,andbookbagsmustbekeptinthestudent’sindividuallocker.
CellularPhoneandOtherCommunicationDevicesDuringExamsCellularphones,pagers,andanyothertwo-waycommunicationdevicesmustbeturnedoffandkeptwithpersonalitemsinthestudent’sindividuallocker.Astudenthavingacellphoneinhis/herimmediatepossessionorinexaminationroomsduringtheexaminationorthegroupexaminationwillforfeithis/herrighttoparticipateintheexaminationorteamexamination,andwillreceivenocreditforeitherexamination.Possessionofsuchitemswillbeconsideredaprofessionalismviolation.Ifastudentneedstobereachedduringanexambyanoutsideparty(e.g.,spouses,children,etc.)foranemergency,thestudentshouldcontacttheADAAbeforetheexamtodeterminepotentialsolutions.
IssuesPreventingExamParticipation:EvaluationsStudentswhofailtosubmitrequiredcourseand/orinstructorevaluationsasassignedonthreeincidencesthroughouttheacademicyearwillnotbeeligibleforgrouppointsonthenextexam.Thestudentwillbeallowedtoparticipatewiththeirgroupsonthegroupexam,butnogrouppointswillbeawardedforthatindividual.Anyadditionalmissedrequiredevaluationswillresultinthestudentnotbeingeligibleforgrouppointsonthenextscheduledexaminationaswell.Thescheduleforallblockandfacultyevaluationsisprovidedtostudentsatthebeginningofeachsemester.

Repeatexaminationsorotherhelpsessionsmayoccurattimesandplaceswhichareknownthroughoutthestudentbody,whichmayallowclassmatestosurmiseone’sparticularacademicstandingbyvirtueofattendanceattheseexaminationsorhelpsessions.Inaddition,duringre-examinationsorextendedblockexaminations,P1andP2studenttaketheirexaminationsinthesameroom.ThefactthatstudentsmaydeducethisinformationaboutanotherstudentisnotaviolationoftheirrightsundertheFederalEducationRightsandPrivacyActof1974(FERPA)bythefaculty,staff,oradministratorsofthePacificUniversitySchoolofPharmacyProgram.Duetothenatureoftheprogram,weaskthateachstudentreviewthere-examinationandExtendedLearningpolicyandsignawaiver/releaseforthedurationoftheprogram.Anyviolationoftheaboveexampolicieswillforfeitastudent’srighttoreceiveanypoint(s)andmayresultinacademicandprofessionalsanctions.StudentProgressionPoliciesandProceduresThesepoliciesandproceduresincorporatetherequirementsforastudenttosuccessfullycompletetheprogram.Successisdefinedasthesatisfactoryprogressionofastudentthroughtheacademic,professional,andexperientialaspectsoftheprogram.TheStudentProgressionCommittee(SPC)administersthesepoliciesandprocedures,withtheassistanceoftheSchoolofPharmacyfaculty,staff,andadministration.

UponaccrualofoneEL,theADAAwillsendthestudentanemailcopyinghis/heradvisorandencouragethestudenttoreachouttotheiradvisorassoonaspossibletodiscussanyhelptheymightneedanddevelopanyplansnecessarytosucceedmovingforward.Duetotheacceleratednatureoftheprogram,itisimportantforanydifficultiestobeaddressedswiftlyandforassistancetobesoughtearly,asneeded.

5. Withanyissuesurroundingacademicdishonestyoccurringduringadidacticcourse,theADSAwillnotifythestudentandfacultyadvisorinwriting,byemail,oftheprofessionalissueinquestionandmayreferthestudenttotheSPCaftermeetingwiththestudent.
6. WithanyprofessionaloracademicissueoccurringduringanexperientialcourseoranySchoolofPharmacyrelatedactivityorevent,thestudentandfacultyadvisorwillbenotifiedinwriting,byemail,oftheprofessionalissueinquestionandthestudentmaybereferredtotheSPCbytheADSAafterconsultingwiththeappropriateindividual,suchastheADAA,DEE,etc.

HearingproceduresforAcademicperformanceandUnprofessionalBehavioraredescribedasfollows:
AcademicPerformanceHearing
Within5Universitybusinessdaysoftheinitialreview,thestudentisnotifiedofthedayandtimeofthehearing,whichwillbescheduledforatimethatdoesnotconflictwiththestudent’sclassscheduleorotheracademicactivities.ThenotificationshouldincludealistofCommitteememberswhowillbeinattendancesothestudenthasanopportunitytoidentifyanyconflictofinterest.(SeeAPPENDIXGfordefinitionof“ConflictofInterest”)Concurrently,Committeemembersaregiventhenameofthestudenttobereviewedinordertoidentifyapossibleconflictofinterest.PotentialconflictsofinterestshouldbedisclosedtotheChairwithinareasonabletimeinadvanceofthehearingtoavoidadelay.

Re-examinationandExtendedLearningIfastudentearnsfourExtended-Learnings(blocksand/orlongitudinalcourses)duringasemester,mandatorydismissalfromtheprogramwilloccur.Thestudent’sstatuswillbedismissed“notingoodacademicstanding”andthestudentmayrequestre-admissionthroughtheSchool’sAdmissionsApplicationprocess(SeeMandatoryDismissalsection).Studentsfailingtopassthreeextendedlearningblocksduringasemesterwillbedismissedfromtheprogram.Studentsfailingtopassoneortwoextendedlearningblocksmaynotprogresstothenextterm.Ifthestudentintendstoremainenrolledintheprogram,theblock(s)mustberepeatedthenexttimetheyareoffered,whichwillresultinadelayinthestudent’sgraduationdate.

Studentsarerequiredtoundergoabackgroundcheckanddrugtestatthefollowingthreetimes:

UNIVERSITYBUSINESSDAYS–Whendefiningtimelines,“UniversityBusinessDays”isdefinedasdaysthatarenotSaturdayorSundayanddaysthatarenotUniversityrecognizedholidays(HumanResourceswebsite).UniversityBusinessDaysmayincludedayswhenclassesarenotinsession,buttheUniversityisopen.

StudentswhowithdrawcompletelyfromallclassesmaybeeligibleforarefundofalloraportionoftheirUniversitycharges.ThepolicyofrefundingUniversitychargesreflectstheformulaprescribedinFederallawthatgovernsthereturnofFederalfundstotheappropriatefinancialaidprograms.Refundsonallchargeswillbeproratedonaperdiembasisuptothe66%pointinthesemester.Studentswhowithdrawbeforethefirstdayofclasseswillberefunded100%oftheircharges;studentswhowithdrawafterthe66%pointinthesemesterwillnotbeeligibleforarefund.AcopyoftherefundscheduleisavailablefromtheFinancialAidOffice.
