THE ADDENDUM IS ISSUED FOR THE PURPOSE OF AMENDING THE REQUIREMENTS OF THE REQUEST FOR PROPOSALS AND IS HEREBY MADE A PART OF SAID REQUEST FOR PROPOSALS TO THE SAME EXTENT AS THOUH IT WERE ORIGINALLY THEREIN.

Key Personnel for the Project: Submit Attachment 3, which identifies the individuals to be assigned to the Project for its

entire duration as Key Personnel. The required titles of Key Personnel for the Project are set forth in Attachment 3. Minimum requirements for individuals assigned to the Project as Key Personnel are set forth in Exhibit C to the Contract, and Section III of the RFP.

Submit a resume for each individual identified in Attachment 3. The resume shall detail

the individual’s managerial and technical qualifications, as well as his/her experience with similar projects.

Please note that only the proposer (not the subconsultants) is required to complete

SF330 section E and G to demonstrate their Key Personnel’s experience.

Individuals serving as the Project Executive, Resident Engineer and the Office Engineer must be employees of the proposer.

3. Strategic Approach: Provide a three page statement describing the proposer’s strategic approach to the Project, including (1) its understanding of the technical issues and complexities of the Project, (2) its methodology for tracking and maintaining the Project’s budget and schedule, and (3) its techniques for problem solving.

4. Organizational Capability: Demonstrate the organizational capability of the firm. The

proposer and its subconsultants shall provide the following information in Part II and Part I, Section H of Standard Form 330. Part II a) The number of full-time people currently employed by the firm. Part I, Section H a) The projects on which the firm is currently working on. b) Up to 10 projects the firm has completed in any scope that demonstrates the company’s

organizational capacity. c) Future projects to which the firm is committed.

All project information shall include the dollar value of the contract, and the year that was completed. Please note that only the proposer (not the subconsultants) is required to complete the above referenced sections of the SF330.

5. Standard Form 330 (“SF 330”)

The Proposer must complete all sections of Part I and Part II in its entirety of the Standard Form 330. The subconsultants must complete Part I, Sections A, B, F, and I. In addition, subconsultants must complete all of Part II. The form is available at https://www1.nyc.gov/assets/ddc/downloads/SF330-16e.pdf

6. Statement of Understanding and Certification: The Statement of Understanding and Certification form included as Attachment 1 of this RFP should be signed by a responsible partner or corporate officer of the proposing firm and submitted with the firm’s Technical Proposal.

D. Proposal Package Contents (“Checklist”):

The Proposal Package should consist of the following THREE packages:

1. Technical Proposal (1 original*, 3 copies and 1 electronic version saved as a PDF on a clearly

marked compact disc (CD) or a clearly marked USB Flash drive). Sealed envelope, clearly marked as “Technical Proposal”, including:

Certification of Compliance with Iran Divestment Act
