STEP 2: Fill-in Application form 2.1. Once at the K12- Overview screen, go to the K12- Admission tab at the menu bar.

2.2. From the sub-menu, choose Admission Assistant, then Application Form.

2.3. Fill-in all the information fields in and click on the NEXT button at lower right corner of

the screen. Follow the STEPS under Admission Assistant.

2.4. On the Step 4 at the Admission Assistant, follow the instruction FOR ONLINE

APPLICATION and click on NEXT.

2.5. On the Step 5 at the Admission Assistant, read the Declaration, Check the checkbox if you agree on the conditions stated and click on FINISH at the lower right corner of the screen.

Note: After finishing steps 1 to 5, the system will direct you to the CONFIRMATION page which displays your Admissions Confirmation Number or Adcon Number and the accomplished Application Form.

STEP 3: PROGRAM TAGGING FOR GRADE 11

3.1. Since Admissions Test is waived for SY 2020- 2021, incoming Grade 11 students need to SKIP the K12- ENTRANCE EXAM BOOKING and proceed to TRACK TAGGING.

3.2. Fill-in the Track Tagging fields and click on CONFIRM.

STEP 4: Submission of enrollment requirements and duly accomplished enrollment forms.

Note: New Student will receive a system generated email confirmation with the ADCON No. and other instructions to complete the enrollment process. The email also contains this link https://bit.ly/FEUCEnrollmentForms to a google drive to access the enrollment forms. Follow the instructions in the email.

4.1. Download and accomplish the following forms:

✓ Medical and Dental form ✓ Waiver for Credential Form ✓ Scholarship and Discount Form ✓ Data Privacy Consent Form ✓ Bureau of Immigration Form ✓ Schedule of Fees (for reference only)

4.2. Email accomplished forms and the minimum requirements to [email protected].

Note: All requirements should be submitted in soft and hard copies together with the application form. Email the available soft copies of requirements and accomplished forms to [email protected] for evaluation. Make sure to duly accomplish and affix your signature on the Waiver for Credential Form in case of incomplete requirements. Send all the hard copies via courier (on personal account) or hand carry to the Registrar’s Office. We will start accepting your documents starting on the following dates and until two (2) months after the start of the respective classes:

• Aug. 3, 2020- Senior High School Dept. (SHS) • Aug. 5, 2020- Basic Education Dept. (BED)

Please allow us with 1-2 days to evaluate the completeness and correctness of your submitted requirements and forms, as well as to prepare the assessment of fees with applicable discounts.

5. MAKE PAYMENT:

4.1. Refer to the Schedule of Fees that will be sent via email. Pay a minimum down payment of Php 10,000.00 to be able to enroll this school year. Student has the option to pay in cash to avail of discounts. The Schedule of Fees will serve as a guide to plan the payments and assess the extent of discounts for cash or early enrollment.

Note: Student may reserve for the enrollment slot for only Php 2,000.00 and will be guided with the progress of the reservation to enlistment and enrollment when he/she is ready to do so.

Whether partial or full payment, please note below the payment methods that you can choose from:

4.2. Scan/email a copy of the deposit to [email protected]. Please indicate the name of the student, preferred course, and include in the subject “Enrollment ” or "Reservation".

Note: We will send an acknowledgement receipt once we have verified your payment. The official receipt will be issued once we resume office operations. The reservation fee is nonrefundable and nontransferable, but it will be deducted from tuition.

6. Issuance of Certificate of Registration. Please allow 1-2 days after the acknowledgement of the payment made to receive eCOR (Electronic Copy of the Certificate of Registration) via email. This will now serve as a proof of official enrollment to FEU Cavite. Request for printed COR can be made by visiting the Registrar’s Office.
