Section 2: Aims and objectives

Provider’s aims and objectives for the quality of care.

(Philosophy of care, culture of service delivery, how staff will be supported, sustainability and financial stability, staying up to date with best practice, demonstrating continuous improvement.)

Kingswood Surgery is situated in a recognised area of deprivation within an established area of Swindon. We provide a service to a wide age range of patients from a diverse ethnic background.

Kingswood Surgery have a stable workforce of highly experienced personnel across all areas of the business; this allows us to provide a high standard of care to our patients.

The Practice aims to:

Provide high quality, patient responsive services, as measured by the Quality and Outcomes Framework and other appropriate audits etc using the resources available to us.

Assess all Enhanced Services offered to the Practice and take part as appropriate.

Ensure all patients suffering from Chronic Diseases receive a planned programme of care; where they are given the opportunity to be involved in decisions made relating to their health.

Ensure all patients receive a Holistic approach to their health care to ensure that appointments/reviews are organised in such a fashion to allow a "one stop shop" approach.

Provide an opportunity for both Nursing and GP Students/Registrars to work at the Surgery to complete training opportunities.

Meet together on a regular basis with Multi-Disciplinary colleagues to allow the opportunity to review services offered to ensure review where required and as a result continuous improvement.

Encourage all staff to keep up to date with best practice via attendance at training sessions both external and inhouse along with personal reflection.

Provide a working environment that allows all personnel to develop and grow to their full potential, ensuring that individual workloads are managed and monitored to ensure an appropriate work/life balance.

Work with the Patient Participation Group to be responsive to requests received,working together to develop new ideas where appropriate.

Ensure a level of succession planning in readiness for Partner retirement to ensure the least disruption to services.

Work with the local Pharmacy to provide a high level of service to our patients in relation to issues relating to medication.

Organise regular strategic meetings to allow the Senior Management Team (Four Partners and Practice Manager) to plan for the future to ensure the Practice maintains financial stability to allow practice income to remain at a level to enable staffing levels and services to be maintained.

Section 3: Description of service

Provider’s capacity and capability to ensure compliance with the regulations when providing services for people who use them.

(What services are provided, who for, how the will provider work with others, how people who use services will be involved in shaping their care, how will people be safeguarded, how outcomes will be achieved.)

The practice provides a range of appointments, including both pre-bookable routine and urgent, to meet patient need. We are also able to provide a Housebound Service to patients unable to attend the Surgery; this is provided by one of the Practice Nurse Team. The Service is offered to both long term housebound patients as well as those who find themselves temporarily housebound due to illness/recovery from surgery. Wherever possible we offer the patient the interaction they feel the most appropriate eg:

Face to Face consultation with a Practice Nurse

Face to Face consultation with a Health Care Assistant

Face to Face consultation with a Phlebotomist

Opportunity to discuss an issue over the telephone with a GP

Opportunity to discuss an issue over the telephone with a Nurse Practitioner

Where patients are not able to attend the Surgery a Home Visit by the GP will be organised and if appropriate use is made of the SUCCESS Visiting Service which is provided to Swindon Practices by the Swindon Clinical Commissioning Group.

As a result of an interaction with a Clinician a referral may be required; if this is the case then it is made in the most appropriate fashion using the tecnology available to the Practice. The patient is given choice of Treatment Centre and involved in the selection of date and time for appointments where this is available.

In addition the practice provides:

1. Gynaecology Clinics to provide advice and treatment relating to contraceptive services including Implants and IUD's.

2. Child Health Survellance

3. Childhood Immunisations

4. Minor Operations including a regular Wart Clinic

5. Joint Injections

6. Vaccinations including advice relating to travel outside of the UK

7. Counselling Services

8. Midwifery Services

11. Patients who have been classed as "Violent" due to previous behaviour an opportunity to be registered at the Surgery.

In order to provide the full range of Services listed above we work with colleagues from other Agencies within the NHS/Public Health.

The Surgery also works with the Community Service Provider - SEQOL via the Community Matrons, District Nursing Team, Community Phlebotomist.

Regular meetings occur at the Surgery to ensure a high level of communication between both the internal Team and our external colleagues. Following the meetings minutes or notes are created and circulated recording the discussions that have taken place, any action points and who is responsible for completing the action point. Any individual who is not able to attend the meeting receives a copy of the mintues/notes to ensure that they are kept informed/aware of any discussions/outcomes that have taken place.

We receive feedback from patients on a regular basis via the monthly Newsletter that is available for them both at the Surgery and local venues. The back page of the Newsletter gives an opportunity for comments/suggestions to be made which are then passed to the Practice Manager for review/further discussion/action.

Regular staff meetings take place where all are given a formal opportunity to contribute to the running of the Surgery; informal opportunities exist on a day to day basis by general discussion with any of the Senior Management Team. In addition all staff undertake an annual appraisal which provides opportunities to discuss both personal development and Surgery future plans.

The monthly responses received from patients via the Friends and Family Test are reviewed by the Surgery and where anything has not gone smoothly futher investigations entail if appropriate.

There is a Child Protection Lead within the Surgery, Dr Andrew Slater, all staff are given child protection training at the appropriate level. All computers have a short cut link to the South West Child Protection Procedures website (updated Feb16 - so have swapped to swindon local safeguarding children board) which allows for immediate access if required.

We have a robust Complaints Procedure to ensure that patients are listened to and any concerns raised are treated in a professional fashion. The Practice Manager takes the lead with regard to complaints and ensures that they are dealt with in a timely fashion; often speaking with the patient immediately to reduce the upset they experience. There is a leaflet which provides full details of the Complaints Procedure to help patients understand their rights and how they go about raising issues/concerns.

4.2 Description of the place of service(The premises and the area around them, access, adaptations, equipment, facilities, suitability for relevant special needs, staffing & qualifications, service user groups, relevant activities and service types carried on here etc.)

The premises are located in a housing estate on Park North, Swindon close to the local College with playing fields at the back of the building. There is ample parking space both within the car park and on roads surrounding the Surgery; there are also two designated disabled parking spaces.

The premises are purpose build originally in the early 90's with extensions/development in both 2013/4 and 2011/12. The premises are on two levels with all patient services provided on the ground level with access via automatic doors to the building; there are two disabled toilets available to patients.

There is a Hearing Loop System available both at Reception and to take to appointments. There is a call system in place; however where it is not appropriate to use this patients are collected from the waiting room by the Clinician.

All staff understand the need to manage patients whose first language is not English; extended appointments are booked and where required a translation service is booked for the duration of the appointment.

All staff (Doctors, Nursing and Administration Team) working at the Surgery are suitably qualified and maintain professional registration where appropriate.
