Government of India hasnow proposed NEXT after NEET. It’s the habitof the government to shove their ideaswithout consulting the stakeholders. We haveyet to prove our worth to the governmentthat they are not willing to taking our opinionbefore announcing any health policy. Onlyour protest makes them to think back. It’s awonder what can convince the governmentto give the due recognize to our association.

Anti Leprosy Day

About Anti Leprosy Day (Martyrdom Day of Mahatma Gandhi)

30th January, the Martyrdom Day of the great person Gandhi Ji, iscelebrated every year as the Anti Leprosy Day all over the India in order to re-memorize his selfless efforts and care for the people affected by the infectiousdisease leprosy (also known as the Hansen’s disease). It is celebrated tounderstand the real need to fight against the leprosy. Anti-Leprosy Day campaignis celebrated in the schools premises by showing the presentation as well asmovie to the students to aware them towards the much infectious diseaseLeprosy.

Anyone can be affected with this disease, so the day is celebrated tomake aware the people about the scientific reasons of leprosy as well as itsmedical cures. It would be celebrated at the same day at 30th of January in theyear 2014 in order focusing on the attention of people and their awarenessabout this air-borne disease. The day is also celebrated in the memory ofMahatma Gandhi who had tried very much for the eradication of leprosy.Several programmes are organized at this day all across the India to sharethe message with people to fight against leprosy and let them know that thedisease is completely curable.

It is a disease caused by the bacteria (Mycobacterium laprae), it is notthe hereditary disease, so it is fully curable by the regular and adequatetreatment which is very necessary to cure the disease and prevent itsdisabilities.

The celebration of the day targets to aware people about disease mythssuch as it is not spread by simply touching affected person.

Objectives of Celebrating Anti Leprosy Day

� To raise the leprosy awareness among people.

� To offer help to those affected by disease through the regular and free ofcost treatment they need.

� To make the diseased person psychologically strong and help them tocope up with physical impairments of skin sores and nerve damage.

� To ascertain that all affected persons are getting the necessary treatment,rehabilitation and care or not.

� To estimate the marked decrease or increase in the rate of spreadingdisease.

National Leprosy EradicationProgramme - Tamil Nadu

National Leprosy EradicationProgramme with the help of Multi DrugTreatment (MDT) has achievedprogress of bringing down the PrevalenceRate 57 to 60 per 10000 population beforestart of MDT in 1983 to 0.66 per 10000 byMarch, 2016. Leprosy is a chronicdisease with long incubation period(average 5 to 7 years). Though thedisease has been eliminated at NationalLevel there are some Districts and blockswhich are continuously reporting PR >1per 10000 population. The major sourceof infection in the community is anuntreated case (i.e hidden case ofLeprosy) un detected in the community.Early detection and treatment will interruptthe active transmission and reduce thecomplication and disability.

January 30th is envisaged to becelebrated as Sparsh LeprosyAwareness Campaign (SLAC) in whichNationwide Grama Sabha meeting will beorganised in co-ordination with alliedsector of Health Department, PanchayatRaj Institutions, Women and ChildDevelopment etc. In this connection withSLAC January 10th, State Level Co-ordination Committee was formulatedhaving our Principal Secretary for Healthand Family Welfare as Chairman andalso released the guidelines bookletSparsh Leprosy Awareness Campaignin the presence of Mission Director,National Health Mission. �

Unfortunately many patients don’t visit a doctor whenthey develop early symptoms. In a research done by us ,thetime duration between the first symptom and first visit to thedoctor was 118days -4 months!.This is the cause for majorityof them presenting at an advanced stage, making curativetherapy an impossible goal. The survival in India at the endof first year is only around one-third which drops down to10% at the end of 5 years. This is very low compared to 40-60% five year survival in countries like japan and korea.

The main reasons for delay in presentation in india areIgnoring early symptoms;Myths people have;Socio-culturalbarriers;Self medication with their own diagnosis as ulcerdisease and belief in alternate therapies .Even if they want to, there are many other factors which do contribute: mainones being accessability to specialists and availability ofresources. As per a recent study, two southern states namelytamilnadu and kerala is supposed to have highest healthstandards but they manage only less than 10% of gastriccancer india.

Once the diagnosis is made , the management willdepend upon the stage of disease. Ther has been a greatrevolution in therapeutic strategies including the improvementsin the armamentorium of surgeons and medical oncologists.The technology support has made the curative resection apossibility . There was a time in the past , gastric cancerspresenting with a mass was not even considered for resectionalsurgery. Today the surgeon aims at curative resectionwherever possible by removing the stomach and the lymphnodes. Resection is also possible even when adjacent organslike spleen,pancreas and colon are involved. These patientswho undergo multivisceral resection survive long enoughwith good quality of life. Even in advanced stages whereresection is not possible , surgeon can offer a palliativebypass or an endoscopist can place a stent across to makethem eat or eat without vomiting .

The advances in chemotherapy has helped manypatients to lead a disease free life and prolonged survival.The recent advancement is the availability of “Targettherapies” which is useful even in advanced situations.

Though we have improved in all aspects, it is alwaysbetter to create awareness to the society on preventablecausative factors and early warning symptoms , so that wecan give them good quality long life!

THE FACTS WE SHOULD KNOW IN 2017

Gastric cancer commonly known to general public as“stomach cancer” is one of the leading Killers in the world.This is known for variations between countries and betweenthe states in our own country. It is the fourth leading cancerin the world and second most common cause of death due tocancer accounting for 7,30,000 deaths per year.

Men are commonly affected and asian countriesespecially china , japan, korea top the list in terms of incidence.There is a significant regional variation in india . Mizoramand north eastern states topping the list and the least is inGujarat. Moderate incidence is in southern states.

The common age group is after 50 years, though it isnot uncommon to see patients even under 40 yearsnecessitating the need to have a cautious approach whileexamining a patient with upper GI symptoms in the clinic.

Loss of appetite, loss of weight, fullness after small

alcohol.The role of H Pylori in the causation of gastric canceris still being debated.

Once there is a suspicion , especially in high risk areasand high risk population, it is desirable not to start them onany empirical therapy with a possible clinical diagnosis ofulcer disease or reflux disease. It is not uncommon for thepatients to have “pseudo-improvement”in symptoms, delayingthe diagnosis.

The first investigation of choice is doing an uppergastrointestinal endoscopy which will clinch the diagnosis.Once confirmed, assessment of spread can be done bycontrast enhanced CT Scan of abdomen. (Ultrasound of theabdomen is sufficient if the patient clinically has disseminateddisease). PET-Scan is done only in select situations, but itcan detect lesions upto 6 mm anywhere in the body. Lesionsin the abdomen even less than 6 mm can be diagnosed bylaparoscopy.

With a help of respective District Co-ordinators.The members can engage the PPLSSS recognisedlawyers in that District or you can engage your lawyerafter informingto the Secretary of PPLSSS in writingand after getting his acceptance.

After the District forum Judgement, if the case islost and there is any amount to be deposited, themember will deposit the amount in the court as perthe Judgement. The PPLSSS will pay the lawyers’fees. The case will be appealed in the State Forum.There are 2 PPLSSS lawyers for the Sate forum. Theywill take up case and proceed.

The deposit paid by the member in the DistrictCourt will be reimbursed by the PPLSSS after thecaseis over in the State Forum and the Judgement isreceived from the State Forum.

Depending on the State Forum Judgement, it will

be decided whether to appeal to the national forumor not by the PPLSSS.

Still we receive phone calls about theseprocedures. So I thought of repeating these points.

I received a phone call from one of our ladymember stating that she has been asked to come tothe police station regarding a complaint by a patient.When a complaint is given in a police station, theSub Inspector / Inspector will call up for enquiry.Inthis situation we must inform the branch presidentand secretary and then go and explain to them andcome. After returning from the police station, thebranch president and secretary must be informed.The District coordinator may be informed beforegoing to the police station.

Still many IMA Members have not becomePPLSSS Members. I am not able to understand whythey are not becoming Members. Rs.6000/7000/8000for 5 years for acoverage of 5 lakhs or 11000/13000/15000 for a coverage of 10 lakhs for 5 years is a verycheap indemnity which is available with legal advice.

So I request all our members GPS/Non-Surgicalspecialities / Surgical Specialities and Anaesthetiststo come forward to utilize these facilities.

HPS: We request all our members having NursingHomes to become Nursing Home Board members andthen become Hospital Protection Scheme Members.Now a days notices are sent against the hospitalsalso. HPS covers the hospitals- this is for a period of3 years

Family Benefit Scheme is utilized by our members1, 20,000 per year; 60000 at a time with a gap of 6months. This is yearly renewal.
