    Introduction

    The DHS Directory of Contracted Services is published to inform the general public, aswell as potential provider agencies, of the services contracted under DHS throughout thestate of New Jersey. The directory is organized by contracting units and servicecategories, and includes information detailing the spectrum of services provided by DHS.

    The Request for Proposals (RFPs) for new funding are published on the DHS Website athttp://www.state.nj.us/humanservices/grantan.html, along with the service to bedeveloped and the corresponding contact information. The funding for contract servicesmay come from a variety of sources including New Jersey state appropriations, federalfunds and private grants.

    The DHS has eight “Departmental Component Contracting Unit (s)” that purchase thirdparty services including the divisions of Developmental Disabilities; FamilyDevelopment; Medical Assistance and Health Services; Addiction Services; DisabilityServices; along with the Commission for the Blind and Visually Impaired and the Officefor the Prevention of Mental Retardation and Developmental Disabilities.

    DHS makes every effort to work to serve New Jersey’s most vulnerable citizens,including people with developmental disabilities, adults with serious and persistentmental illnesses, individuals who are blind and visually impaired, and people withalcohol and drug addiction problems. The services provided through DHS are madeavailable in concert with the state’s non-profit and for-profit human servicesorganizations, and the state’s public/hospital-based community providers.

    Any concerns, questions or information updates regarding the DHS Directory ofContracted Services should be made by contacting a specific Departmental ComponentContracting Unit, as well as the DHS Contract Policy and Management Office at 609-292-9469. The directory is organized and distributed annually through Contract andPolicy Management Office to assist consumers with understanding the department’scontracted services, along with related budget, legislation, process and procedure.

    Purpose of the Program: To provide legal and advocacy services on behalf ofcommission clients whose accessibility to information,programs and legal assistance is affected by their vision.

    Service Category: Independent Living for Elderly and/or Deaf/Blind

    Departmental Component Name: Commission for the Blind & Visually Impaired (CBVI)

    Authorizing Legislation: Workforce Investment Act of 1998, Title IV,Section 410 and N.J.A.C. 10:91

    Purpose of the Program: To identify seniors and/or deaf/blind adults who are blind orvisually impaired, integrate the seniors into currently existingprograms and develop peer support groups; identify, train andmatch volunteers with seniors and/or deaf/blind adults, andprovide recreational opportunities for this population.

    Funding Amount: $1,082,965

    Provider Agency Profile: Public agencies, public/private non-profit or for-profitorganizations are eligible for consideration. In addition, theagency must have experience and knowledge of the needs ofthe target population.

    Qualifications: Experience, knowledge and ability to provide services toindividuals who are deaf/blind; staff qualified to provideservice (knowledge of sign, assistive technology, etc.);community-based organization. The provider must be able toprovide services throughout the state.

    Purpose of the Program: To identify individuals who are at risk of loosing their sightvia vision screenings to prevent blindness through serviceintervention.

    Funding Amount: $92,705

    Provider Agency Profile: Private/public non-profit organizations are eligible forconsideration. In addition, the agency must have experienceand knowledge of the needs of the target population and/orvision screening/education.

    Purpose of the Program: To identify and train providers of mental health services towork with clients of the commission; to train commission staffon issues relating to mental health system; to develop, organizeand monitor self-help groups relating to blindness; and, toprovide technical assistance to commission staff on individualclients.

    Purpose of the Program: To evaluate, adapt and install equipment in employment,training, and educational setting for adults and children whoare blind; To orient and train adults and children in the use ofsaid equipment; To provide training, technical assistance andresource development to commission staff, with basickeyboarding skills and computer literacy skills to clients whoare blind; to coordinate a technical Aids center at the JosephKohn Center.

    Funding Amount: $1,947,435

    Provider Agency Profile: Private/public non-profit organizations are eligible forconsideration. In addition, the agency must be knowledgeableof and able to communicate information concerning theAmericans with Disabilities Act, technology, word processing,equipment and training (as it applies to the field of blindness)and in identifying job placement opportunities (as it applies tosupported employment).

    Qualifications: Provide one-on-one consultation to education programs and/orCommission staff about techniques, materials, etc. for teachinga visually impaired student or adult to utilize adaptiveequipment. Identify resources for the provision of adaptivematerials, aids and equipment that adults/children’s programscan utilize. Knowledgeable of the labor market andemployer’s needs with respect to skill levels and efficiency.

    Purpose of the Program: To recruit, train, match and monitor volunteers to work withpeople who are blind and visually impaired.

    Qualifications: Knowledge and experience in volunteer recruitment, trainingand matching a volunteer with an individual in need ofassistance.     Purpose of the Program: Adult training provides day programs to adults withdevelopmental disabilities who live in the community.Services include work activities; activities of daily living, andpersonal awareness. Services are aimed at enablingindividuals to live more independent lives. Services in thiscategory include adult activities, work activities training(WATC), extended employment, and supported employment.

    Funding Amount: $96,589,354

    Purpose of the Program: Adult Training Special Projects refers to programs that falloutside the more traditional range of services available foradults with developmental disabilities. It includes medical daycare, transportation contracts for adult day programs, and adultcrew labor.

    Service Category: New Placements/Case Management

    Departmental Component Name: Developmental Disabilities (DDD)

    Authorizing Legislation: N.J.S.A. 30:6D-17

    Purpose of the Program: The Housing and Resource Development Unit coordinates thedevelopment of residential servicesfor people coming off the Community Services Waiting List.Providers are requested to submit proposals toserve specific individuals, once informed of each person’sneeds; proposals are then forwarded to the individual andher/his family for their ultimate service selection. CaseManagement then coordinates the various aspects, residentialprograms, and clinical support servers. An IndividualHabilitation Plan (IHP) identifies service needs andparameters. The case manager is responsible for ensuring theIHP goals are met and revised, as necessary; the case managerinitiates periodic review of the IHP.

    Funding Amount: $26,045,000

    Service Category: Group Homes and Supervised Apartments

    Departmental Component Name: Developmental Disabilities (DDD)

    Purpose of the Program: Group homes provide community residence for (four to six)people with developmental disabilities. Supervised apartmentsserve one or two individuals (per apartment). Group homesand apartments provide services to individuals with a varietyof needs. Therefore, program patterns run the gamut fromminimal supervisions to highly structured environments.

    Funding Amount: $483,718,916

    The Office of Prevention was established by legislation for the purpose of educating the publicabout ways in which mental retardation and other developmental disabilities may be prevented.Working under the guidance of the Governor’s Council on the Prevention of Mental Retardationand Developmental Disabilities, four areas are given priority to receive support for public health:

    prevention of Fetal Alcohol Syndrome; prevention of childhood injuries; prevention of childhood lead poison; and education and recent, non-invasive pregnancy and pediatric HIV/AIDS (and immunization, may also be given consideration for funding). In addition, the Office of Prevention is charged with encouraging and stimulating research into the

  • DIVISION OF DISABILITY SERVICES

    (DDS)

    The DDS was originally established as the State Office on Disability Services in October 1997and upgraded to division status in July 2001. The division serves as a central resource forinformation referral for all individuals with disabilities and their families. The division operatesunder the authority of Chapter 91, P.L. 1999.

    DDS promotes and facilitates the maximum independence of people with disabilities throughinformation and access to services and supports. The division fosters coordination andcooperation among government agencies that serve people with disabilities. The specificfunctions and goals of DDS include:

    Operate an information and referral system; Coordinate services among state government agencies; Create a link to all state and county disability programs; Serve as a central focus for disability issues; Administer the NJ Personal Assistance Service Program; Operate the NJ Personal Preference Program; Administer the Medicaid Infrastructure Grant; Administer/Oversee Seven Home & Community Based Waiver Programs; Administer the Medicaid Personal Care Assistant Services Program; Administer the NJ Ramps Project; Administer the Traumatic Brain Injury Implementation Grant; Administer Address Violence Against Women with Disabilities Grant; and Administer Nurse Delegation Pilot Project.
