SAFETY CONSULTATION AND TRAINING SECTION

SAFETY AND HEALTH VIDEO, DVD, OR CD LENDING LIBRARY AGREEMENT

The Safety Consultation and Training Section (SCATS) of the Division of Industrial Relations provides a safety and health video, DVD, and CD lending library service free of charge to all Nevada employers. Nevada employers are encouraged to use this service to supplement employee safety and health awareness training.

The videos, DVDs, or CDs in the Safety and Health Lending Library are purchased from private production and service companies. While these companies strive to meet all safety and health regulatory requirements, there may be some inaccuracies. Please remember it is your responsibility as an employer to ensure that your employees’ training is current and meets the applicable federal, state, and local regulations.

The videos, DVDs, and CDs in the library are available on a loan out basis and are to be used only by employers to supplement their employee training programs. They are not to be used by any company charging a fee. Please be advised that the private companies SCATS purchases the material from do not allow unauthorized copies of the videos, DVDs, or CDs to be made.

CHECKOUT PROCEDURES:

Videos, DVDs, or CDs may not be reserved in advance, therefore, it is a good idea to

have more than one video, DVD, or CD in mind when checking out material at one of the three Safety Consultation and Training Section Offices. You may wish to call ahead on the availability of a particular title prior to coming to the office.

A maximum of three videos, DVDs, or CDs may be borrowed. Please be

advised that previously loaned videos must be returned before additional videos, DVDs or CDs can be borrowed.

Videos, DVDs, and CDs must be returned within five (5) working days from the date

of checkout. By returning them promptly, other employers may have the opportunity to use them.

After choosing the video, DVD, or CD desired, please sign out on each page that

corresponds to the numbers on the video, DVD, or CD. This is an acknowledgement of the Federal Copyright Laws and responsibility for any damaged videos, DVDs, or CDs not returned. Provide your name and signature, the company name and address, and your phone number.

CARE AND USE OF VIDEOS, DVDs, or CDs: Videotapes and DVDs/CDs are heat sensitive, so please do not leave them in a car

or any location where they may be exposed to high temperature.

Please keep the videos, DVDs, and CDs in the storage container provided when not

in use. The videos, DVDs, and CDs are to be used to supplement employer training.

Watching the audiovisual material for hazard awareness is a good way to initiate discussion of company policies and procedures.

No fees may be charged when using the videos, DVDs, or CDs.

Always review the video, DVD, or CD before showing it to your employees to ensure

it meets your training needs. Any inconsistencies with the safety and health regulations can then be identified and corrected during the discussion portion of your training.

Please return the videos, DVDs, and CDs as soon as you have completed showing

the tape, within five (5) working days.

VIDEO, DVD, AND CD RETURNS Return the videos, DVDs, and CDs to the same office where it was obtained.

Please rewind the video before returning.

Return videos, DVDs, and CDs in their original containers.

If there are any problems with the videos, DVDs, or CDs, please let us know when

you return them. Upon return of the video, DVDs, or CDs, please provide the number of individuals

that watched the video tape to the Safety Consultation and Training Section (SCATS) employee to whom you returned the tape.

Sign and date the video, DVD, and CD checkout book per the corresponding number

when you return the video. This verifies that the tape was received by SCATS.
