ST. MARY VOLUNTEER DRIVER REQUIREMENT

ST. MARY CATHOLIC SCHOOL ARCHDIOCESE OF DENVER

VOLUNTEER DRIVER AGREEMENT

I, _________________________________________________________, do hereby affirm that the

following statements are true:

I have a valid Colorado driver’s license which has not been suspended or revoked during the past five

years.

I understand that as a volunteer driver, my insurance is primary. I have in full force and effect

automobile liability insurance and do carry with me at all times proof of such insurance.

I have not been convicted of or pleaded guilty to driving under the influence of alcohol, driving while

ability impaired, or reckless driving during the past five years.

I voluntarily accept responsibility to provide transportation, and I accept responsibility for the safety of

my passengers, and at all times I agree to:

1. Use and insist that all passengers use seatbelts;

2. Drive carefully within established speed limits and driving conditions;

3. Keep my vehicle in safe operating conditions;

4. Never drink any alcoholic beverages or take any drug which may inhibit my driving ability within

eight (8) hours before transporting any passengers; and

5. Obey all traffic laws.

FOR OFFICE USE ONLY

Dear St. Mary Parent Driver Volunteers,

Thank you for your willingness to help us drive our students to and from various

events. The donation of the two buses has helped to ease the burden of needing

drivers, but once in awhile there is a conflict with the use of the buses and school

activities which requires a need for volunteer drivers.

The Archdiocese of Denver is asking all volunteer drivers to complete a Defensive

Driving course as part of the Risk Management and the Office of Catholic Schools

policies and procedures. Below is the link to the FirstNet Learning "Online University"

for the Archdiocese of Denver.

https://firstnetcampus.com/kaw3/entities/archden/logon.htm

Process:

Login with a user id and password (If you don't have one, follow the instructions on

the screen). Click on the CLICK HERE to ENROLL" link in the middle of the screen.

Once you are logged in, on the left hand side of the screen are all of the

recommended courses. You will click on the Defensive Driving course and then click

ENROLL. Once you complete the online course, you will need to print the certificate

of completion. The certificate of completion will be filed with your volunteer driver

paperwork in the school office.

This course will take approximately 60 minutes to complete. Please complete as soon

as possible so that we can share the information with the Archdiocese. Once the

Archdiocese gets all of the volunteer driver information, they place you on an

"approved driver" list. Once you are an approved driver, then we can allow you to

volunteer to drive the students.

Thank you for your cooperation with this new policy.

In Christ,

Mrs. Bornhoft
