Go to St. Joseph’s Health URL: https://stjosephshealth.org/portal and click the “ENROLL NOW” Link

1. Form displays: “Self-Enrollment for MyStJosephsRecord”

2. Complete all *required fields (First Name, Last Name, DOB, How would

you like to verify your account?)

3. You can verify your account one of three ways:

Select email address and enter a valid email address on record

Select text message and enter a mobile phone number on

record

Select Personal identifier(s) and enter your CMRN (Community

Medical Record Number).

You can locate the CMRN # from your Proof of Vaccination

Report, Acute Discharge Instructions or the Clinical Summary

Document

4. Select the check box “I’m not a robot” and follow re-CAPTCHA process

5. Click Next. The system searches to validate your identity

To qualify a patient match by email or text, a message containing a verification code will be sent to the email

address or mobile phone number on record. Enter the verification code. Note: Msg&Data rates may apply for

text messages

An exact match redirects to successful Patient Match Found page. Select the check box to Identify verification

and Terms of Use to continue to self-enroll and claim account. Click Next.

If a match is not found a message will display, “You don’t qualify for self-enrollment. To request an invitation,

talk to your health care organization during your next visit for an invitation”. If you need assistance or have

a week.

Create your account. If you don’t Already Have an Account, choose I Need an Account and enter your email

address and click Sign Up.

Create your username and password. Agree to Terms of Use and Privacy Policy. Click Create Account.

Your enrollment was successful and you are now logged onto MyStJosephsRecord portal page.

After completing the self-enrollment process a confirmation email will be sent to you.
