SOUTH VALLEY ELEMENTARY SCHOOL
October 17, 2013: South Valley Work Order 38638 – Report of Urine smell in wooden cabinet. Found mold
under the shelving in built in wall cabinet.
October 21, 2013: Physical inspection conducted with moisture meters and digital hydrometer‐sling
Pyschrometers. Visible mold noted on 4 of the 8 built in wall cabinets. Further inspection found visible mold
under stage and sink cabinet in staff lounge. Schedule a full inspection with air sampling
October 21, 2013: B Wing Mold Assessment IAQ performed by Quality Air Specialists
October 23, 2013: Written report from Quality Air Specialists, Inc. outlines the procedure and findings
regarding the October 21, 2013 inspection. Good color pictures for explanation.
“Visible mold and water damage were identified in two wings of the school building on October 21, 2013. A
mold assessment was conducted that day by Quality Air Specialist which included recommendations for
remediation. Additional inspection of the building October 26 and 27 identified more potentially affected
areas. A second assessment confirmed the presence of mold in other location within the building and called
for additional remedial activities.” (AIG letter of March 5, 2014)
Request for Information:
‐ CIH certification of the written recommendations for remediation from Quality Air Specialists;
‐ A copy of all invoices (or Scope of Work and estimated costs for work not yet completed and
invoiced);
‐ A copy of the final CIH inspection clearing the remediated areas for use.
The cleaning of the school began from the Teacher’s Convention until first weekend in January, seven days a
week. The way the school is laid out we were able to close off sections at a time a relocate the students and
staff.
October 26, 2013: An inspection of the entire building. Visible mold found in multiple rooms throughout the
building. There was also evidence of prior water damage due to roof leaks and plumbing leaks since the
building was built. Loose furniture also found to have mold. Loose furniture, mainly furniture with press
board type construction circa 1970’s or earlier and mold found under the tables. Another source for the mold
was found to be the crawl space was full of water. The water was naturally occurring due being built on
wetlands.

November 4, 2013: A follow up inspection was performed in the crawl space. Mold was visible and an
inspection of a suspected sewer gas smell. PID alarmed and measured VOC’s at 1,210ppm. Due to the
unknown safety concerns it was determined to evacuate the crawl space and abandon the inspection.

November 15, 2013: Meeting and walk thru with AIG Insurance Claims Adjuster and JIF Administrator’s Scott
Tennant and Bob Oldt about the Mold Claim.
November 8, 2013: PRV* B Wing IAQ performed by Quality Air Specialists
November 25, 2013: PRV CD Wings IAQ performed by Quality Air Specialists
December 28, 2013: PRV A Wing and Stage IAQ performed by Quality Air Specialists
February 24, 2014: Art Room Closet IAQ performed by Quality Air Specialists
March 17, 2014: (AIG letter regarding claim of $259, 414.54)
Invoice totaling $259,414.54 was reduced by $47,177.36 of items that either needed further clarification or
that was denied because it was not mold related. (Examples of items not paid for; Inspection of crawlspace
and water sample analysis, items replaced due to water damage or structural damage – not due to mold)
Deductible is $25,000, so payment was recommended in the amount of $187,237.18.
April 18, 2014: IAQ Surveys at all Elementary Schools
June 26, 2014: (AIG email)
AIG confirms reimbursement of $41,081.34. $12,916.76 was for school supplies that had to be discarded and
replaced. These items either could not be cleaned effectively or was not cost effective to clean. $28,164.58
was for remediation work in the stage and storage areas done during winter recess.

During multiple meetings with Quality Air, Administration and contractors, the district would remediate in 3
phases. The building was divided in to 3 sections and remediation was completed over teacher convention,
Thanksgiving break and winter break. In each phase of remediation the area was secured, all items deemed
not salvageable was discarded and all other items, walls, doors, etc. were cleaned. All HVAC duct work was
professionally cleaned as well as selected univents. Before any area could be reoccupied mold sampling was
conducted. During each phase, staff and students were moved to other areas in the building.
The district in consultation with Quality Air Specialists along with reviewing QSAC, Fire Code, Environmental
best practices , Safety and Energy best practices, started to implement the following

LESSONS LEARNED
Developed classroom guidelines targeting items that could promote mold growth such as:
‐ Limit classroom throw rugs
‐ Eliminate stuffed animals
‐ Eliminate all wicker items (chairs, baskets, etc.)
‐ Eliminate tennis balls on chair feet
‐ Limit decorations
‐ Limit house plants to educational purposes only
‐ Eliminate all personal furniture (rocking chairs, shelving, microwaves, refrigerators, etc.)
‐ Use of dehumidifiers were in limited use prior. All rooms currently use them
Replaced all furniture that deemed susceptible to mold, those with porous surfaces
Strive to have all new furniture as portable as possible. Cubbies, book shelves and wardrobe closets
on wheels. No stationary furniture and no furniture to be permanently attached. All furniture comes
out for summer cleaning. Room and furniture can be completely cleaned and inspected for mold
Planned and budgeted for the above items by requesting to keep insurance claim open until all built in
suspected furniture was removed in summer of 2014. Mold was found behind the old built in cabinets
and new cabinets were purchased through insurance. Budget for further remediation through a
referendum the following year including drainage installation around the perimeter of the building.

Classroom Item/Concern Guideline Notes
Houseplants Limit the number of houseplants due to mold, pest and allergen issues
Use for educational purposes only
Stuffed Animals Eliminate stuffed animals in classrooms due to mold, pest and allergen issues
Exception for preschool classrooms only
Personal Furniture Items - Upholstered/fabric furniture, non sealed/particle board or wood shelves, wicker, etc.
Upholstered/fabric furniture not purchased by the District (brought from home) should be removed. Shelving units and wood furniture could be conducive to mold growth or have lead in paint.
Remove all personal furniture items from classroom. Any need for shelving units should be communicated to the Building Principal.
"High Odor" Markers Use "low odor" markers only Ionizing Air Cleaners Remove all ionizing air cleaners. Report any odors to building
Principal, Business Administrator and through School Dude.
Air Fresheners Do not use air fresheners in classrooms. Can create respiratory issues for sensitive individuals.
