    Case example 1: Vicky and Paul

    Vicky is a woman with Downs syndrome, a severe learning disabilityand severe visual impairment. She was at the pre-intentional / earlyintentional stage o communication development and presented withseveral challenging and sel-stimulatory behaviours.

    I was rst approached by Paul, Vickys new 1:1 support worker atthe clients residential home, as he elt unsure how to communicatewith Vicky. I carried out our lming and eedback sessions over a ourmonth period. At the beginning, Paul described communication withVicky as hard work. He elt he would never be able to have a longerconversation with her.

    During the rst eedback session, Paul became aware o the ContactPrinciple o using a riendly voice. He had been a ootball coach and

    used the terminology o cheering Vicky on, which I reinorced. Paulsubsequently experimented with his tone o voice between lmingsessions and reported back on his success: Im so thrilled I now eelthat Vicky knows who I am!

    The nal lm was done whilst Vicky and Paul were in a busy ca.Normally Vicky did not tolerate such noisy environments and wouldstart screaming. However, on this occasion, she calmly engaged invocal and tactile turn-taking behaviours with Paul or 10 minutes. Paulnoted that Vickys communication was now much more directed athim. The video revealed how, or the rst time, Vicky touched Pauls armto gain his attention, as opposed to throwing objects or stripping o herclothes. Other sta also noticed a change in Vicky and reported that herchallenging behaviours had decreased.

    Case example 2: John and George

    John is 25. He has Downs syndrome, a moderate learning disabilityand a severe stammer. John requently isolated himsel at his DayCentre. He only engaged with his key worker and tended not tospeak to other sta or join in with any groups. His brother reportedthat, at home, John did not usually talk to visitors and would otendisappear to his room.

    I lmed John interacting with George, a less amiliar sta memberrom the Day Centre. First, I did VIG with George. Following one lmand eedback, George reported how John had talked to him in thecorridor, asking him how he was. Another breakthrough was whenJohn joined in with Georges Art Group. George had set up a table atthe back o the art room especially or John and came over to speak

    to him at regular intervals.I then carried out some VIG therapy with John himsel. To my

    surprise, John soon started talking about his thoughts and eelingsabout having a learning disability. To him it meant he was rubbishat talking. I showed John several video clips highlighting laughterand him eeling relaxed during conversations. No severe stammeringepisodes were shown. The VIG therapy gave John a real sense thatboth he and George had un and enjoyed talking to each other.

    Towards the end o therapy John thought dierently about hislearning disability: My learning disability is alright. Talking toGeorge is un! Ater three lming and eedback sessions, there weresigns that Johns communication behaviours started to change andthat these changes were carried over into dierent environments.Sta noticed that he spoke to other sta members at the Day Centre

    more oten. At home, John began to socialise more with visitors.
