About this booklet

Commissioners Working Together wrotethis booklet.

We are 8 local Clinical CommissioningGroups (CCGs). CCGs plan and pay for NHSservices for local people.

The booklet is about some specialiststroke services in South Yorkshire,Bassetlaw and North Derbyshire.

A stroke happens when blood cannot getto part of your brain. This means parts ofyour brain are damaged or can die.

Specialist stroke services are calledhyper-acute stroke services (or HASUs).They bring experts and equipmenttogether to give the best treatment justafter a stroke happens.

This can stop people dying or mean theyget over the stroke better.

Last year we asked people about strokeservices.

People said they want:

● ambulances to get them to thehospital quickly

● to see a doctor quickly when they getto hospital

● to be treated by staff who know aboutstrokes

● to have a good, safe service

● good, local care to help them getbetter and learn to do things again.

At the moment this is not happening foreveryone. People in some areas get bettercare than others. This is not fair.

This booklet explains how we think wecould give everyone a better service.

Which services would change

We only want to change services thattreat people just after a stroke happens.

We want to give a better service to peoplewho need stroke care in:

● Barnsley

● Bassetlaw

● Chesterfield

● Doncaster

● Rotherham and

● Sheffield.

We are not looking at services inMid Yorkshire (Wakefield).

We do not want to close services orchange services that:

● care for you from a few days after astroke until you can leave hospital

● services that help you get better andlearn to do things again when youleave hospital.

Why things need to change

We want all patients to get the same goodcare and treatment.

Sometimes this does not happen because:

● some stroke services treat morepatients than others. This means theyare experts and can give good, safecare

● we do not have enough doctors andnurses in the UK who are trained tocare for people who have a stroke. Thismeans we sometimes have to closeservices if we do not have enough staff

● Some hospitals deal with scans andtests more quickly than others. Thismeans some people do not get thetreatment they need as soon asothers.

We want to give all patients the samechance to have good, safe care and nothave a disability because of the stroke.

How we decided what to do

We worked with:

● NHS staff and managers from strokeservices

● People who pay for services

● People who check services.

We asked:

● can patients get to a hospital in 45minutes or less?

● if services change would services beable to treat more patients?

● would we be able to run these services7 days a week?

● how will changes affect patients andservices in other areas?

● will this give people the care they saythey want?

● is this the best way to use our staff togive good care?

We thought about 3 things:

1. If we do nothing then we cannot giveeveryone good, safe services

2. We do not have enough expert staff tomake the 5 services better

3. We need to change the way we runservices to make them better foreveryone.

We then thought about how manyservices we need.

2 or 4 services might work, but we decidedthat 3 services would be best.

If there are changes to services inChesterfield, we might only need 2services in our area.

What we want to do

We want to know what you think aboutour idea.

We would like to have 3 specialist strokecentres for people who live in:

● South Yorkshire

● Bassetlaw or

● North Derbyshire.

People would get care just after a stroke inone of these hospitals:

● Chesterfield Royal Hospital

● Doncaster Royal Infirmary

● Royal Hallamshire Hospital in Sheffield.

Barnsley and Rotherham hospitals wouldnot care for people just after they have astroke. Patients would go to Doncaster orSheffield.

Then, after the first few days, they wouldgo to their local hospital (Barnsley orRotherham if that's where they live).

Nothing will change yet. We are justlooking at how to help people get well andnot have a disability because of the stroke.
