SCREEN WISE APPLICATION FLOW
1. Login Screen
a. Theme Topology - A user can select the Application Theme (Color Scheme)
as per his/her wish.
b. User Name - User has to enter the provided user name in the
Captioned text field. In case you forgot your User Name,
please send a e-mail to Medical Health Department at
c. Password - User has to enter password. A password must be a
Combination of 10 digit Alpha-Numeric-Special Charter
Value set, which should consist of One CAPITAL
Alphabet + one small alphabet + One Special Numeric
Value + One Special Character (Like; ~, !, @, #, $, %,
^, &, *, (, ), _, -, ?) etc. e.g; Password@1
d. Captcha - User must enter CAPTCHA Code as appear in Picture.
e. Log In - Click on Log In Button to log-in the application.

3. Beneficiary Identification Form
Beneficiary Identification Screen
Option 1 – Where Admission Type = NORMAL
Step 1: Select Admission type from two values. By default, it is set to
“Normal”. Here at first, Admission Type is selected as NORMAL.
Step 2: Select either of the IDENTITY (Bhamashah Family ID / RSBY Card /
Ration Card / Aadhaar Card) and enter respective Identity ID and Click
on Search Beneficiary Button.

Step 3: Below details will append on the screen
Step 4: Enter Beneficiary’s Aadhaar Number or (Any Family Member’s Aadhaar
Number) and Click on Verify Aadhaar Button. Below Window will open
which contain various Aadhaar Verification Options.

Step 5: Select Verification Method either by Clicking on FMR or OTP.
- FMR will ask to scan finger/thumb of the person verifying his/her
Aadhaar.
- OTP will send the One Time Password (OTP) to the registered
mobile number of the person verifying his/her Aadhaar.
Step 6: Below Status will append on screen for the verified Aadhaar. If
Aadhaar is verified, status will come as Verified otherwise Not Verified.

Step 7: If Aadhaar is not verified for whatsoever reason, Beneficiary can
approach Hospital MOIC (Medical Officer In Charge) to get the duly
signed and verified document of his/her from MOIC and represent the
same at Hospital Kiosk to proceed with treatment process. And Kiosk
User will upload the document and Click on Submit Button to proceed
further. Otherwise go to Step 8 and proceed further.
Step 8: Select Patient from the Family Member Details for whom Medical
Treatment is required and he/she required to be admitted in the
Hospital.
Step 8: Enter CRN (Central Registration Number) from Aarogya Slip of OPD
and Click on Submit Button to proceed further.
Step 9: After clicking on Submit Button, a popup-window will append on the
screen which has Unique TID (Transaction Number). Please note this
number on the Patient’s records for further uses as and when required.

Option 2 – Where Admission Type = EMERGENCY
If any Patient is bought to the hospital following any Medical Emergency or during
the Hospital Emergency Hours. Below steps to be followed by the Kiosk User.
Step 1: Select if the Patient is Identified or Un-Identified.
Step 2: If the Patient is Un-Identified, as selected above please fill
- Approximate Age of Patient
- Gender of Patient

Else, if the Patient is Identified, fill below details
Step 3: If Patient being admitted is falling under MLC (Medico Legal Case)
Category, irrespective of either the Patient is Identified or Un-
Identified, please select YES from radio button otherwise select NO.
Step 4: Where MLC Case tpe is select as YES, a dropdown list will append on
screen, now user have to select type of MLC from dropdown list as the
case may be (as shown above).

Step 5: After filling the required details, as the case may be, please click on
Submit Button. System then generate a unique TID (Transaction
Identification Number). Please note the same for further use.

4. Pre Authorization Request Form
Pre Authorization Request Screen
After a Patient is being identified in the First Form “Beneficiary Identification Form”
system will automatically redirect to next process step i.e; Pre Authorization
Request Form. Please follow below steps to register the Pre Authorization Request
in order to Start the Medical Treatment as the case may be.
Step 1: Enter TID generated finally in the previous screen in TID input box as
mentioned below and press Tab key or click Mouse anywhere on the
form but outside the text input field.

Step 2: Select Package from Package List. There are two options to select a
package. User can search and select any package by either option:
- By entering the Package Code
- By entering the Package Name
Step 3: After selecting the required package, please click on Add Package
Button. The package will be selected and appear on the screen as
below snap-shot.

Step 4: Wallet details will be shown on screen as per below snap-shot
Step 5: Go to File Upload section by scrolling down the page, and click on YES
option under Patient’s Photographs block. This will start your web-
camera and you can now click photograph of the patient.
Step 6: After Step 5, you will see below screen, when your web-cam is on.
Now, click on Capture Button.

Step 7: Upload Doctor’s Prescription – as per OPD slip
Step 8: Upload other reports and documents if required or available with
patient, as the case may be and click on Submit Button.

5. Patient Admission Form
After Pre Authorization Request is Approved by the competent authority (Insurance
Company/SHAA), the patient required to be admitted in the hospital, where MRD
number gets generated (IPD).
For the same, Kiosk User require to admit the patient in BSBY Application.
Next Form in process hierarchy is introduced for the same. So that, when a patient
gets admission in Hospital through Aarogya (IPD) at the same time parallelly that
patient gets admitted in BSBY scheme also.
Step 1: Enter TID number in the TID field and press Tab or click mouse outside
on form anywhere
Step 2: Select Date of Admission and Time of Admission

Step 3: Enter MRD No. as per Aarogya Slip in MRD field and press Tab or click
on form anywhere outside the text box.
Step 4: Click on Submit button to Admit the patient.
Step 5: Upon successful admission user will get below message

6. Patient Discharge Form
After a patient is admitted and medical treatment is completed, the patient needs
to be discharged from the hospital. Below steps take you through the same process
Step 1: Enter TID in the TID text field as shown below and press tab or click
mouse anywhere on form. Patient’s Admission details will come on
form from records automatically for that particular TID only.
Step 2: Select/enter Patient’s discharge date & time. Please note patient’s
discharge date cannot be an earlier date than that of admission date.

7. Claims Submission Form
After Patient is discharged from hospital. Kiosk User required to submit the Claim
immediately or at max with 7 days of discharge of patient.
For the same, refer to below snap-shot, where user required to fill the TID and then
upload the required documents one-by-one as the case may be.
After uploading the required documents, click on Submit Button.
