Bedsharing in context
• Bedsharing has become a controversial practice, despite its growing prevalence.
• There is little population-based U.S. data on who bedshares.
• This information would be useful to:– interpret the significance of case series and
anecdotal reports of SIDS deaths. – inform public policy.– guide clinical practice.– help to focus research.
Oregon PRAMS
“Oregon PRAMS, the Pregnancy Risk Assessment Monitoring System, is a project of the DHS Office of Family Health with support from the national Centers for Disease Control and Prevention (CDC). PRAMS collects data on maternal attitudes and experiences prior to, during, and immediately after pregnancy for a sample of Oregon women.”

Conclusions
• Bedsharing is very common in Oregon, as elsewhere.• Low income, single, breastfeeding mothers are more
likely to bedshare than others.• Black and Asian women are more likely to bedshare
than others, and this appears to be independent of income level. Hispanics are also more likely to bedshare but SES status is an influence.
• Non-economic, as well as economic, factors are important determinants of maternal-infant bedsharing.
• Bedsharing is not solely due to lack of access to cribs and provision of cribs will not eliminate bedsharing.
Published studies on SIDS & bedsharingSummary of studies
– 8 uncontrolled case series raising a question of risk– 9 large case control studies
• 3 did not report results stratified by maternal smoking status but none found an association between bedsharing & SIDS (1,2,3)
• 6 provided stratified results– 3 found no association between SIDS and bedsharing among
nonsmoking mothers; among smoking mothers, risk was increased (4,5,6)
– 1 found an increased risk for infants < 11 weeks old, adjusted for maternal smoking, but not > 11 weeks old; and an increased risk for smoking mothers but not nonsmokers (7)
– 2 found no association between SIDS and bedsharing among nonsmoking mothers of infants > 11 weeks old but did find a risk for infants < 8-11 weeks old and for all infants of smoking mothers (8,9)
SIDS and diagnostic drift
• There is good evidence of diagnostic drift– The decline in overall postneonatal mortality has been
less than that of SIDS mortality– Since 1999, SIDS mortality has continued to fall, while
postneonatal mortality has been flat and other causes of sudden unexpected death have risen
• The evidence regarding bedsharing is clear and consistent and unlikely to have been significantly affected by diagnostic drift
• Further studies must take this phenomenon into account
Crib provision programs
• Valuable public health campaign?• Culturally insensitive?• Supported by scientific evidence?• Bedsharing appears to be the result of combination
of economic, cultural and racial/ethnic factors • Follow up study in St. Louis found that less than
half of those given cribs were using them regularly
Recommendations
• The popularity of bedsharing must be considered when evaluating uncontrolled studies or anecdotal reports.
• The relative importance of cultural and economic factors in choice of bedsharing should be further explored.
• Bedsharing recommendations should be based on the best available scientific evidence, not prejudice or “common sense.”
Recommendations• Cultural values should not be imposed on diverse
groups, without good scientific evidence; bedsharing should be a matter of choice, unless and until there is scientific justification to discourage all bedsharing.– There is strong evidence that bedsharing by smoking mothers
is a significant risk for SIDS.– There is some evidence that bedsharing with infants < 3
months of age by nonsmoking mothers is a risk for SIDS and this practice should be discouraged based on current scientific evidence.
– There is no evidence that maternal bedsharing with infants > 3 months of age by nonsmoking mothers is a significant risk for SIDS.
– Cribs should be a choice and an option, not an obligation
