Securing a Social Case Summary Report (SCSR) ABOUT THE SERVICE

As one of the requirements in availing medical assistance from other government or non-government hospitals/agencies, the beneficiary/client is required to secure a Social Case Summary Report (SCSR) from the CSWDO.

Availing of Services for Persons With Disability (PWDs) (Issuance of ID Card) ABOUT THE SERVICE Special privileges and discounts are given to bonafide PWDs in the City in accordance with RA 9442. PWDs can avail of the benefits provided he/she must have a Privilege Identification Card. A PWD ID together with the Purchase Booklet should be presented when purchasing medicine and food commodities to avail of discounts. Other benefits provided are the following:

Availing of Services for Solo Parent ABOUT THE SERVICE

This service aims to develop a comprehensive package of social development and welfare services for solo parents and their children carried out by the DSWD as the lead agency, various government agencies including other related NGOs. The following benefits and privileges to solo parents and their children under Republic Act 8972 or the Solo Parent’s Welfare Act are as follows:

a. Livelihood Services d. Employment-Related Benefits b. Psychological Services e. Health Services c. Educational Assistance f. Housing Services

A solo parent should have these qualifications:

- A woman who gives birth as a result of rape and other crimes against chastity even without a final conviction of the offended.

Provided that the mother keeps and raises the child. - Parent left solo or alone with the responsibility of parenthood due to death of spouse. - Parent left solo or alone with the responsibility of parenthood while the spouse is detained or is serving sentence for a criminal

conviction for at least one (1) year. - Parents left solo or alone with the responsibility of parenthood due to physical and/or mental incapacity of spouse as certified

by a public medical practitioner. - Parent left solo or alone with the responsibility of parenthood due to legal separation or de facto separation from spouse for at

least one (1) year, as long as he/she is entrusted with the custody of the children. - Parent left solo or alone with the responsibility of parenthood due to declaration of nullity or annulment of marriage as declared

by a court or by a church as long as he/she is entrusted with the custody of the children. - Parent left solo or alone with the responsibility of parenthood due to abandonment of spouse for at least (1) year. - Unmarried father/mother who has preferred to keep and rear his/her child/children instead of having others to care for them or

give them up to a welfare institution. - Any other person who solely provides parental care/support to a child or children. - Any family member who assumes the responsibility of head of the family as a result of the death, abandonment,

disappearance, or prolonged absence of the parents or solo parent.

Availing of Senior Citizen ID Card ABOUT THE SERVICE Under RA 9257, a Senior Citizen shall avail of the various benefits and privileges provided he/she must have a Senior Citizen Identification Card. A Senior Citizen ID Card together with a purchase Booklet should be presented when purchasing medicine, food commodities and other items to avail of discounts. These benefits are the following:

Availing of Services for Disaster Victims ABOUT THE SERVICE THE CITY SOCIAL Welfare and Development Office (CSWDO) pro-actively provides assistance to individual and families who are victims of natural or man-made calamities such as typhoons, floods, fire and etc. The program provides services for the following: - Provision of relief assistance - Provision of financial/housing materials for the restoration of damaged/destroyed houses

- Counseling/stress debriefing for individuals and families who have been emotionally and psychologically affected by the disaster - Food for work

The Office also facilitates the provision of financial assistance from national government offices (e.g. Department of Social Welfare and Development, Office of Defense) and other agencies to victims.

Securing Services for Pre-Marriage Counseling (PMC) ABOUT THE SERVICE A half day orientation for couples applying for marriage license has been instituted and designed to provide pre-marriage couples with a realistic overview of what marriage is all about. Presidential Decree 965, issued on July 20, 1976 requires all applicants for marriage license who need parental advise (aged 21-25) shall attach a certificate of PMC to their applications for marriage license.

Securing a Phil-Health Membership

ABOUT THE SERVICE

Indigent Program or “Medicare Para sa Masa” refers to the Indigent Sector Component of National Health Insurance Program (NHIP). It is the tool by which the State envisions to provide Medicare privileges to the indigent population, as a supplement to its Medicare Program for the employees and individually paying sectors. Individual who wants to be a member shall have the following qualification: He/she must be Indigent, No SSS/GSIS, No pension and Sickly/Disabled
